Responsibilities
AlDS Healthcare Foundation (AHF), a nonprofit organization, is the largest specialized provider of HIV/ AlDS medical care in the world. Over the past 37+ years AHF has grown to include 78 healthcare centers in 20 U.S. states and Puerto Rico and D.C., pharmacies, health plans, testing and linkage to care, STI clinics, research, and advocacy. In addition, AHF's global program provides HIV care in 47 countries in Africa, Asia, Europe, Latin American and the Caribbean. AHF's mission is to treat all HIV-positive persons, regardless of their ability to pay.
We are currently seeking exceptional MDs or DOs who have completed residencies in Family Medicine or Internal Medicine to join us in our fellowship program beginning in August 2025. The AHF Fellowship Program will be based in Southern California area with the option to expand the program to other states like; Florida, Georgia, Texas, and New York by 2026.
Our 1-year paid Clinical Fellowship in HIV Primary Care will consist of rotations that will encompass a robust mix of; HIV primary care, hepatitis B & C treatment, HIV-related mental health, HIV in an aging community, public health / STI treatment, overcoming challenges faced within underserved communities, managing poly-pharmacy, transgender care, PrEP and PEP management, Quality care initiatives and addressing LGBTQ-specific needs. Additionally, the program will offer a unique opportunity to take part in our global initiative with an embedded rotation at one of our international healthcare centers.
Upon completion of this program, physicians will possess the requisite number of clinical and dicatic hours to sit for the AAHIV exam.
Education and/or Experience:
Medical degree (M.D., D.O., RNP) from an accredited university.
Successful completion of an ACGME accredited residency program in Family Medicine or Internal Medicine and Board Certified/Board Eligible in either of those

A PCS2 contributes to AHF's mission by presenting a friendly, helpful face to patients, people seeking testing or pharmacy patrons. Within their duties of greeting patients in a courteous and sensitive manner, scheduling appointments and assisting with the needs of our clients, this position has the opportunity to give standout service that enhances the reputation of AHF, encourages clients to continue to seek medical treatment from an AHF Healthcare Center and recommend our services to others. The PCS2 role includes offering patient intake interviews, financial screenings and registering for Ryan White and AIDs Drug Assistance Program (ADAP) services. Ensuring that appropriate documentation for patient eligibility is filed and entered into our computer database.
Within their duties of accurately collecting new patient medical history, determining what tests/screenings are due during medical visits, taking patient vitals and documenting all patient health-related complaints, this position has the opportunity to provide excellent patient centered care.
By communicating with patients between provider visits for condition updates, and follow through on test/procedures ordered by providers, the PCS2 can help ensure the patient is receiving the highest quality of care.
The PCS2 manages the referral process by processing specialty referrals in the most timely and accurate manner to optimize care and to address clinical needs; providing all pertinent information to patients for timely appointment scheduling; assisting with appointment scheduling as patients require; and obtaining specialist consults and scanning into the patent's EMR to provide the most complete picture of a patient's clinical condition. The Temp PCS2 will track and monitor the status and progress of the referral being completed. They will notify a provider of a patient's lapse in completing the referral and will remain in contact with the patient to improve adherence.
The PCS2 adds consistent value to the Healthcare Center Team especially in high-stress environments that demand immediate attention, by being flexible and professional at all times.

Responsibilities
As a Care Management Program Community Health Worker (CHW), you will work to create connections between our vulnerable patient population and their AHF Healthcare Center healthcare providers.
As a CHW you will be called upon to utilize your many unique talents to help patients navigate the healthcare and social service systems in various and challenging ways. The CHW assists with the transition of patients back into care, advocating for underserved individuals to receive services and resources to address health and social needs, documenting and collecting data to inform providers, the program and policies, assists the Care Coordination Team in building community relations to address patient needs and proactively addressing individual patient social determinants of health (SDOH) needs. As a CHW you will be a frontline agent of change in helping to reduce health disparities in our vulnerable population and increase their retention in care. A Quality Care Management Registered Nurse contributes to AHF by improving health outcomes of patients receiving services throughout the AHF Healthcare System through compassionate proactive collaborative care. The position may be hybrid of both remote HCC locations and field work. QCMRN will demonstrate strong knowledge of Healhcare Effectiveness Data and Information Set (HEDIS), HIV AIDS Bureau (HAB) Ryan White Standards and other preventative measures as appropriate.
A QCMRN enhances the reputation of AHF by collaborating with providers and patients in addressing preventive health measure outcomes of all payor sources through improving HEDIS and HIV/AIDS Bureau measures, gaps in care, retention, barriers to care, transitions of care, and when needed, chronic care management.. With radical collaboration with AHF departments and entities, QCMRN work will directly to capture value based bonus revenue based upon meeting quality, administrative and risk parameters outlined in Third Party (TPP), AHF Health Plan(s) and specific Ryan White contracts. The Member Services Representative (MSR) serves enrollees in California, Florida and Georgia who are enrolled in AHF's Medicare and Medi-Cal managed care plans. The MSR answers enrollee, prospective enrollee and AHF Healthcare Center patient calls, emails and electronic chats coming into the Member Services and AHF Patient Relations call center. He or she also answers provider calls regarding enrollee eligibility or claims status enquiries. The MSR actively supports enrollment and eligibility processing functions and enrollee materials distribution.
The MSR answers incoming contacts to the call center to meet contractual and regulatory requirements for call center operations that support Medicare and California Medi-Cal managed care plans. He or she participates in the Member Services Department's processes to meet enrollment and disenrollment timeliness; complaint, grievance and appeal intake documentation and acknowledgement; and enrollee non-emergency transportation booking rules.
The MSR demonstrates his or her success in this role by contributing to Member Services operation so it complies with Centers for Medicare and Medicaid Services (CMS) and California Department of Health Care Services, regulations, yet delivers thorough service to enrollees and providers in a professional, competent and compassionate manner.

We are currently seeking a F/T Night Shift Pharmacy Technician working in our Central Fill Pharmacy. 4x10hr shifts, rotating schedule.

Collaborative contributions by the Pharmacy Technician is essential to our success including working with various departments within Pharmacy as well as other divisions of AHF, our patients and partners. The skills discussed below are representative of the knowledge, experience and abilities that lead to our success together.
Pharmacy Technician contributes to AHF's mission by presenting a friendly smile, a helpful voice, a helpful attitude, to patients, clients and customers seeking services, information, requesting to transfer their prescriptions to AHF, filing grievances or complaints.
Within their duties of handling incoming calls, meeting and greeting patients in the Pharmacy they are problem solvers, providing a "helping hand" and provide a calming influence to patients that seem to be upset or in a stressful situation.
The Pharmacy Technician can enhance the experience of other AHF Pharmacy employees by being friendly and helpful and by offering to give them a helping hand when they need it. In addition, giving feedback to their manager about how service could be improved will allow the team member to enhance their contribution.
The Pharmacy Technician adds consistent value to the AHF Pharmacy Team by adapting to various situations with ease and by being flexible.

A PCS2 contributes to AHF's mission by presenting a friendly, helpful face to patients, people seeking testing or pharmacy patrons. Within their duties of greeting patients in a courteous and sensitive manner, scheduling appointments and assisting with the needs of our clients, this position has the opportunity to give standout service that enhances the reputation of AHF, encourages clients to continue to seek medical treatment from an AHF Healthcare Center and recommend our services to others. The PCS2 role includes offering patient intake interviews, financial screenings and registering for Ryan White and AIDs Drug Assistance Program (ADAP) services. Ensuring that appropriate documentation for patient eligibility is filed and entered into our computer database.
Within their duties of accurately collecting new patient medical history, determining what tests/screenings are due during medical visits, taking patient vitals and documenting all patient health-related complaints, this position has the opportunity to provide excellent patient centered care.
By communicating with patients between provider visits for condition updates, and follow through on test/procedures ordered by providers, the PCS2 can help ensure the patient is receiving the highest quality of care.
The PCS2 manages the referral process by processing specialty referrals in the most timely and accurate manner to optimize care and to address clinical needs; providing all pertinent information to patients for timely appointment scheduling; assisting with appointment scheduling as patients require; and obtaining specialist consults and scanning into the patent's EMR to provide the most complete picture of a patient's clinical condition. The Temp PCS2 will track and monitor the status and progress of the referral being completed. They will notify a provider of a patient's lapse in completing the referral and will remain in contact with the patient to improve adherence.
The PCS2 adds consistent value to the Healthcare Center Team especially in high-stress environments that demand immediate attention, by being flexible and professional at all times.
