Here Is What You Will Be Doing:
Completes nursing assessment of patient and documents appropriately into patient chart.
Develops and reviews and revises a plan of care, including a discharge plan, based upon the assessment of patient's needs.
Implements medical and nursing orders per plan of care and documents appropriately.
Evaluates patient's response to care and engages in patient and family teaching as appropriate.
Oversees staff (LPN, CNA, and PSA) assigned for providing patient care.
Participates in Bedside Shift Report.
Collaborates with the Clinical NurseManager to assure the integration of the ANA's Scope & Standards of Practice, assuring that these standards are identified as the measurement criteria for nursing practice on the unit.
Coordinates patient care in collaboration with providers and ancillary departments to achieve quality outcomes.
Completes Unit specific orientation and ongoing training and competencies as assigned based on department role, scope of services, and organizational standards.
Desired Skills:
Demonstrates effective leadership and professional/clinical skills according to Standards of Care, to include drug calculations.
Ability to assess a patient's condition
Ability to utilize critical thinking to formulate plan of care, select appropriate interventions, and evaluate patient's response to plan of care and problem solve as needed.
Ability to explain to patient/family the nursing process and how it helps meet the patients' needs and improve patient outcomes.
Verbal, written, and interpersonal communication skills

Cheyenne Regional AHA RQI within 14 calendar days of start date
Six (6) months or more of nursing experience required (12 months preferred)

REWARDING WORK YOU WILL DO:
Oversees and coordinates patient and family care in accordance with hospice philosophy and delivery of care. Provides skilled nursing, including intervention, documentation, supportive care and assessments for assigned patients. The RN CaseManager is a member of the Interdisciplinary Team/Group (IDT/IDG).
Develops and oversees the care plan with patient and family/caregiver, primary physician and members of the IDT/IDG. Reviews and evaluates the patient's Plan of Care (POC) in IDT/IDG meetings on a regularly scheduled basis. Approaches patient and family care in a holistic manner.
Coordinates patient care with the physician, NP and all members of the Interdisciplinary Team through ongoing communication and appraisal of patient's changing condition. Refers to appropriate team member for assistance as needed.
Ensures the educational needs of the patient and family/caregiver are met in order to promote optimal care.
Provides continuous assessment and monitoring of the physical, emotional and spiritual needs of the patient.
Performs complex telephone interviews and triages as appropriate.
Evaluates the appropriate time to coordinate transfer of patient from home hospice to inpatient care.
Supervises Certified Nurse Aides (CNA); delegates specific tasks per agency policy.
Participates in related services, such as community education, team training, staff/board committees and orientation of employees.
Responsible for meeting visit frequency, case load and/or visits per shift requirements; takes on-call rotation as needed and/or assigned.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
Minimum Experience: One-Two (1-2) years of experience in hospice/palliative care/home health, medical surgical, public health, oncology, acute care or home health.
Required License: Current Colorado RN license. Current CPR certification. Valid Colorado state driver's license and proof of auto insurance.
Required Travel: This position requires daily travel.

Minimum Qualifications

Basic Life Support (BLS) certification.

The Role
The RN in Pre-Op/PACU provides expert professional nursing care during the pre- and post-operative course. The RN assesses, implements and evaluates the care of the patient. The nurse will assure that the patient is prepared for discharge to a home environment by providing the necessary education to the patient and their responsible caregiver.
Major Responsibilities:
Works with members of the health care team to provide optimal care to the patient, throughout their surgical experience.
Must be available to work 3 shifts per month. Flexible Monday - Friday.
Must be available to work (1) Summer Holiday Shift - day before or after the Holiday (Memorial Day, July 4th, Labor Day) and (1) Winter Shift - day before or after the holiday (Thanksgiving, Christmas, New Year's).
Attends all Mandatory Staff Meetings.
Maintains a friendly, positive and caring attitude with all patients, physicians and staff.
Assists in patient education during all phases of their experience in the ASC.
Assists in assuring that the equipment is functioning properly.
Monitors the use of all supplies during patient stay. Assist with inventory control and stock accordingly.
Documents appropriately the care that is given to the patient.
Maintains continuing education to meet state requirements and keep current with health care.
Displays professionalism at work and in the community, as a representative of the Eye Center.
Identifies actual and potential risks to patients and others, and takes action to minimize or eliminate risk and maximize safety.
Assists in Pre-Op/Post-op Phone-calls.
Communicates with other health care team members to assure continuity of care in the ASC.
Credentialed to receive all patients in PACU including MACs, pediatric and adult generals after surgery. Assesses, evaluates, and documents on the patient. Monitors the patient continuously during their recovery phase. Provides discharge instructions to the patient and their caregiver, verifying confirmation prior to discharge.
Incorporates a sense of wellness by allowing the patient's family to be reunited with them as soon as feasible and encouraging self-care by the patient.
Competently uses NextGen.
Prepares for emergency situations by knowing the Policies and Procedures of the ASC.
Monitors that Universal Precautions are maintained.
Completes all necessary documentation on the Nursing Notes record and all patient records.
Wipes down gurneys, recliners and medical equipment between patients with a germicidal solution. Remakes beds as needed.
Accept other duties as assigned.

Two years of pre-operative, recovery room, intensive care or emergency room experience is desired.

REWARDING WORK YOU WILL DO:
Provides patient care using nursing skills and processes in accordance with applicable scope and standards of practice, keeping within the policies, values and mission of the organization. The RN is a member of the Interdisciplinary Team/Group (IDT/IDG).
Provides skilled nursing coordination of care to patients (neonates to geriatrics) as determined by the Plan of Care (POC). Uses education, mentoring, guidance and interventions to achieve goals of care.
Collaborates across disciplines to coordinate patient care; participates in the POC in IDT/IDG meetings as assigned.
Demonstrates appropriate nursing care, including communicating with attending provider for medications and supplies.
Educates patients, families, caregivers and community regarding hospice philosophy and palliative treatments or home health, if applicable.
Supervises Certified Nurse Aides (CNA); delegates specific tasks per agency policy.
Participates in related services, such as community education, team training, staff/board committees and orientation of employees.
WHAT WE ARE GOING TO LOVE ABOUT YOU:
Minimum Education: Graduate of an accredited school of nursing; BSN preferred.
Minimum Experience: One to two (1-2) years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.
Required License: Current Colorado RN license. Valid Colorado state driver's license and proof of auto insurance.
Current CPR certification. CPR must be provided and/or obtained within 14 days of hire.
PHYSICAL REQUIREMENTS:
Ability to lift/push/pull minimum of 35 lbs.

Care Synergy provides mission support services to better equip Pathways Hospice to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
