On-Call: Participation in the on-call rotation is a requirement of this position. It is anticipated that an incumbent in the position would be on-call approx. once every eight (8) weeks for two (2) weekdays per month and one (1) weekend. The Per Diem Registered Dietician collaborates with Upstate Home Care clinical teams to ensure optimal nutrition support for patients receiving enteral or parenteral therapy. This role supports qualification of TPN patients, secures appropriate clinical documentation, ensures compliance with insurance guidelines, and provides clinical expertise as needed. Additional responsibilities include assisting with nutrition support services, maintaining formularies, contributing to policy and procedure development, participating in staff education, and supporting reimbursement efforts related to nutrition therapy. QUALIFICATION REQUIREMENTS Bachelor's degree in nutrition or related field. Minimum two years of professional nutrition experience. Minimum one year of experience in a home care setting. Current, active registration as a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR). Current, valid state licensure or certification to practice dietetics. Certified Nutrition Support Clinician (CNSC) through the National Board of Nutrition Support Certification, Inc.
