ESSENTIAL RESPONSIBILITIES
Demonstrates both autonomy and proficiency in performance of comprehensive health assessments.
Collaborates with member's primary care physician to create a care plan focused on prevention, early detection and timely intervention.
Observes the scope of practice in the state in which he or she is licensed and working in.
Communicates with physicians and family members during transitions of care.
Effectively communicates with all members of the care team and facilitates a medically appropriate and effective plan of care to avoid unnecessary duplication of testing, and fragmentation of care.
Participates in case rounds to review skilled and hospitalized members.
REQUIRED PROFICIENCIES
Experience in skilled nursing or long-term care settings required (will consider newer NP grads if candidate has a strong RN background in long term care or a skilled nursing setting)
Case management experience preferred
Knowledge of current standard medical practices and experience with Medicare/Medicaid.
Knowledge of I-SNP MOC and I-SNP population.
LICENSURE, CREDENTIALING AND CERTIFICATION
Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice.
Basic Cardiac Life Support certification.
Holds or is eligible for DEA licensure and CDS licensure (if required in the state of
practice)
Current unrestricted RN license for the state in which the provider is practicing.

You will confirm service times with the customer in your tech portal. Services will be completed at the customer site (residential and commercial)
You will pick up parts daily from the assigned Fed Ex or UPS store for your territory before going onsite. Once all calls are completed for the day drop off the old parts with the pre-paid label that is provided.

Job Summary:
Unlocking The Spectrum is currently seeking talented ABA Therapists with RBT status to implement programs for children with autism using the principles of Applied Behavior Analysis. Candidates should be interested in working within a center, home and/or community-based model. Primary duties will include implementing behavioral programs addressing both behaviors to increase and decrease, including assisting child with daily self-help skills. The ideal candidate must be able to multi-task and use sound judgement at all times.

Responsibilities:
Implement treatment goals under the direction of the Behavior Analysts (BCBA).
Provide 1:1 therapy to children with autism.
Document goal and program information/data collection throughout the day.
Provide feedback regarding client progress to program supervisor.
Teach communication, behavior, self-help, and social skills as stated in the treatment plan.
Help children with autism and other special needs to meet their goals using proven ABA methods in a variety of settings, such as the center, school, home, or community.

Job Title: Head Tiger Sharks Swim Team Coach
FLSA: Part-time
Reports to Aquatics Director
Coach beginner, intermediate, advanced and middle/high school age groups within the Tiger Sharks competitive swim team.
Manage team practice and competition.
Part-time staff should work a minimum of fifteen hours per month on a consistent basis, or instruct a class a minimum of twice a week on a consistent basis.
ESSENTIAL FUNCTIONS:
Develop, promote, and manage the Monroe County YMCA competitive swim team in an efficient and fun manner consistent with YMCA standards.
Effectively operate swim team as budgeted and approved.
Develop a season plan to prepare swimmers for the physical and emotional growth and write and maintain daily practice plans to accomplish season plans. Maintain practice group attendance records.
Supervise pool and branch facilities for upkeep, cleanliness, and proper usage during swim team practices or other swim team usage. Ensure pool deck/facility is clean and equipment in its proper place when practice is over.
Inspect the facilities and equipment within the department to ensure high standards of maintenance, cleanliness, and repair of equipment.
Understand and implement all emergency procedures.

WHAT YOU WILL DO
Communicate injuries and treatment plans to coaches, athletes, parents, team physicians, and the Sports Medicine Institute Staff.
Complete thorough and accurate evaluation of athletic injuries, provides first aid and immediate care of acute injuries.
Develop, implement, and oversee appropriate rehabilitation and return to play programs for athletes.
Maintain proper documentation and record keeping of all athletic injuries, and related referrals and doctors notes.
Participate in Franciscan Health Sports Medicine educational outreach programs for local schools, youth sports organizations, parent, and community groups to promote conditioning and injury prevention programs.
Promote prevention of injuries through the recommendation of appropriate conditioning programs, monitoring environmental conditions and maintaining protective equipment.
Treat injuries using appropriate treatment modalities within the scope of athletic training certification and state practice act.
Work within established Sports Medicine and their current assignment's athlete care protocol regarding physician referrals, treatment, and return-to-play.
