Job Summary
This is a Registered Staff Nurse position with responsibility for reasonable and comprehensive care for all obstetrical patients. Accountable for the delivery of quality patient care, utilizing the nursing process for all patients.
Qualifications
Essential Job Duties and Responsibilities
* Collects and documents all data pertinent to each patient's actual and potential needs within twenty-four (24) hours of admission.
* Documents and reports significant findings of ongoing patient assessment.
* Formulates the patient plan of care based on acceptable standards of practice.
* Communicates the patient plan of care (written/verbal) through shift reports and other established procedures.
* Plans individualized patient care using nursing theory in collaboration with patient and other health care providers.
* Initiates patient plan of care, using assessment data upon admission based on age-specific needs.
* Incorporates in the patient plan of care the:
* Nursing diagnosis specific to the patient's actual problems/needs.
* Nursing Diagnosis which reflects complex etiology.
* Individual teaching plans using identified teaching needs.
* Nursing interventions consistent with nursing diagnosis and etiology.
* Implements patient's plan of care in collaboration with patient, family members and significant others.
* Communicates the initial plan of care and revision (written/verbal) through shift reports and other established procedures.
* Formulates discharge plans within twenty-four (24) hours of admission and as part of the revision process.
* Implements discharge plans by:
* initiating and completing discharge screening tool upon admission,
* completing appropriate referrals to other care services,
* implements individualized teaching plan based on actual discharge needs.
* Implements and documents teaching plan in the patient record.
* Evaluates outcome of patient care.
* Evaluates nursing interventions to changing patient situations as demonstrated by patient outcome, documentation in the patient record and revision of the plan of care.
* Prioritizes nursing interventions in complex and rapidly changing patient situations, as evidenced by the patient outcome and documentation in the record.
* Demonstrates proficiency in administering treatments, procedures and equipment appropriate to the unit/department as demonstrated by the skills checklist.
* Administers safely and documents medication regimen according to established procedures with no more than three (3) documented errors per year.
* Uses standard precautions as outlined in the Nursing Department Infection Control Manual.
* Participates in performance improvement activities by evaluating standards of care and initiating revisions as necessary.
* Demonstrates competency in the following areas by physical demonstration, verbal or written exam (medical equipment, age specific assessment and intervention, procedures such as IV therapy, medication administration, initiation of telemetry, etc.), as may be required in the performance of job duties and responsibilities.
* Participates in activities designed for professional growth.
* Attends all mandatory Hospital/Nursing Department in-service training sessions and meetings.
* Reviews and signs all in-service updates.
* Presents one (1) patient care conference and one (1) in-service training session relevant to the unit per year.
* Actively participates in 75% of staff meetings and Maternal Child Health Committee Meetings.
* Identifies own limitations in skills and knowledge as demonstrated by seeking guidance when needed from appropriate colleagues.
* Identifies and meets individual annual goals that are congruent with the Nursing Department's long- and short-term goals. Participates in decision-making process in one committee or one department-based project.
* Actively participates in own performance appraisal.
* Demonstrates leadership skills.
* Organizes own patient care and demonstrates ability to set priorities, providing most acutely needed care first and implementing physician and nursing orders within appropriate timeframes.
* Recognizes acute problems that occur in the unit and intervenes in a safe manner, in accordance with Nursing Department and Hospital procedure.
* Provides assistance to other staff members when needed and requested.
* Reports significant findings of ongoing occurrences to the appropriate Nurse Manager or Physician.
* Participates in the development and implementation of unit and Nursing Department goals and objectives.
* Participates in the orientation of new staff members.
* Maintains order by maintaining adequate stock of supplies and ensuring adequate forms for the clinical record, patient education, etc.
* Maintains equipment, participates in the recommendation and purchase of new equipment.
* Assists in the performance improvement plan by gathering data and recommending implementation of a plan of action.
* Maintains confidentiality of patient information in accordance with Hospital policy and applicable statutes and regulations.

Credentials and Experience Required
* Graduate of an accredited nursing school required.
* Current, valid Massachusetts Registered Nurse license required.
* BLS/CPR certification required within one week of hire. AHA & ARC accepted.
* ACLS preferred.
* NRP certification required upon hire.
* Breast feeding related certification required within one year of hire.
* At least two (2) years of prior Labor and Delivery experience preferred.
* One (1) year of prior, related medical/surgical experience preferred.
Special Requirements
* Must be available to work day, evening and/or night shift as well as rotation on weekends as assigned. Must be able to rotate to units as assigned.
* Must be willing to work additional shifts as assigned.
* Must be available to stay overnight should need arise during patient transports off island.
* May be exposed to the following hazards: temperature fluctuations, moderate noise levels, minimal dust, infectious diseases and other chemicals as customarily used in a hospital setting. Exposure to various hazards in handling patient transports.

Job Summary
The Perioperative Nurse assesses the patient preoperatively; plans care during the surgical intervention, implements those nursing actions and evaluate the outcomes. * Identifies and prioritizes activities that are necessary to meet patient needs.
* Evaluates the effectiveness of intraoperative measures taken, documents them and reassesses to take additional actions as needed.
* Communicates and collaborates with other members of the health care team.
* Maintains a procedural log as it pertains to the operating room and other units.
* Covers central supply responsibilities in times of illness/vacation and as needed.
* Ensures that appropriate supplies are available and replaced as used/needed.

RESPONSIBILITIES
As an Anticoag RN, your responsibilities will include:
* Delivering direct patient care and support, ensuring high nursing standards.
* Conducting patient assessments, devising care plans, and assisting in outpatient procedures with a focus on patient comfort and safety.
* Instructing patients and families about treatment plans, medications, and care following procedures.
* Collaborating closely with healthcare professionals to coordinate comprehensive patient care.
* Upholding infection control protocols and maintaining a safe environment for patients and staff.
* Demonstrates an understanding of the coagulation process, the role of Vitamin K, thrombogenesis, hypercoagulable states and antithrombotic therapy.
* Identifies indications for anticoagulation therapy.
* Identifies EBP goals INR for patients on warfarin.
* Communicates to AMS NP when indication for or goal INR is unclear or is outside of EBP.
* Understands the mechanism of action of warfarin and other anticoagulants used in clinic setting.
* Assesses and manages anticoagulation with respect to patient medications, disease states, diet and lifestyle/activity.
* Understands and properly uses the Point-of-Care meter
* Identifies need for and performs QC on the Point of Care meter
* Understands the meaning of prothrombin time (PT), International Normalized Ratio (INR), and International Sensitivity Index (ISI) and how they relate to each other.
* Provides care to patients at AMS INR clinics and the potential to travel to different INR clinic sites. Provides care to patients remotely (requires AMS CNP approval and MGB compliant device).
* Utilizes IDTF web portals for patients who perform self INR testing from home
* Utilizes Inbasket in Epic & external labs to obtain INR results.
* Interprets INR and adjusts warfarin dosing, using practice guidelines and clinical assessment and clinical judgment for each patient.
* Collaborates with AMS CNP when care falls outside of practice protocol, complex medication interactions, critical high or low INRs
* Provides ongoing education to patients regarding warfarin, other medications, health status, diet, activity and lifestyle
* Assesses for signs and symptoms of bleeding, thromboembolism, falls.
* Records all INR's, assessment findings, anticoagulation medication doses, next INR date in the medical record.
* Schedules INR appointments at the INR clinic(s) or lab. May adjust appointments within protocol for return INR to meet patients' needs, i.e. rehab, MD appointments, device checks, research, work/driving issues.
* Updates Epic &/ or the AMS episode with pertinent patient care information.
* Communicates to CNP when a patient is scheduled for invasive cardiovascular or other procedure, surgery.
* Assists with transitions in care anticoagulati
on management (e.g. hospital, STR, VNA/ home health care, hospice)
* Pends anticoagulation medication orders/ refills to AMS NPs
* Communicates and coordinates with other providers regarding mutual patients to provide continuity of care.

We are seeking a part-time, 20-hour RN to join our Beantown team! The team is flexible regarding which days these shifts occur between Monday and Friday and will work with you to accommodate your availability.
In this role, you will mainly perform admissions, ensuring accurate documentation and compliance with regulatory standards. You will provide support to other nurse case managers while collaborating closely with LPNs, therapists, and social workers. Visit numbers will range from 3-6 patients per day based on the needs of the team and the type of care provided.

Job Summary
The Registered Nurse position is at our Main Operating Room, it is a full-time 36 hours per week. The shifts are available M-F, generally 2 8-hour and 2 10-hour shifts at our Main Campus, Charles St Boston.
The Registered Professional Nurse maintains the standards of professional nursing practice and demonstrates competence for patient care through the utilization of the nursing process. The process includes continuous assessing, diagnosing, planning, implementing, and evaluating the patient care and patient education.
The Registered Nurse directs and guides ancillary personnel while maintaining standards of nursing practice.
Mass Eye and Ear Operating Department has 12 rooms in the main OR. This position will be caring for Ophthalmology and Otolaryngology patients.
The RN staff work collaboratively with interns, residents, fellows and attending physicians. Excellent observational skills and the ability to set priorities. Ability to function under stress with good interpersonal and communication skills.
Qualifications
Graduated from an accredited School of Nursing, baccalaureate degree in Nursing preferred.
Licensed and currently registered as a professional nurse in Massachusetts.
A minimum of 2 years of medical/surgical Nursing experience is preferred. - OR nursing experience required.
Related experience required for advanced clinical and specialty areas. Demonstrates leadership activities; able to work under stressful situation; and possesses well developed verbal and written communication skills.
Membership in a professional nursing organization desired.

As one of the largest service lines at BWH, there has never been a more exciting and rewarding time to join the Oncology team at BWH.
