    Under supervision of the Director of Case Management, assists in the integration of Social Service functions into the patient care, discharge and home planning processes with other hospital departments, external service organizations, agencies and healthcare facilities. Provides resources and services to assigned patients as appropriate. Ensures compliance with quality patient care and regulatory standards. The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. In addition, offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. This role will receive referrals for individuals from at-risk populations from interdisciplinary team member (including physicians, Case Managers, staff nurses and other members of the care team). Performs psychosocial assessment and interventions. Intervenes with patients and families regarding emotional, social, and financial consequences of illness and/or disability; accesses and mobilizes family/community resources to meet identified needs. Provides intervention in cases involving child abuse/neglect, guardianship (temporary/permanent), foster care, adoption, mental health placement, advance directive, domestic violence, elderly abuse/neglect, institutional abuse and sexual assault. Serves as a resource and provides counseling and intervention related to treatment decisions and end-of-life issues. Assists indigent patients in securing prescribed medications, transportation, and equipment using available community resources. Advocates for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system. Participates in discharge planning activities for complex patients, in order to ensure a timely discharge and to provide appropriate linkage with post-discharge care providers. Ensures safe care to patients adhering to policies, procedures and standards within budgetary specifications: including; time management, supply management, productivity, and accuracy of practice. Promotes individual professional growth and development by meeting requirements for mandatory/continuing education, skills competency, supports department-based goals which contribute to the success of the organization. Serves as preceptor, mentor, and resource to less experience staff. Demonstrates adaptability, problem solving and professional behavior at all times. Abides by the HMH Legal Compliance Code of Conduct. Maintains patient confidentiality and appropriate handling of PHI. Maintains a safe work environment and reports safety concerns appropriately. Performs all other related duties as assigned Requirements Education: Master's degree in Social Work required. Experience: One year of social work experience preferred. Licensure/Certification: Current Licensed Master Social Worker in the State of Texas, or immediately eligible to obtain within one year of employment. The Crisis Intervention Specialist works closely with the principals, counselors, students, parents, community members, and other concerned individuals to ensure that identified Communities In Schools of Southeast Harris and Brazoria County programs are provided. ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING: Coordinates services for identified students and families during the school day and possibly before and after school. Interview students and/or parents and/or guardians with problems such as personal and family adjustments, abuse, criminal conduct, finances, employment, food, clothing, housing, and physical and mental impairments to determine the nature and degree of the problem. Secures information such as medical, psychological, and social factors contributing to a student's situation and evaluates these and the student's capacities. Counsels students individually, in family, or in other small groups regarding plans for meeting needs and aids students to mobilize inner capacities and environmental resources to improve social functioning. Helps students to modify attitudes and patterns of behavior by increasing understanding of self, personal issues Maintains working knowledge of community resources and agencies and connects the appropriate resources to individual students' and families' needs Refers and follows up with student and/or parent and/or guardian to community resources and other organizations. Compiles records and prepares reports, meeting required deadlines in a timely manner. Review the service action plan and perform follow-up inquiries to determine the quantity and quality of services provided to the student and the status of the student's case. Accesses and records student and community resource information. Secures supplementary information such as free and reduced lunch status, discipline reports, grades, and other school data reports necessary to assess the student's needs accurately. Actively involve volunteers in their areas of interest and expertise. Work closely with principal(s) and counselor(s) to effectively structure programs, meeting the needs of the individual campus setting. When required, determines student's eligibility for financial assistance. Effectively meeting the objective of the CIS-Crisis Intervention program through individual mentoring and/or counseling and/or organized group sessions. Organizing and maintaining student case files in compliance with the Perfect File Checklist as outlined by CIS. Plan, organize, and implement program design to meet the needs of the students and families effectively. Relate, motivate, and encourage students, parents/guardians and/or administrators, faculty, and staff. Have knowledge of Southeast Harris and/or Brazoria County resources. Must be able to interact positively with a diverse group of individuals of the school, city, and state entities. Compile and submit reports and information in a timely manner. Exhibit excellent work attitude, strong work ethic, and self-motivation. Be responsible for program planning and implementation of objectives. Facilitate, develop, and/or conduct workshops as deemed necessary by the Chief Executive Officer, Chief Operating Officer, and/or Program Director. (See organizational chart) EDUCATION Masters Degree is strongly preferred in Social Work, Psychology, Sociology, or a related field. Complete all necessary arrangements for transfer of care from one health care setting to the next after the rehabilitation care phase. Execute DC plans that include home health referrals, durable medical equipment (DME) and identification of other social services needs 8. Reconcile services rendered with patient benefits and communicate with physicians and third party payers to ensure appropriate services and reimbursement 9. Identify when services rendered no longer meet reimbursement guidelines and apprise &/or initiate denial of benefits process 10. Comply with all required standards for medical/legal reporting situations (elder abuse, commitment, guardianship, etc.) 11. Ensure consistent communication with all team members throughout the care continuum, including patient, families and other health care providers in acute settings.
