Participate in staff recruitment, new hire orientation, mentor staff, and make recommendations for employee promotions, transfers and terminations. Qualifications:
Must be a graduate of an accredited school of nursing, college or university with a current registered nurse licensure by the State Board of Nursing.
A minimum of three years full-time or equivalent clinical experience is required and a minimum of two years of clinical experience in long-term care nursing with one year in a management/administrative or supervisory capacity is preferred. BSN preferred.
Must maintain current BLS/CPR certification and RN license.

We are seeking a Registered Nurse (RN) to join our team and provide skilled nursing care to patients in the comfort of their homes. The RN will administer care in accordance with physician orders and the individualized plan of care, under the supervision of the Director of Clinical Services/Clinical Manager. The role requires performing skilled nursing services on an intermittent basis, ensuring the highest quality of patient care.
Responsibilities:
Provide skilled nursing services in accordance with the plan of care.
Make the initial evaluation visit and regularly reassess the patient's nursing needs.
Initiate and update the plan of care based on patient evaluations and needs.
Administer specialized nursing skills as needed for patient care.
Initiate appropriate preventive and rehabilitative nursing procedures.
Document clinical and progress notes for each patient visit, and provide summaries of care conferences.
Coordinate patient services and inform the healthcare team of any changes in patient condition.
Counsel patients and families/significant others regarding nursing care and related needs.
Participate in and present in-service education programs for staff.
Follow established agency policies and procedures.
Process physician orders and notify the physician of any changes in the patient's condition. Complete necessary documentation (certification/recertification orders, discharge summaries).
Assess and determine the level of nursing care needed by each patient.
Refer patients to appropriate specialists (e.g., Physical Therapist, Occupational Therapist, etc.) when necessary.
Supervise and provide guidance to other nursing personnel (LPNs, Home Health Aides).
Conduct patient care conferences for assigned patients.
Participate in peer reviews and the Quality Assessment and Performance Improvement (QAPI) program.
Provide total patient care as required.
Take on-call duty for nights, weekends, and holidays as assigned.
Complete OASIS assessments, reassessments, transfers, resumptions of care, discharges, and any significant changes in condition within Agency-defined time frames.
Use appropriate ICD-10 codes for patient care documentation.
Working Environment:
Primarily works indoors in patient homes and the agency office, with required travel to/from patient residences.
