Summary:
The LPN is responsible for providing supervision and coordination of medication management and resident care delivery. Additionally, the LPN assists the Director of Nursing in setting standards of nursing care and management of residents under the direction of a physician.
Duties and Responsibilities:
Prepares and accurately assists residents with the self-administration of prescribed medications to residents in accordance with individual medication care records.

Assures that all resident records are accurate. Completes and signs paperwork, resident care notes, medication administration records and incident reports and provides follow-up as required. Maintains confidentiality of resident and community information.
Interviews and examines current residents to assess needs, monitors and records resident medical condition and behavioral changes, assists in the formal process to re-assess resident care needs and recommends adjustments to care plans.
Report any suspicion or evidence of elder abuse, mistreatment, or neglect.
Attends training courses and participates in other professional development activities as required.
Qualifications:
Current LPN License.
Minimum of three (3) years in similar position, with retirement living or home-care background preferable.
High school degree required; college degree preferred.
Must be able to clearly and proficiently communicate with people of all ages, while maintaining an energetic and positive attitude.
Must be able to read and write English and follow verbal and written instructions.

Qualifications
Must successfully complete all Atria specified training programs.
Able to add and subtract two digit numbers and to multiply and divide with 10's and 100's. Able to perform these operations using units of United States' currency and weight measurement, volume, and distance.
Able to deal with standardized situations with only occasional or no variables.
Able to work various schedules and shifts as needed.
Able to safely use basic maintenance and landscape related hand tools and light power tools, such as shovels, rakes, blowers, mowers, drills, saws, sanders, buffers, vacuuming, extractors, etc.
Porters will be required to work with caustic materials and cleaning equipment in order to fulfill their job responsibilities. Responsibilities
Provide cleaning services for business offices, apartments, interior and exterior public and common areas and amenities of the community in a safe, neat, clean and attractive condition.
Provide floor care services for carpet, tile, and wood floors and may include vacuuming, extracting, buffing, sealing, stripping, etc.
Compact and remove trash and debris from community areas.
Maintain all grounds; including sidewalks, public walkways and stairways, building exteriors, garden areas in a neat, clean and attractive condition.
Performs basic maintenance related work requests.
Mows lawn, trims shrubbery, and cultivates flowers (varies by community).
Notifies management concerning need for repairs.
Reports potentially unsafe conditions promptly to Supervisor.
Demonstrates accuracy and thoroughness in work performed.
May perform other duties as assigned.

Qualifications
Licensed Vocational Nurse/Licensed Practical Nurse or Registered Nurse with a current license in good standing.
Certification or licensed per state guidelines.
Basic First Aid and CPR certification if required by State.

Responsibilities
The Wellness Nurse is responsible for assisting the Wellness Manager with the overall implementation, delivery, and coordination of Resident Care services at the Community. The Wellness Nurse will also assist in monitoring the medication management program.
Assist in supervising the medication program, activity in the medication room, and assisting the residents with self-administration of their medications.
Assist residents with Activities of Daily Living, as needed.
Maintain positive relations with residents, families, and physicians.
Report any significant incidents and/or changes in the resident's needs to the General Manager/Wellness Manager.
Assist in the completion of resident assessments and services plans.
Assist in the training of Assisted Living staff and participates in orientation of new staff.
Attend and participates in scheduled in-service programs, training programs, and staff meetings.
Assist with scheduling and staffing of Wellness Assistants and Medication Assistants as needed.
May perform other duties as needed and/or assigned.
