The role
The RN Case Manager provides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs hospice aide staff in the provision of personal care to the patient. As a RN Case Manager you will:
Key Responsibilities:
Under the physician's order, admit patients eligible for hospice services
Assess and evaluates patient needs/problems, identifies mutually agreed upon goals with patients
Report patient status and need for other disciplines to clinical leadership, attending physician and hospice physician
Update care plans on an ongoing basis; revise and resolve patient problems and goals as changes occur and/or recertification
Complete informational visit and obtain patient consents for hospice admission per office procedure
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Skills and Experience Required:
Current unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
Must maintain a valid driver's license and good driving record
Ability to work in a field setting and exhibited ability to make sound nursing judgments
Ability to assess patient needs and formulate individualized patient care plans to meet those needs

We are hiring a Triage Nurse on the night shift at OCHN's Resource and Crisis Center in Pontiac , MI. This position works under the direct supervision of the Nursing Supervisor. The Triage RN is responsible for delivering high-quality, trauma-informed, and person-centered care. Operating in a fast-paced crisis setting, the RN plays a critical role in supporting assessment, safety, and recovery. NOTE: This position will work 7PM - 7:30 AM (Night Shift) The schedule is 3, 12-hour shifts per week, including every other weekend.
Essential Functions
Conduct initial nursing assessments and behavioral health triages for walk-in individuals to evaluate presenting concerns, acuity, and immediate safety needs.
Prioritize and triage individuals based on clinical urgency, risk factors, and level of care required.
Collaborate with the Crisis Assessment Team to determine appropriate interventions, referrals, or dispositions.
Participate in shift change reports and communicate pertinent updates to team members and leadership.
Maintain accurate, timely, and compliant nursing documentation in accordance with agency policies and regulatory requirements.
Support de-escalation and crisis intervention efforts to ensure the safety and stabilization of individual and staff.
Monitor individuals in the walk-in center for changes in medical or psychiatric status, responding promptly to emergent needs.
Ensure adherence to clinical protocols, infection control procedures, and best practice standards.
Assist in coordinating care transitions, including admissions to higher levels of care or community-based follow-up.
Provide education, reassurance, and support to clients and families during crisis stabilization.

Job Summary
The Triage RN provides clinical assessment and guidance to patients via incoming calls transferred from the call center. They will also assist with patient clinical questions/concerns that come in via the phones or in-person from waiting room, patient emergencies in-office, and call EMS/give report if needed. This role uses evidence-based protocols and professional nursing judgment to assess symptoms, determine urgency, and direct patients to the appropriate level of care while supporting safe, efficient clinic operations.
Duties/Responsibilities
Receive transferred patient calls for clinical triage from call center staff
Perform comprehensive symptom assessments via telephone
Utilize established triage protocols to determine level of urgency and disposition
Identify red-flag symptoms and initiate escalation to provider or emergency services as appropriate
Work closely with medical assistants, providers, and front desk staff
Assist in coordinating urgent add‑on visits
Route patients appropriately (same-day visit, future appointment, urgent care, ED, or self-care)
Document all assessments, advice, and dispositions accurately in the EHR
Document assessments and triage decisions accurately in the EHR
Maintain patient safety, confidentiality, and professional boundaries in all interactions
Collaborate with call center staff to ensure smooth call transfers and accurate patient information
Manage high call volumes while maintaining clinical accuracy and professionalism
Prioritize multiple calls based on clinical urgency
Participate in quality improvement efforts related to access, triage accuracy, and patient satisfaction

This is a second-shift role working 11:00 AM - 11:30 PM, three 12-hour shifts per week, including every other weekend.
The Triage RN works under the direct supervision of the Nursing Supervisor and provides high-quality, trauma-informed, person-centered nursing care in a fast-paced crisis setting. This position plays a critical role in supporting assessment, safety, and recovery.
This is an in-person role at our 24/7 Resource and Crisis Center in Pontiac, MI.
Essential Functions
Conduct initial nursing assessments and behavioral health triages for walk-in individuals to evaluate presenting concerns, acuity, and immediate safety needs.
Prioritize and triage individuals based on clinical urgency, risk factors, and level of care required.
Collaborate with the Crisis Assessment Team to determine appropriate interventions, referrals, or dispositions.
Participate in shift change reports and communicate pertinent updates to team members and leadership.
Maintain accurate, timely, and compliant nursing documentation in accordance with agency policies and regulatory requirements.
Support de-escalation and crisis intervention efforts to ensure the safety and stabilization of individual and staff.
Monitor individuals in the walk-in center for changes in medical or psychiatric status, responding promptly to emergent needs.
