Responsibilities: The Social Services Specialist works with nursing center patients/residents and their family members/significant others to identify patients/residents' strengths, social, emotional, and mental health needs and then procures the services to meet those needs. This position provides patients/residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life. Services are provided in accordance with the National Association of Social Workers (NASW) Code of Ethics and compliance with federal, state, and local guidelines and regulations, policies and procedures, and standards of care. *Assists with planning and implementing a comprehensive Social Services program.
*Works with the interdisciplinary team to promote and protect patients/residents' rights and the
psychosocial well-being of all patients/residents.
*Complete a comprehensive Psychosocial Assessment for each patient/resident that identifies
social, emotional, and psychological needs and strengths. Assesses each patient/resident for
discharge.
*Identifies patient/resident discharge goals at admission and documents initial discharge plan and
involves the patient/resident and family throughout the process.
*Educates staff regarding the role of the Social Services in the facility and the psychosocial needs
of the patients/residents and their families/significant others including the problems of aging and

How you make a difference
The Social Worker Licensed Master's will work under the direction of the Manager of Rehab Program, Psychologist, and Social Worker Licensed Certified, and will be responsible for conducting screening, diagnostic evaluations, and providing therapeutic interventions to patients. This will include brief therapy and crisis intervention using evidence-based practices, while adhering to licensure standards, as well as state and federal rules and regulations. The Social Worker Licensed Master's will play a crucial role in delivering high-quality care to patients in need of social work services within the organization.
Key Responsibilities
* Conduct intake evaluations and assessments for patients referred by various sources, including security, healthcare staff, and community sources, and make appropriate recommendations for further care, including referrals for involuntary treatment or specialized housing/observation.• Develop and implement treatment plans, including individual and group therapy, psychoeducational groups, and substance abuse counseling, based on clinical findings and patient needs, and ensure regular updates in accordance with local policy.
* Provide crisis intervention and suicide prevention interventions as needed, assess for suicide risk, and provide consultation to security and healthcare staff on patients' behaviors, safety concerns, and mental health concerns.• Participate in treatment team meetings, classification meetings, and other required administrative meetings, as well as CQI and QA studies, and provide staff training on relevant mental health topics.• Document findings, diagnosis, treatment course, and other required documentation in patients' health records, and ensure compliance with Facility and company policies and procedures, including safety and mental health policies.

**PRINCIPAL DUTIES AND RESPONSIBILITIES:**
+ Patient Assessment / Care Planning / Counseling
+ As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.

+ In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care.
+ Knowledgeable of and adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation.
+ Documents based on MSW interaction and interventions provided to patient and/or family.
+ Quality
+ Provides psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level.
+ Participates in monthly Quality review meetings with the interdisciplinary team. Reports on quality indicators related to adherence, such as Missed and Shortened Treatments, Quality of Life Trends, and Service Recovery.
+ Patient Education
+ Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs.
+ With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
+ Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
+ Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
+ Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
+ Collaborates with the team on appropriate QAI activities.
+ Patient Admission and Continuity of Care
+ Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.

**PRINCIPAL DUTIES AND RESPONSIBILITIES:**
+ Patient Assessment / Care Planning / Counseling
+ As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
+ Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
+ Utilizes FKC patient education programs, established social work theory and methods, social work focused interventions, and quality of life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals.
+ Provides monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.
+ Provides supportive counseling services to patients as permitted within the scope of their clinical training and state license.
+ Provides educational and goal directed counseling to patients who are seeking transplant.
+ Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
+ Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
+ Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
+ In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care.
+ Knowledgeable of and adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation.
+ Documents based on MSW interaction and interventions provided to patient and/or family.
+ Quality
+ Provides psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level.
+ Participates in monthly Quality review meetings with the interdisciplinary team. Reports on quality indicators related to adherence, such as Missed and Shortened Treatments, Quality of Life Trends, and Service Recovery.
+ Patient Education
+ Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs.
+ With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
+ Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
+ Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
+ Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
+ Collaborates with the team on appropriate QAI activities.
+ Patient Admission and Continuity of Care
+ Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.
+ Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment.
+ The Social Worker will interview the patient to identify root causes or concerns for the discharge request, (i.e. transfer to hospice, relocation, dissatisfaction with services or staff) and share causes/concerns with operational leadership.
+ Insurance and Financial Assistance
+ Collaborates and functions as a liaison for patient with Insurance Coordinators to address issues related to insurance.
+ In collaboration with Insurance Coordinators, provides information and education to patients about payment to dialysis (federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs).
+ Collaborates with the Insurance Coordinator of any changes to patient state that impacts insurance i.e. transplantation, discharged, loss of coverage, or extended travel.
+ Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills
+ Staff Related
+ Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager.
+ Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
+ Provides training to staff pertaining to psychosocial topics as needed.
+ Contributes and participates with weekly team huddles. Discusses any urgent patient issues with staff.

This position has the primary responsibility of providing the full breadth of Child and Youth Behavioral (CYB) counseling services to children of all ages of military service members and their families at military installations and off military installations. These services may include non-medical counseling, training/ health and wellness presentations, consultation with parents, personnel, schools, and consultation to installation command regarding behavioral health issues affecting children and youth. CYB School counselors will be assigned to Department of Defense (DoDEA) schools and community schools and will need to have experience working in a school setting to qualify for a school position. CYB School counselors will also be assigned to youth programs, summer camps, and on demand assignments. The counselors work closely with the installation and military branch Points of Contact (POC) to assure that the program is provided within scope and meets the needs of the installation and the schools.
Provides non-medical, short-term, solution focused, counseling directly to eligible children and youth of service members at assigned primary and secondary locations. Services include assessment, brief counseling and consultation, action planning, referral to resources (assuring linkage as appropriate), and follow-up as indicated.
CYB counselors will also have a focus on supporting the staff and personnel of DoDEA , community schools, summer camps (if available/assigned) as well as providing parent training and guidance.
Provides training and health and wellness presentations, participates in health fairs and other base/installation activities focused on children and youth.
Enters counselor activity data daily through smart phone, or web application, to assure that reporting is accurate from assigned installation while maintaining confidentiality and anonymity of service/family member.
Establishes and maintains working relationships with community resources focused on children and youth and provides appropriate linkages. Consistently visible within the school which allows MFLC services to be accessible when working with children, the counselor must abide by line of sight protocol (another adult with responsibility for the child must be able to see the counselor and child they are working with at all times).
Partners with POC to provide CYB services in a manner that addresses the needs of the installation/facility. Develops an effective and professional working relationship with the installation/facility POC.
Manages duty to warn and restricted reporting situations according to DoD protocol and staffs the cases with Regional Supervisor/Regional Director.
Communicates with Regional Supervisor and participates in regular individual and group supervision, sharing information regarding trends and issues on the installations and in facilities to which they are assigned. Responds to critical incidents and special requests as directed by the POC and approved by the Office of Secretary of Defense (OSD) program manager. The CYB will flex schedule to respond to urgent requests as needed or at the request of the Regional Supervisor/Regional Director.
Participates in regular in-services/training, Quality Improvement committees, initiatives, studies, pilot programs other contract activities as assigned/appropriate as directed by the customer and/or Magellan. This includes participation in pilot program certification and training processes, completion of activity documentation, integration of pilot activities in non-medical counseling work, and the utilization of applicable technology to complete required activities.

Responsibilities
Master's Degree from an accredited Graduate Program in a Mental Health related field.
Valid unrestricted independent counseling license from a State, District of Columbia, US Territory or Commonwealth that grants authority to provide counseling services as an independent practitioner. A minimum two years of post-licensure clinical experience
Demonstrate intermediate-level competencies in technology and software (i.e., Microsoft Suite, Internet navigation, Smartphone/Tablet and application navigation, video-conferencing platform navigation).

ESSENTIAL JOB FUNCTIONS
1. Performs clinical services, including intakes and a variety of therapy programs and techniques such as group, individual, and family therapy, expressive therapy, crisis intervention, psychodrama and modeling.
2. Maintains and completes service documentation such as psychosocial assessments, case notes, case reviews, court reports, letters, etc.
3. Conducts case reviews and determines current and future treatment and treatment planning based upon resulting quantitative results and professional opinion of client progress.
4. Collaborates with external agents including CHR, school officials, family members, and other health agencies regarding organizational programs and client needs and conditions.
5. Attends and participates in staff meetings, treatment planning meetings, case conferences, and clinical seminars providing information relating to program administration and development.
6. Writes correspondence on behalf of clients and their families such as court reports, letters of recommendation, etc.
7. Conducts case presentations and consultations with other agencies regarding programs and services offered.
8. Formulates clinical diagnosis for both mental health and substance abuse.
9. Formulates a clinical treatment plan to be followed for interventions.

EDUCATION
· Master's degree in Social Work and licensed as a certified social worker (CSW)
· Understanding of Kentucky's board of social work laws and regulations regarding clinical practices or Master's degree in social services field with certification or licensure in area of specialization (e.g., LPCC, LPCA, LMFT, Marriage and Family Therapy Associate)
EXPERIENCE
· Three to six months of related internship or practicum experience.
· Knowledge of mental and physical characteristics of applicable population (e.g., growth and development and range of treatment needed), psychotherapy, human behavior, and treatment modes.
· Case load with varying levels of client disability and need
· Knowledge of community resources and SCS services
PHYSICAL DEMANDS/REQUIREMENTS
· Occasional exposure to threat of bodily harm from clients or their families
· Normal work tasks may involve exposure to blood, body fluids or tissue.
· Position at times may require incumbent to remain on twenty-four-hour call.
· Reliable transportation for frequent travel between work sites
