The RNCM- bilig partners closely with Clinical Operations, Care Team Operations, Lead Care Managers, Community Health Workers, Behavioral Health providers, Compliance, and external medical and social service partners. Collaboration occurs daily to support assessments, care planning, escalations, transitions of care, and member outcomes.
Responsibilities
Conduct comprehensive clinical assessments (including medical history, risk factors, and medication review) and develop person-centered care plans with SMART goals based on medical, behavioral, and social needs.
Provide medication reconciliation, health education, and condition-specific teaching to strengthen member understanding, self-management, and adherence.
Collaborate with Lead Care Managers, CHWs, Behavioral Health, Housing Navigation, providers, and community partners to coordinate services and resolve medical and social barriers.
Triage clinical concerns, identify red flags, and escalate appropriately to NP/MD partners; provide brief interventions within RN scope as needed.
Participate in interdisciplinary Systematic Case Reviews (SCR), IDT meetings, and case conferences, offering clinical recommendations and follow-up planning.
Coordinate transitions of care (TOC) by supporting post-hospital follow-ups, reconciling medications, scheduling timely appointments, and ensuring continuity.
Maintain accurate, timely, audit-ready documentation in ECW, Google Suite, and payer/health plan portals; ensure all clinical assessments, screenings, and care plans meet required timelines.
Engage members through relationship-based and trauma-informed approaches, building trust with individuals who may struggle with traditional healthcare systems.
Identify gaps in care, clinical risk, or environmental barriers and collaborate with care teams to implement timely interventions.
Support HHN's startup model by adapting to evolving workflows, contributing to clinical process improvements, and helping build scalable care coordination practices.
Skills Required
Strong clinical assessment, triage, and critical-thinking skills, bilingual speaking, writing.
Expertise in care planning, chronic disease management, and clinical documentation.
Proficiency with eClinicalWorks (ECW), Google Suite, RingCentral, and payer/health plan portals.

Position Summary:
The Gary and Mary West PACE (GMWP) Assessment RN is responsible for conducting initial nursing and home safety assessments of prospective participants at their place of residence and managing State level-of-care (LOC) review process with Department of Health Care Services nurse. The Assessment RN is a member of the Gary and Mary West PACE Clinic and Marketing Team and works in close collaboration with the PACE IDT team, marketing specialist, and prospects' caregivers to thoroughly and promptly evaluate each prospective participant interested in PACE enrollment.
Essential Functions
•Conduct initial nursing assessments, including but not limited to nursing evaluation, functional assessment, mini mental status, home safety evaluation, in prospective Gary and Mary West PACE participants' homes, to determine if they meet PACE eligibility requirements.
•Promptly prepare documentation from home visits, submit to CA State Level-of-Care (LOC) nurse for review and respond to questions to expedite review process for participants.
•Track and manage the annual State level-of-care review process.
• Comply with the Marketing Team's compliance with all rules pertaining to allowable and prohibited marketing practices.
•Cross-train on other Marketing Team roles and provide back coverage as directed.
•Provide clinical coverage and backup to the nursing team, at clinic, home or alternative care sites including RCFE, ADHC and Congregate living communities.
•Deliver and document nursing interventions as agreed upon in the participants' care plans including but not limited to maintaining a healthy and safe environment, promptly and accurately responding to physician orders, and correctly administering medications and performing ordered tests and treatments.
•Collaborate with the Gary and Mary West PACE IDT and Marketing Team members.
•Complete all required documentation in a timely and accurate manner, including in CRM software, Salesforce.
•Protect privacy and maintain confidentiality of all company procedures and information. about employees, participants and families.
•Follow Gary and Mary West PACE policies and procedures and participate in any required Quality. Improvement activities, staff training and meetings.
•Other duties as assigned.
Education and Certification:
•Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
•Unencumbered CaliforniaRegisteredNurse (RN) License
•Current Basic Life Support (BLS)/ First Aide Certification required
•Original valid driver's license with proof of auto insurance.
Skills and Experience:
•Minimum of two years nursing experience in a clinical setting with a frail or elderly population.

Coordinates and monitors patients through the hospitalization process utilizing physicians, primary care nurses, and third party payors to maintain quality care and fiscal responsibility.
• Assists the admitting department in identifying and resolving problems regarding appropriate admissions.
• Acts as a resource/advisor to physicians for adequate medical record documentation, appropriateness of facility services as they relate to diagnoses, and options for post-discharge care.
• Reviews the post hospital care plan with the patient/family. Establishes a contract regarding time frames and responsibilities. Follows plan through to discharge.
• Utilizes community services to provide optimal discharge planning, monitors/expedites timely completion of services to avoid delays in care or discharge, facilitate third party reimbursement approval and/or authorization, conducts post discharge activities and documentation, responsible for completion and interpretation of patient statistical reporting.
• Coordinates with multi-disciplines to include Dietary, Physical Therapy, Occupational Therapy, Speech, Social Services, Infection Control, etc., to insure efficient use of hospital resources.
• Answers questions from patients and their families, including providers regarding reimbursement of covered and non-covered services.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
The RegisteredNurseCaseManager completes an initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient's status by:
Writing and initiating plan of care
Regularly re-evaluating patient and family/caregiver needs
Participating in revising the plan of care as necessary
The RegisteredNurseCaseManager uses health assessment data to determine nursing diagnosis.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
Acts as CaseManager and assumes responsibility to coordinate patient care for assigned caseload.
Communication
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
Communicates with community health related persons to coordinate the care plan.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.
The RegisteredNurseCaseManager provides and maintains a safe environment for the patient.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.
Attends interdisciplinary group meetings.
Additional Duties
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services
are available.
Supervises ancillary personnel and delegates responsibilities when required.

POSITION QUALIFICATIONS
Registerednurse with current licensure to practice professional nursing in the state of California.
Graduate of National League for Nursing accredited school of nursing.
Maintains a current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Registerednurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. Minimum of two years' experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.

Supervise and manage clinical staff, ensuring adherence to CMS, CHAP, and state/national practice standards.
Conduct initial and ongoing comprehensive patient assessments.
Develop, implement, and revise care plans tailored to individual patient needs.
Provide physical therapy services requiring advanced clinical expertise.
Oversee compliance with clinical documentation standards using the HomeCare HomeBase (HCHB) system.
Support and mentor team members, fostering professional development and a collaborative work environment.
Coordinate care with physicians and interdisciplinary team members.
Participate in on-call rotation, providing guidance and support during off-hours as needed.
Conduct patient and family education on therapies and techniques to support recovery and independence.
Ensure optimal resource utilization and maintain high levels of patient satisfaction.
Qualifications
Licensed RegisteredNurse in the state of California.
A minimum of two years of recent clinical experience, preferably in Home Health or Rehabilitation settings.
Proven leadership or management experience is highly desirable.
Proficiency in clinical documentation and electronic health record systems (HCHB experience preferred).
Strong organizational, interpersonal, and communication skills.
Licensed driver with a reliable, insured vehicle.
