Provides services within the scope of practice as defined by the state laws governing the practice of speech therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
Evaluates the patient's level of function by applying diagnostic and prognostic functional ability tests. Assists the physician in the development of the therapy plan of care.
Treats patients to communicate effectively by expressing thoughts according to the patient's condition using acceptable standards of practice.

Location: This position will perform patient home visits in the Palm Coast to Ormond Beach area and surrounding areas.
The role you'll contribute:
Performs evaluation of the homecare patient to determine the type of speech or language disorder and the appropriate corrective therapy; provides rehabilitative services for swallowing, speech and language disorders; records activities and findings in the clinical record, submits reports to the physician regarding changes in the patient's condition or deviations from the plan of care; instructs other health team personnel and caregivers in methods of assisting the patient to improve and correct speech disabilities.

Performs an evaluation and utilizes the findings to determine diagnosis (for SLP services), prognosis and plan of care in consultation with the physician. Determines specific interventions, duration and frequency needed to achieve anticipated goals and outcomes.
Observes and records activities and findings in the clinical record and reports to the physician the patient's reaction to treatment and any changes in patient's condition, or when there are deviations from the plan of care.
Instructs the patient and caregiver regarding the patient's speechlanguage improvement program including adaptive equipment and the home program.
Instructs other health team personnel including, when appropriate, home health aides and caregivers in certain phases of speech therapy with which they may work with the patient.
Selects, applies or modifies skilled intervention to the patient using various skilled interventions consistent with the state practice act to achieve the best outcome possible for the patient. Assesses patient response to interventions and performs reassessments as required.
Documents in medical record, accurately reflecting treatment provided. Updates the care plan as needed including the aide care plan; writes modified orders for changes in the plan of care, performs and documents the aide supervision by the due date.
Completes all documentation within the time frame required by the agency, including reassessments, discharge summaries and OASIS documents.
Participates in clinical record review, agency staff meetings, therapy meetings and other activities as requested.
The expertise and experiences you'll need to succeed:
Minimum qualifications:
Masters level graduate of a SpeechLanguage Pathology program
Current state of FloridaSpeech/Language Pathology license
Valid in state Driver's License with current car insurance
Active American Heart Association BLS
Preferred qualifications:
At least 2 years of clinical experience as a speechlanguagepathologist
Experience working with adult and geriatric rehabilitation cases
Home health experience within the past 3 years
Current knowledge of homecare requirements and OASIS assessments

Essential Functions and Responsibilities:
Provides a comprehensive evaluation of communication skills and/or swallowing potential.
Provides direct patient care to geriatric patients.
Evaluates patient's abilities.
Develops, plans and implements treatment based on evaluations to develop and improve the patient's communication and/or swallowing abilities.
Interprets results of patient's treatment program and maintains appropriate records of patient's progress.
Instructs and counsels patients and family members on the resident specific care program.
