This on-call position is a member of an outstanding nursing and direct care staff and plays a vital role in providing person-centered care and support in a safe environment for up to forty-five (45) elders.

What You Will Be Doing: Provide high quality personal care to assisted living home residents in a warm, supportive, and caring manner. Following the resident's Assisted Living Plan, this position assists residents with lifting, turning, moving, positioning, and with transporting into and out of bed, wheelchairs, bathtubs, lifts, and using walkers and canes, etc. This position is also part of a team providing programs that promote physical well-being and relationships by involving residents in activities where they engage in meaningful person to person social interchange with other residents, family, friends, and staff.

Minimum Qualifications
Eligibility for certification to practice as a CertifiedNurseAide in the State of Alaska.
Special Note:
Candidates not currently certified by the State of Alaska, who otherwise meet the eligibility requirements for certification as a NurseAide in the State of Alaska and requisite qualifying experience, may apply for vacancies in this job class. Applicants qualifying under this option may receive a conditional job offer; however, proof of certification will be required prior to final appointment and reporting for duty.

This position also participates in planning, coordinating, and leading social and recreational activities for the residents and leads group sessions and programs that promote physical well-being and relationships by involving residents in activities where they engage in meaningful person to person social interchange with other residents, family, friends, and staff. Duties include planning, organizing, and implementing individual and group recreational programs. Uses therapeutic techniques to motivate the socialization and constructive uses of leisure time and/or skill. Leads and encourages residents in individual and group activities of a physical, social, and cultural nature. Examples includes physical exercise activities, hobbies and crafts, parties, picnic, religious activities, holiday celebration, etc.
The job duties may include driving, escorting, and transporting residents for various reasons operating a wheel chair accessible passenger van. The Home/Community Provider implements a prescribed habilitation plan for eligible participants while providing routine direct care either in the home, community or center based. This position works closely with the family or guardian and the FRA Home/Community Services Manager to design, implement, and modify consistent intervention strategies that can ultimately be maintained by the family or guardian in their daily routines.
DUTIES AND RESPONSIBILITIES:
1. Depending upon the eligible participants needs provides direct care as per the Plan of Care (POC). Services may include acquiring, retaining and improving self-help, socialization and adaptive skills such as meal preparation, eating, toileting, bathing, dressing and recreational activities.
2. Assures that health and safety needs are met.
3. Prepares and/or assists the individual to prepare meals. Ensure that nutritional needs are met during the time of care.
4. Observes, supervises and records eligible participant activities. Discusses with Home/Community Manager and other staff members the participants' responses to intervention strategies and makes suggestions for modifying activities.
5. Maintains records and prepares reports on services provided.
6. Performs or requests emergency aide as needed or required.
7. Maintains current knowledge of developmental disabilities.
8. Inputs participant information and case notes into electronic medical record (Therap), in accordance with agency procedures, state and federal requirements when applicable.
9. Attends training seminars, in-service sessions and staff meetings as requested.
10. Responsible for maintaining training compliance in accordance with FRA policies and procedures, state and federal regulations.
11. Provides other support services and duties as necessary.
MINIMUM EDUCATION AND EXPERIENCE:
1. High School Diploma or equivalent.
2. Six months experience working with individuals who experience developmental disabilities is preferred, but not required.
3. Must meet agency insurance criteria for operating agency and personal vehicles on agency business (21 y/o and licensed 3 years).
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Demonstrate proficiency in direct care activities. Ability to communicate effectively, both orally and written, with participants, families, guardians, supervisor, other agency staff and the general public. Excellent observational and organizational skills to assess and record participant responses to interventions. Ability to work flexible schedule. Ability to work independently, learn intervention strategies, and prioritize activities with minimal direction. Ability to be punctual, reliable, enthusiastic and professional. Demonstrated interest in working in direct care position serving the emotional, physical and health needs of eligible participants. Must have reliable transportation and maintain a good driving record. Must possess the knowledge and ability to utilize a computer and electronic medical record (EMR) system. Ability to learn new software and hardware applications, including ADP for time and attendance.
