In this role you will assess, plan, and implement individualized therapy programs in collaboration with patients, families, and other healthcare professionals to address physical, cognitive, and emotional challenges. Schedule: PRN, Day Shift, Weekends and Holidays if needed Primary Responsibilities and Duties: Conduct comprehensive assessments of patients' physical, cognitive, and emotional needs to develop personalized therapy plans. Design and implement individualized therapy programs to address impairments, functional limitations, and performance deficits. Educate patients and their families on techniques and strategies to improve daily living skills and adapt their environment to support functional independence.
