The Clinical Case Manager & Therapeutic Coach is the main change agent for clients and families who benefit from the broad range of Active Recovery Network's services. This position teaches hard and soft life skills and instills behaviors that will empower clients to drive their own growth and personal development toward a life of satisfaction, engagement, joy, and mastery. This position fosters mental health and addiction recovery behaviors and utilizes the community as a resource and views it as the healing milieu. Must love working independently, be self-directed, and be a team player. Position requires driving to and from client homes in San Diego County (central San Diego to North County San Diego). Key Responsibilities Include: · Direct work with clients and families working to manage and/or overcome mental health and addiction issues · Utilize assessments, self-assessments and ARN curriculum to map out client wellness and recovery plan and serve as an accountability partner and coach · Hold clear boundaries and communicate directly and effectively with clients and parents regarding the change and growth process · Develop strong, healthy relationships with clients in order to support and challenge them to do the tough work of changing and thriving · Teach and model hard and soft life skills · Maintain fluency in supportive clinical and community resources and develop strong working relationships with community treatment and clinical service providers · Coach parents in a way that supports change and growth for the entire family system · Exhibit high levels of self-awareness by continually assessing performance, identifying challenges and needs, and developing areas of weakness · Maintain complete and accurate records · Model a growth mentality and be doing your own growth work in parallel with the client's and family's work · Contribute to the mission and vision of the company Qualifications Required: · A Master's Degree in Psychology, Social Work or related field. · 3-5 years experience working in a clinical and/or coaching environment within the drug and alcohol treatment and/or mental health treatment industry. · Knowledge of and fluency in the use of psychological tools, recovery resources, insight into motivation and behavior, understanding of the role of coaching as a change agent. · Clinical license is a plus but are not mandatory. · Clinical experience PLUS coaching and leadership experience a significant plus. · The ideal candidate is self-motivated, coachable, trustworthy, ethical and works well both as a team member and independently. *We do not provide clinical supervision hours toward licensure.     Job Description ACT Supported Living Services to adults with developmental disabilities who live in their own homes or share an apartment with a live-in caregiver. ACT's services are not medical in nature, but instead are focused on caregiving/ ADLs. Some clients of ACT have higher medical needs and qualify for hours for a Nurse Consultant. This role provides in-home consultation(reliable transportation is a requirement) for clients and support to staff for basic medical care (medication management, after-care procedures, checking and reviewing vitals). A nurse assessment is generally done once a month per client with follow up visits weekly (the hours available per client are determined by the San Diego Regional Center- some clients have 3 hour per week, while others have 2 per month, all depending on their medical needs). Nurse Consultant provides notes of assessments, examinations, and any developing concerns for ACT staff to follow up with the client's medical providers. The role is not to be the provider for the client, but to help ensure that ACT staff are not missing any developing concerns, are trained to address the concerns present, and provide questions, concerns, or requests for staff to communicate to medical providers. This is a highly part-time position, currently we have 4 clients who receive Nurse Consultant hours, with a total of 22 hours per month. We can be highly flexible for scheduling to accommodate any outside work/ schedule you may have. Mileage for travel is reimbursed.
