Minimum of 2-3 years' experience in family support, grief counseling, or a related field, preferably in healthcare or organ donation settings. Backgrounds that demonstrate advanced interpersonal communication, conflict resolution, and the ability to guide individuals through complex, emotionally charged decisions Knowledge of the organ donation process and relevant regulatory frameworks is highly desirable. Ability to navigate sensitive and complex situations with professionalism and poise. Proficient in Microsoft Office Suite and experience with data management systems. Job Description Assesses psychosocial, emotional, and / or economical factors affecting patient's limitations and potential for and / or lack of improvement. Assesses patient's needs for long term care including home and family situation, exploring alternatives to in home care arrangement for placement. Counsels patient, caregiver regarding long term planning and decision making. Collaborates with physician and other member of health team to develop a Plan of care. Communicates significant information regarding patients with other members of health care team and makes necessary recommendation, suggestion to improve the Plan of care. Clinical notes should be written each visit and include progress, or lack of, specific plans, goals, anticipated length of service, referrals made, and follow-up on referrals. Submits initial evaluation visit, interim evaluations (as deemed necessary) and discharge evaluation from Social Services. These are required documentation. Identifies high risk indicators potentially endangering patients and provides intervention reports of the situation to the Director of Nurses and proper authorities.

Under supervision of the Director of Case Management, assists in the integration of Social Service functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies and healthcare facilities. Provides resources and services to assigned patients as appropriate. Ensures compliance with quality patient care and regulatory standards. The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies.
