JOB SUMMARY
The Licensed Clinical Social Worker (LCSW) provides clinical assessment/problem identification, individual, family and group therapy and case management. Develops and monitors treatment plans, monitors treatment progress, documents significant interactions, and maintains clinical charts and other reporting data. Must be licensed or provisionally licensed and requires a Master's degree in a mental health field. Reported to the Department Lead.
The Kid Clinic is a school-based pediatric clinic that serves children ages 2 weeks to 21 years old and offers counseling services for children 4 years old to 21 years old. The Kid Clinic is a collaborative effort between Campbell County Health, Campbell County Medical Group, and Campbell County School District to provide primary medical care, behavioral health, and substance use counseling services in Northeastern Wyoming.
ESSENTIAL FUNCTIONS
Provides assessment, diagnosis, and treatment of behavioral health clients.
Provides clinically appropriate therapy utilizing best practices whenever possible.
Is actively engaged in the department, solves problems, participates in strategic planning, and contributes positively to workplace moral.
As assigned by manager and location participates in on call crisis rotation after hours and assists with walk-in crises during business hours.
Works positively and collaboratively with courts, law enforcement, physicians, hospital staff, Department of Family Services, Probation Offices, Campbell County School District, and other community agencies as applicable.
Maintains clinical documentation in a manner as required by department policy, the Standards for the Operation of Community Mental Health and Substance Abuse Programs, and the Medicaid Policies and Procedures Manual, and in accordance, as applicable with SAMHSA TIPS and TAPS.
Maintains competence in multicultural issues and demonstrates respect and understanding for people from diverse cultural backgrounds.
Adheres to established policies and procedures for Behavioral Health services or Kid Clinic and Campbell County Memorial Hospital.
Actively engages in departmental quality assurance and utilization management activities and processes to include monthly chart reviews.
Provides community-based psychotherapy to individuals, groups, and families as applicable.
Participates in activities to promote mental health and/or the prevention of substance abuse, particularly among children and adolescents, as appropriate.
Must be free from governmental sanctions involving health care and/or financial practices.
Complies with the hospital's Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
Demonstrates working knowledge of state and federal laws pertaining to HIPPA, CFR 42 and 45, EMTALA etc.

Education
A master's degree in a mental health discipline or related field is required.
Licensure

JOB DUTIES (SUMMARY):

Assessment shall include pertinent information collection from all parties regarding alleged maltreatment, surrounding circumstances, adult and child functioning, parenting practices, and discipline to ensure an accurate safety determination. Visits are conducted in homes, schools, hospitals, and other community settings as required
Completes intake assessment of reports alleging child maltreatment by telephone and/or face-to-face contact. Collects sufficient information regarding the alleged maltreatment, surrounding circumstances, adult and child functioning, parenting practices and discipline. Utilizes interpersonal skills to professionally and respectfully engage each individual in sharing behaviorally-specific and detailed information. Responsible for information collection through hospitals, law enforcement, etc. to complete an assessment of child safety. Completes intake assessment on reports of elder abuse and/or neglect by telephone and face to face contact.
Researches previous agency history, criminal history, court involvement and other applicable information providing a comprehensive summary for all individuals associated with report. Maintain written documentation of assessment activities.
Clinically evaluates information obtained creating a screening recommendation by identifying the presence or absence of Present or Impending Danger threats or alleged maltreatment and/or a need for agency involvement. Review presence or absence of safety threat(s) in consultation with Intake Supervisor. Implement necessary safety intervention when applicable.
Prepares Request for Legal Action and provides court testimony.
Completes appropriate jurisdictional referrals to other Public Child Welfare agencies or law enforcement agencies as required. Participates in required Unit Meetings and Agency Trainings as required.
Performs other related duties as required. Attends required training and unit meetings.

CLASS CHARACTERISTICS
Social Worker I - Adoptions : This is the entry-level classification in the Adoptions Social Worker series. Initially under close supervision, incumbents learn and provide advanced social services casework in the area of adoptions, requiring coordination with county welfare departments, public and private social services, adoption agencies, public and private attorneys, judges, and other representatives of the court system. As experience is gained, assignments become more varied and complex; close supervision and frequent review of work lessen as an incumbent demonstrates skill to perform the work independently. Positions at this level usually perform most of the duties required of the positions as the Social Worker II - Adoptions but are not expected to function at the same skill level and usually exercise less independent discretion and judgment in matters related to work procedures and methods. Work is usually supervised while in progress and fits an established structure or pattern. Exceptions or changes in procedures are explained in detail as they arise.
Social Worker II - Adoptions: This is the journey-level classification in the Social Worker Adoptions series. Positions at this level exercise judgment and initiative in their assigned tasks and handle more difficult, sensitive and specialized cases and decisions are made where consequence of error may be significant. This class requires advanced casework skills, a high degree of program knowledge, judgement, and creativity. Understands and determines parent/caregiver protective capacities. Conducts interviews in homes, schools, hospitals, and other community settings as required. The Greater Nashville Regional Council (GNRC) is seeking a full-time Social Worker to serve as a counselor on its TennCare CHOICES Intake team within the Department of Aging and Disability Services. The Departments responsibilities include 1) planning and coordination of state/federal investments into programs and services aimed at improving quality of life for senior adults across the region, 2) coordinating services among a network of senior centers and providers, 3) providing counseling/ assessments to help Middle Tennesseans enroll in programs and services, and 4) monitoring the performance of grant-funded programs to ensure compliance with state and federal requirements.
GNRCs CHOICES Intake team helps to screen and assess older adults who may be eligible for the TennCare sponsored CHOICES program funded through Medicaid. CHOICES provides nursing facility services and home and community-based services for adults 21 years of age and older with a physical disability and seniors (age 65 and older).
Responsibilities of this positionmay include:
Help inform older adults about programs and services available to assist with long-term care, transportation, nutrition, or other needs as appropriate,
Conduct telephone and in-person screenings and assessments with clients across the Middle Tennessee area,
Complete pre-admission evaluations and collect financial and medical history information from potential clients,
Keep informed on aging and disability related issues and gather information about available resources and local service providers,
Help conduct periodic customer satisfaction surveys to assist GNRC and TennCare with monitoring the quality of the process,
Document information within the appropriate systems, and
Other duties as assigned by the CHOICES Intake Program Manager or the Director of Aging and Disability Services.
Qualifications:The ideal candidate would have abachelors degree in social work, with an active social work license, or LPN. Must have excellent verbal and written communication skills. Experience in geriatrics and case management strongly preferred. Bilingual Language Skills a Plus.Foreign degrees and credentials will be considered.
Required Technology Skills:Proficiency with Microsoft Office and Adobe Acrobat software is required.
Travel Expectations:This position will require travel throughout GNRCs thirteen county region. A valid drivers license and/or access to personal transportation is required.
Remote Work Flexibility:This is not a remote position. The selected candidate will be expected to work onsite on core business days. Work from home flexibility is provided, when possible, at the discretion of the Manager.
Compensation:The minimum starting pay will depend on experience and qualifications. GNRC offers a fully paid pension through the Tennessee Consolidated Retirement System and provides an optional 401k with dollar for dollar match of up to 3% of gross salary. Position Summary:
The Geriatric Social Worker will provide trauma-informed services to older adults to ensure client safety, dignity, and access to appropriate resources. The Geriatric Social Worker will work collaboratively with multidisciplinary teams to provide comprehensive assessments and supportive interventions to address elder abuse, neglect, and exploitation.
Principal Responsibilities:
Conduct in-depth assessments of older adults with identified abuse, neglect, or exploitation.
Evaluate risk factors, determine the immediate safety needs of the client, and conduct safety planning.
Perform home visits as necessary to gather information.
Provide supportive counseling, case management, and referrals to appropriate services (i.e. medical, legal, housing).
Assist with applications for emergency food cards and/or financial assistance as needed.
Work closely with law enforcement, healthcare providers, legal professionals, community partners, and Adult Protective Services.
Advocate for client s safety and assist with navigating systems (i.e. Family Court Order of Protection petition).
Conduct community outreach to promote awareness of elder abuse.
Maintain timely and accurate case notes and records in compliance with program standards.

Competencies:
Detail oriented and must be able to document all work in client data system in a timely manner
Patience and ability to manage clients with complex and multiple needs
Ability to work autonomously in the field and take the initiative to seek out information and resources that meet clients needs
Strong interpersonal skills
Comfort using online technologies and database systems daily.
Skill and Education:
LMSW (required).
Previous experience working with older adults and/or providing case management (preferred).
