Document each visit, intervention, and patient progress accurately and timely in accordance with agency policy. QUALIFICATIONS: Graduate of an approved school of Physical Therapy with a preferred minimum of two (2) years clinical experience. Have ability to provide for patient's physical rehabilitation and psychosocial well-being. Ability to meet lift in classification. Ability to communicate with others both medically and general public. Have a nice appearance being well-groomed and demonstrate appropriate contact. Must have or be eligible for State Licensure or Registration. Ability to drive vehicle within Umatilla/Morrow County for Home Health patients. The car must be in reliable condition with insurance coverage and have a valid driver's license. Must have a current certification in CPR.

This role is responsible for assessing, planning, implementing, and evaluating physical therapy care to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities. The therapist ensures all care is delivered in compliance with Title 22 regulations, Medicare Conditions of Participation, ACHC standards, and agency policies and procedures. Reporting Relationship Director of Patient Care Services/Nurse Manager Scope of Supervision None Responsibilities include the following: Provide skilled physical therapy services to patients in the home setting, including evaluation, treatment, and patient/caregiver education. Develop, implement, and revise individualized therapy plans of care in collaboration with the interdisciplinary team and the patient's physician. Document all patient visits, assessments, and interventions in accordance with Medicare, Title 22, ACHC, and agency standards for accuracy, timeliness, and completeness. Monitor patient progress and communicate changes in condition or plan of care to the physician and interdisciplinary team promptly. Participate in case conferences, staff meetings, and ongoing training as required. Submit all clinical documentation within 48 hours of each patient visit. Minimum Qualifications: Current, active Physical Therapist (PT) license in the state of California. Minimum of one (1) year recent clinical experience, preferably in home health or acute care. Valid California driver's license, reliable transportation, and proof of auto insurance. Strong clinical assessment, communication, and documentation skills.
