Major Responsibilities:
Conducts a physical therapy evaluation of each patient and establishes a plan of care and treatment goals. Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integument evaluations utilizing appropriate methods. Evaluates outcomes within an appropriate time frame.
Provides on-going assessment of patient response to treatments and teaches and applies interventions as appropriate. Updates and revises plan of care as indicated and provides physician with status report on a regular basis.
Communicates problems to physicians and management and clearly states plans, actions and goals so changes are reflected as they occur. Completes a plan of care as indicated and provides physician with status report. Maintains and updates clinical and patient records according to state and federal guidelines.
Manages assigned caseload including scheduling and informing site leadership of coverage needs and ability to provide coverage. Manages patient's medical condition including medications. Acts as a patient advocate to insure optimal outcomes. Manages assigned caseload for optimal functional and financial outcomes; reviews caseload for Low Utilization Payment Adjustments (LUPA), optimal number of visits based on progress, Prospective Payment Plan (PPS), and Outcome Based Quality Initiatives (OBQI) outcomes.
May manage a caseload of Physical Therapist Assistants (PTA) including evaluating their patients, insuring compliance with established Care Plan, and adhering to all State of Wisconsin regulations regarding PTA supervision.
Performs Home Health Aide joint visits as appropriate to maximize patient's functional abilities in the home. May assist with evaluating patients, turning patient over to PTA and informing PTA of Plan of Care (POC) per State of Wisconsin regulations. Teaches appropriate treatment procedures and supportive activities to patients and caregivers and develops plan for obtaining adaptive devices as needed.
Discusses caseload issues and changes in case census with management. Communicates problems to physicians and management, and clearly states plans, actions and goals so changes are reflected as they occur.
Participates in chart audits and continuous quality improvement activities. Facilitates discharge planning process and utilizes interagency/system and community resources to assure continuity of care after discharge.
Acts as a resource to employees, which includes training/orienting, mentoring, and providing guidance on more complex issues. Serves as an instructor for students, meeting all expectations of the Clinical Student Program.
Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Physical Therapy License in the State of Illinois or State of Wisconsin, must be licensed in the State upon which you work.
Basic Life Support (BLS) for Healthcare Providers (obtained within 90 days unless department leader has determined is not required) by American Heart Association (AHA)
A valid drivers license issued by the Division of Motor Vehicles.
Education/Experience Required:

Main Purpose of Job
A Residential Child and Youth Care Practitioner (RCYCP) at the Maryland School for the Deaf is the full performance level of work providing guidance to deaf and hard of hearing students by serving as a mentor and role model, offering advice and assistance with everyday life skills, and providing referrals to other staff at the Maryland School for the Deaf. Employees receive general supervision from a RCYCP Supervisor, School for the Deaf, or from a designated administrator. Employees might be asked to work evenings, nights, weekends, holidays and on other campuses. Employees may be required to travel to various locations for events such as field trips, workshops, and recreational activities.
POSITION DUTIES
Maintain a safe environment and ensure of student's safety at all times; Possess responsibility for the students at all times; Makes parents/guardian and teacher contact 1 time a week or as required by supervisor; Informs parents of student development through telephone calls, letters, and meetings with the Parent, Teacher, and Counselor; Interacts with students to offer them guidance and discuss their problems as needed, and serves as a mentor and role model for their assimilation into the deaf culture; Teaches and assists student in complying with daily and weekly schedules of completing homework, sleeping, eating, cleanliness of their living areas and performing personal hygiene; Documents and reports and reviews student concerns to the RCYCP Supervisor; Assesses emergency and crisis situations, makes appropriate contacts and referrals, and provides interim emergency medical and other assistance until specialized assistance is available; Develops, implements, and monitors student activities; Maintains weekly reviews on student's progress/developments in the dorm with team; May participate in the development of educational programs and workshops; May supplement classroom instruction by tutoring students in academic subjects; Maintains strict confidentiality of students and school personnel information; Participates in appropriate professional development; May make periodic safety checks of doors and windows of student residences; Provides support while traveling to and from events such as extracurricular activities, workshops and field trips. Employees may be subject to call-in based on staffing needs. Individuals who are assigned to perform the direct care responsibilities related to activities of daily living, self-help, and socialization skills in a residential child care program; Performs other related duties.
MINIMUM QUALIFICATIONS
Education: Must be either 21 years old with graduation from an accredited high school or possession of a high school equivalency certificate. OR At least 18 years old with possession of an Associate of Arts degree or a Bachelor's degree from an accredited college or university. Experience: One year of experience providing guidance to deaf and hard of hearing youth and assisting them with everyday life skills in a student life setting. Note: Candidates may substitute additional education from an accredited college or university at the rate of sixty credit hours for the required experience. Fluency in American Sign Language is required for this position.
DESIRED OR PREFERRED QUALIFICATIONS
Knowledge of the physical, emotional and social aspects of deafness; Understanding of Legal and Ethical Issues in Residential Care; Post secondary training in counseling, child development, or education; Experience at a residential school for the deaf or another residential childcare facility; Experience in teaching and caring for children and youth with and without disabilities; Possess Maryland medication administration or equivalent certification; Ability to learn emergency medical procedures; Ability to maintain a dormitory environment conducive to positive student development; Ability to work harmoniously with students and with other employees. Hold or obtain a valid driver license.
LICENSES, REGISTRATIONS AND CERTIFICATIONS
1. Employees in this classification will be required to obtain and maintain RCYCP certification. 2. Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator's license valid in the State of Maryland. 3. Employees in this classification will be trained in cardiopulmonary resuscitation and first-aid training leading to certification. BARGAINING UNIT STATUS
In accordance with AFSCME Article 44, all positions advertised as contractual are temporary assignments. BENEFITS
Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits. Performs lab related duties such as venipuncture, specimen collection/labeling/packing, preparation of lab orders/requisitions, and results tracking. Point of Care testing (POCT) per standing order/provider order. Completes Clinical Laboratory Improvement Amendments (CLIA) approved waived laboratory tests and practice-specific diagnostic testing in adherence to CLIA standards.
Administers medications and vaccines safely following the rights of medication administration.
Performs other duties as assigned such as medication refills, insurance authorizations, safety/regulatory log completion, patient messaging, medication/supply ordering, appointment scheduling, and referrals.
Cross trained to perform duties that are relevant to specialty or clinic practice within the scope of the Medical Assistant.
Demonstrates effective and timely communication, teamwork, and appropriate escalation.
Maintains a clean work environment ensuring instruments and equipment are cleaned appropriately. Ensures equipment and supplies are in working order, and areas are appropriately stocked.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Position Summary
The Home Visitor is responsible for initiating and maintaining regular contact/support with families enrolled in our Home Visiting programs. This activity will occur primarily in the family's home and each visit should last at least an hour. The interventions used should be family centered and strength based and directed at establishing a trusting relationship; assisting in strengthening the parent-child relationship; assisting parents in improving their skills to optimize the home environment; improving the family support system; and increasing the family's ability to problem solve and assume the role of advocate for themselves and their children. The Home Visitor will also be responsible for assisting the family in establishing goals and a plan for accomplishment of those goals, as well as the assessment of the normal growth and development.
Position Requirements
High School Diploma or Equivalent required. Bachelor degree preferred.
One year experience working in community programs with families of diverse ethnic, cultural and socioeconomic background.
Experience working with or providing services to children and families.
Infant Mental Health endorsement preferred. Demonstrates an openness with the concept of Reflective Practice.
Bilingual in Spanish/English or Hmong/English is a plus.
Requires a valid driver's license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier.
Must use personal vehicle to transport children, complete in-house car seat transporter training within one month of hire and re-train biannually thereafter to maintain.
Knowledge of milestones of infant and child development along with the ability to demonstrated knowledge of psychosocial, economic and cultural determinants of healthy growth and development. Working knowledge of current trends and developments in the prevention and family support field. Knowledge of the ecological theories and family support principles.

Excellent verbal and written skills. Ability to communicate effectively both in oral and written communication, including the ability to write factual progress notes, effectively present information in individual and group settings with a variety of audiences.
