What To Expect In This Role Provide high-quality Applied Behavior Analysis (ABA) services to students across school, home, and community settings, including designing and modeling evidence-based behavior strategies for staff and families in a trauma-informed way. Collaborate with the IMPACT Mental Health Clinician to determine student's behavioral needs and develop IMPACT Support Plan. Conduct behavioral assessments including direct observations, interviews, and data review, and interpret results to guide intervention planning. Develop, write, and update individualized treatment plan goals and train Behavioral Support Specialists on the implementation of these goals. Provide weekly supervision to 2 Behavioral Support Specialists in order to monitor student progress, evaluate intervention effectiveness, and provide clear recommendations based on trends. Provide direct support sessions to students when needed. Collaborate closely with school administrators, teachers, counselors, mental health providers, and other members of the interdisciplinary IMPACT team to promote consistency and aligned support across environments. Communicate regularly with families and caregivers to review progress, provide family coaching/training, and ensure continuity of strategies across school and home. Maintain detailed and accurate documentation, including assessment reports, treatment plans, session notes, supervision summaries, and progress updates in alignment with program and district expectations. Manage an assigned caseload, maintain consistent communication with the IMPACT team, school leadership, and caregivers, and support smooth coordination of services. By integrating both behavioral health and social support services, we strive to build a healthier community by ensuring individuals have the resources they need to achieve stability and wholeness. Children's Services encompass the services that help ensure children's mental and physical well-being are protected and safe. The Child Welfare division of the branch investigates alleged abuse and neglect of children, and it licenses foster and adoptive homes for children who are experiencing traumatic events in their lives such as the removal from their caregiver due to alleged abuse and neglect. This division also works within the confines of the Dependency Court to reunify families when and where they can demonstrate appropriate change to meet the safety needs of their children. The branch also encompasses children's mental health teams which work in conjunction with the Child Welfare division along with community providers to ensure that the youth that are being served are having their mental health and substance use challenges addressed. This branch also works with juvenile justice and education to coordinate services for clients. Adult Services focuses on services to support people eighteen and older. Adult mental health services include outpatient mental health care, 24-hour crisis stabilization services for people at risk of hospitalization, 24-hour crisis line, social rehabilitation residential services, long-term placement services and referrals to local non-profit organizations that provide additional services. This branch also provides Adult Protective Services to follow-up on elder abuse reports. In-Home Supportive Services help those who need assistance with daily activities in order to remain safely in their own homes, and the Public Authority keeps a registry that helps In-Home Supportive Services recipients find a screened caregiver to provide those services. The Public Guardian helps citizens who are unable to manage their personal and financial affairs and may require guardianship or conservatorship. Mental Health Services Act administration is also housed in the Adult Services Branch. ABOUT THE POSITION Under supervision and in cooperation with Mental Health and Substance Abuse professionals and paraprofessionals, to learn to perform a variety of paraprofessional Mental Health and Substance Abuse services in the community or clinic setting, and to perform related work as required. This opening is in the Behavioral Health and Social Services Branch of the Health and Human Services Agency (HHSA) and serves the CalWORKs Behavioral Health Team (BHT) and at times the Women's Recovery & Resiliency Services program (Drug and Alcohol Treatment). This position requires working level knowledge of Microsoft Office. As a Community Mental Health Worker, you will be working front reception, scheduling clients, screening intakes, maintain medical records, preform community outreach/presentations and provide transportation, and childcare cooperative services in addition to other support tasks. DISTINGUISHING CHARACTERISTICS The Community Mental Health Worker is a paraprofessional class and is distinguished from licensed and case management personnel in that the Community Mental Health Worker does not assume primary responsibility for mental health or substance abuse clients. The Community Mental Health Worker performs a variety of less complex functions in the mental health department which do not require the skill and training of a mental health professional or case manager. IDEAL CANDIDATE The ideal candidate will have knowledge and experience with clerical duties including using Word, Excel, database entry, scheduling software, and management of spreadsheets. Ability to collaborate with county and community partners. The ideal candidate understands that most of the duties are clerical and supportive in nature, however, has the ability to assist clients in crisis.     Job Title: Case Manager for Circle of Friends Department: Community Programs FLSA Status: Non-Exempt Reports To: Wellness Program Manager Hours: Monday through Friday 8:30am to 5:00pm PROGRAM DESCRIPTION: Hill Country Community Clinic has been awarded a SAMHSA grant to become a Certified Community Behavioral Health Clinic (CCBHC). This enables Hill Country to expand and refine our recovery-oriented approach to substance use and mental health services, especially for people with very complex needs. CCBHC's are designed to provide whole person care with a comprehensive range of mental health and substance use services to vulnerable adults and children. Whole Person Care means all aspects of a person's health is considered, including their physical, mental and behavioral health, socio-economic status, housing and other psychosocial needs that can impact their health. CCBHC's are presently considered the "gold standard" care model for people with complex needs. In practice, the CCBHC team partners with primary care and other programs to screen and engage with people, including identification of social factors that may be contributing to health concerns or risks. Treatment plan goals will be developed, together with the participant and their family members, with special attention to participants' goals, needs and risks. POSITION SUMMARY: Under minimal supervision, work independently and together with Circle of Friends and Hill Country Community Clinic teams to provide outreach and direct case management services to help clients achieve wellness and recovery goals. The Case Manager is responsible for managing an assigned caseload. Individuals have diverse, complex health care needs that require comprehensive case management services, including SUD, mental health disorders, chronic health conditions and behavioral issues. This is a versatile position that may require field work and collaborative efforts within the community. The CCBHC Case Manager will provide engagement and support services, particularly to program participants engaging in behaviors that pose health and safety risks by: making meeting individuals "where they are" and providing non-judgmental, non-coercive services, and resources to meet their needs. ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned which may become essential to the position. * Provide outreach and direct case management services in a professional and culturally competent manner to persons/families. * Initiate contact and establish positive rapport with clients/potential clients. * Provide clients with empathy, encouragement, support, and facilitate problem solving. * Interview clients to identify areas of need, strengths, resources, and goals. * Help clients make use of their own strengths and resources in addressing their needs/goals * Conduct mental status exams, provide crisis intervention, and coordinate emergency care as needed. * Assist clients in obtaining basic needs (i.e. food, clothing, shelter). * Advocate for clients as needed to obtain housing, public benefits, and other community resources. * Help clients develop budgeting and money management skills. * Coach clients regarding activities of daily living, social skills, and communication skills. * Making referrals to community resources to best meet their needs and follow up as needed. * Assist clients in finding and using opportunities and resources to improve quality of life (e.g. education, employment, volunteering, recreation, etc.). * Work creatively with other team members to implement a "whatever it takes" approach to support resilience, wellness and recovery among clients. This includes participation in team meetings, daily communications with team members, collaborative problem solving and advocacy. * Complete documentation of interactions, assessments, interventions, and services provided on a daily basis. * Complete data collection for tracking and reporting of the CCBHC grant activities, including NOMS data at intake, six-month follow-up and discharge. * Build collaborative relationships with HCHWC staff, social service providers, schools, employers, emergency service personnel, and other community agencies and individuals. * Improve quality of services provided at Hill Country Community Clinic by developing and sharing knowledge and expertise about community resources, public benefits programs, work/volunteer opportunities, etc., with other staff. * Increase professional competence by participating in available training opportunities. * Attend weekly team meetings, and other meetings as appropriate. * Other projects and tasks assigned SUPERVISORY RESPONSIBILITIES: This job has no direct supervisory responsibilities, however, is responsible for collaboration with CCBHC and Circle of Friends staff, as well as the larger HCCC staff as needed to serve our clients and community.
