If you meet these qualifications, we want to meet you!
Master's Degree from accredited school of Social Work or related field; Valid license/registration of permit to practice in state(s) of agency operation if required
Medicare/Medicaid/Insurance specialty preferred
Unless otherwise dictated by the state, at least one (1) year of professional social work experience (post-graduate) in a healthcare setting required; 2+ years preferred
Experience or education in gief counseling preferred.
Discharge planning experience preferred.
Responsibilities:
As a Hospice Clinical SocialWorker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical socialworker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Job Description Summary:
Responsible for the delivery of social work services for a variety of patients including infant, pediatric, adolescent, adult, and geriatric populations to all medical areas of the hospital. Develop creative interventions, collaborate with the multidisciplinary team (i.e. psych, child life, nursing, PT, etc.) to address behaviors when limited to no placement options are available.
Conduct brief solution focused/strategic clinical interventions utilizing motivation interviewing.
Complete legal paperwork to purse involuntary psychiatric hospitalizations and probate court guardianship hearings. Collaborate with hospital attorney and testify when required.
Specialized knowledge of chronic illness and the impact on patient mental health.
Knowledge of community service organizations (i.e. CMH, DHHS, shelters, transportation, etc.) and the types of assistance they provide.
Work by consult (i.e., physician, RN, patient, family, etc.).
Participates in multidisciplinary rounds and provides necessary report to advance patient care.
Collaborates with other members of the patient care team to understand each situation from different perspectives and recommends or provides immediate treatment and solution focused interventions.
Identifies needs, develops plan of care, and makes referrals to community agencies and other healthcare specialists and plans for patient discharge to ensure the appropriate follow-up treatment and support is available and provided to support a safe discharge.
Thoroughly documents each case and enters appropriate information in patient's electronic medical record in an effective and timely manner.
Participates in the development of departmental policies, procedures, and protocols.
Identifies and provides for learning needs of patients, families, and health care co-workers.
Assumes responsibility for professional development. Participates in required in services, staff meetings, and completes all required competencies.
Develops and supports a positive work climate and the overall team effort of the department.
Participates in orienting new MSW hires when assigned, following orientation checklist, best practice and standard work.

This position is responsible for facilitating smooth Home Care admissions, assessing and planning appropriate interventions to meet the clinical needs of patients and families prior to and upon admission to Blessing Home Care. The position works diplomatically and effectively across professional disciplines, particularly with the professional staff, as well as with patient/families, and outside agencies in accomplishing an acceptable outcome for the benefit of those served.
This position requires providing services to one or more age populations, including infant through geriatric, in a manner that demonstrates an understanding of the functional/developmental age of the individual served.

RESPONSIBILTIES:
* Participates on the IDT's initial assessments, care planning and on-going re-assessments of participant care.
* Attends IDT meetings; actively participates in team meetings by sharing pertinent information, providing follow up to assigned tasks and helps to develop participant's plan of care.
* Completes all assigned assessments - (initial, semi-annual, annual, service request and/or significant event).
* Assesses the psychosocial needs of the participant and provides supportive counseling, working collaboratively with behavioral health providers.
* Facilitates hospital, rehabilitation and nursing home (NH) admissions and discharges as determined by the Interdisciplinary Team. Ensures that PASRR documentation is completed for NH admission.
* Assists in the conversion process of the participant from community to long-term care. Works collaboratively with Medicaid Specialist, skilled nursing facility, and participant's caregiver to complete conversion.
* Arranges and facilitates family meetings, as needed.
* Refers participants and families to appropriate community services and acts as liaison and/or advocate with community organizations for participants.
* Maintains professional, accurate and timely social service documentation in the participants' medical records.
* Conducts participant council meetings as assigned.
* Works collaboratively with Director of Social Work and Behavioral Health provider to ensure guardianship is up to date. Educates participant regarding health care proxy (HCP). Assists participant in completion of HCP form.
* Works collaboratively with fiscal department to maintain participant insurance benefits and completes required documentation of fiscal information in the medical record.
* Reviews plan of care with participants, guardian, and/or activated health care proxy as assigned.
* Complete authorizations for home care and other approved services timely and accurately.
* Completes home and/or skilled nursing facility visits to assess participant as indicated.
* Works collaboratively with Palliative care team; Assists with end of life planning as indicated.
* Provides timely communication to appropriate staff regarding the following: (disenrollment, conversion to long term care, transfer of sites, participant and/or caregiver demographic changes).
* Reports allegations of abuse to appropriate state agency; provides support and resources to participant as he/she will accept; completes required documentation.
* Ability to pass a fit test. Position requires mask where seal is critical. Incumbent is required to not have facial hair that interferes with a tight seal of the respirator.
* Performs other duties as required.
* Frequent local travel.
Qualifications:
* Current Social Work licensure in the Commonwealth of Massachusetts at the Masters level (L.I.C.S.W. or L.C.S.W.) required
* Minimum of 1 experience in Social Work providing traditional Clinical or Case Management services with a geriatric population
* Current C. P. R. Certification or ability to become certified
* Treat all participants in a welcoming and professional manner.
* Strong verbal, written and listening skills with ability to multi-task in a fast pace environment
* Vietnamese and/or Khmer speaking but all MSW eligible candidates to apply.
