Works with Interdisciplinary team to determine resident significant change in status and OMRA assessments.
Completes Nursing Sections of MDS per facility policy and ensures completion of assigned sections by other disciplines.
Follows and updates facility MDS schedule and tracking forms per policy and procedure.
Follow facility policy and procedure on Resident Care Planning.
Ensure that care plans accurately reflect the cares and clinical monitoring provided.
Audits completion and review of completed MDS assessments randomly on each floor monthly.

1. BACKGROUND
Flandreau Indian School (FIS) is a federal bureau operated school under Department of the Interior, Bureau of
Indian Education. FIS is an off-reservation boarding school located in Flandreau, South Dakota, Moody
County. The school provides service and support to approximately 100 Native American / Alaska Native High
School students from across the nation, representing 48 different tribes. Students reside at the school in
dormitories from mid-August until mid-May of each year with the exception of a winter break from mid-
December through the beginning of January.
2. OBJECTIVE
The purpose of this contract is for School Nurse services.
3. SCOPE
Contractor shall provide qualified personnel, necessary supplies, materials, and equipment needed to coordinate
School Nurse services.
Essential Duties:
o Shall provide onsite service.
o Shall provide services in a professional and respectful manner when dealing with students and
colleagues.
o Shall respect and maintain the basic rights of students including personal dignity.
o Shall not collect student data for personal research nor non-BIE case study activities.
o Shall follow and remain in full compliance with the agency policy, procedures, regulations, laws,
and accrediting organizations which govern the performance of daily operations.
o The Contractor is expected to demonstrate sensitivity to cross-cultural diversity.
o Contractor provider shall report to the security desk when entering or leaving the facility.
o Provide afterschool quality health assessments and treatment, including obtaining a health and
developmental history, screening and evaluating findings of deficits in vision, hearing, scoliosis,
growth, etc., observing the student development and health patterns in making nursing assessments
and nursing diagnosis, and identifying deviant health findings.
o Prepare and administer prescribed medication and treatments for students.
o Coordinates and assists with vision screenings for students.
o Responds to Medical emergencies within the school.
o Communicates medical issues/concerns with parents, school staff, and medical agencies according to
the guidelines set forth in the Indian Child Protection Act and HIPAA.
o Contacts parents concerning emergencies, serious/chronic health issues and assist with Dr. /clinic
referrals.
o Reports medical neglect/abuse concerns to appropriate agencies.
o Maintains health files and daily logs.
o Coordinates and actively orders medical supplies and maintains a healthy work area.
o Initiates referrals to parents, school personnel and community health resources for intervention,
remediation, and follow-through.
o Provides on-going health counseling with pupils, parents, school personnel and health agencies.
o Maintains, evaluates, and interprets cumulative health data to accommodate individual needs of
students.
o Hours will occur before and during school hours 7:00AM-4:30 PM with a 60-minute lunch break. Start
and end times will be determined based on need, with on call (emergency call back) services as needed
to FIS students so that all students are being serviced and evaluated according to their individual needs
during the school year.
o The contractor is expected to provide accurate case notes, assist with program planning, network with
internal and external resources, and provide documentation and record keeping tasks necessary to
compile reports for funding agencies.
o The contractor will attend internal and external meetings with Flandreau Indian School staff and/or off
campus agencies when necessary for service provision.
o Assist with student placement in inpatient facilities and emergency medical needs if required.
o Participation in regular case consultation.
o Provide documentation as needed for student's flies, and a quarterly report to the Acting Student
Services Director or Chief School Administrator detailing services provided.
o Collaborate with fellow FIS nurses and counselors in establishing comprehensive mental and physical
health.
o Provide service during the school year which occurs mid-August through June 30th.
4. SCHEDULE
On site services will be provided before, during, and after school hours
Service shall occur between 7:30 am to 4 pm Monday through Friday, excluding holidays, during the school
year. The school year is mid-August through June 30th. 8-hours per days, 5-days per week, 40-hours per week,
40-weeks per school year. Schedules are subject to change due to staffing changes, emergencies, holidays,
vacations, special events, and unexpected events. The government reserves the right to verify the hours worked
and shall utilize time and attendance logs, verification of computer logs, etc.
5. HOLIDAYS / SCHOOL CLOSURE
Closures: During school closure the Contractor shall not be scheduled to work. The Contractor shall not invoice
for planned closures, closures caused by emergencies, inclement weather, etc. which prevent school operations
unless approval to work during a closure is granted by the COR.
