Job Responsibilities:
Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment
Assesses for muscle strength, mobility, gait, ROM -- potential for rebab
Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences
Identifies patient and family/caregiver needs for other home health services and refers as necessary
Qualifications:
Valid PT License to Practice in CA
Current BLS/CPR certification
Valid CA Driver's license
Minimum of one year Physical Therapy experience is (required/preferred)

The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. Providing personal care including
Baths
Back rubs
Oral hygiene
Shampoos
Changing bed linen
Assisting patients with dressing and undressing
Skin care to prevent breakdown
Assisting the patient with toileting activities
Keeping patient's living area clean and orderly, as appropriate
Planning and preparing nutritious meals
Assisting in feeding the patient, if necessary
