Participates in established intra-departmental and inter-departmental communication systems and methods to achieve departmental and hospital goals.

Maintains required records/documentations as specified in department policy and procedures manual.
Participates in medical chart peer review, case records review and utilization review as deemed necessary by management.
Performs all other duties as assigned.

Responsibilities
Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. Design and implement engaging, evidence-based exercises and activities tailored to each patient's communication goals
Provide counseling, education, and support to patients and their families throughout the treatment process
Establish measurable goals and track patient progress, identifying both improvements and ongoing challenges
Ensure proper setup, use, and sterilization of speech therapy equipment in accordance with safety protocols
Review patient records prior to sessions to develop a clear, personalized treatment approach
Participate in continuing education to maintain licensure and stay current with advancements in speech-language pathology

Requirements:
Certificate of Clinical Competence (CCC-SLP) and active license to practice in the state of Texas
Master's degree in Speech-Language Pathology from an accredited program
Proficient with clinical documentation systems and related software used for reporting and patient records
Solid understanding of and adherence to HIPAA regulations and patient privacy laws
Experience managing confidential and sensitive patient information with discretion and professionalism
Comfortable troubleshooting basic technical issues related to speech therapy tools and digital platforms
Must be able to perform duties primarily in a standing position
Friendly, professional demeanor with strong communication skills, especially when explaining procedures and treatment plans

As a Speech Language Pathologist you will assess, diagnose, and treat speech, language, cognitive, communication, voice, swallowing, fluency, and other related disorders while integrating company values into daily practice.
This position will work PRN (as needed) Day shifts.
Essential job functions include but are not limited to:
Responsible for administration and direct application of Speech Language Pathology treatment modalities as prescribed by a licensed physician performing patient assessments, establishing plan of care, and preparing discharge plans.
Assesses educational and cultural needs of the patient, family, caregiver, or significant other and communicate accordingly to promote maximum benefit of care.
Coordinates with interdisciplinary team on patient needs, dietary requirements, treatment modifications, and progress.
Completes patient documentation in a thorough, timely, and efficient manner.
Provides an environment conducive to safety for patients, visitors, and staff. Assesses the risks for safety and implements appropriate precautions. Complies with appropriate and approved safety and Infection Prevention standards.
Requirements for consideration:
While we are open to recent graduates, two (2) years of direct patient care experience in rehabilitation hospital setting is preferred.
Current state licensure in Speech Language Pathology.
Certificate of Clinical Competence from ASHA or Clinical Fellowship Year status or equivalent preferred.
Basic Life Support/CPR card required.
Physical Requirements Over the Course of a Shift:
A significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time.
Both gross and precise motor functions.
Lifting/exerting of up to 50 lbs.
Possible exposure to bodily fluids.
Visual acuity required for patient assessment and documentation of care.
Acute hearing required for accurate patient assessment.
Sufficient manual dexterity to operate equipment and computer keyboard.
Close vision and the ability to adjust focus.
