Serve as a resource for staff, families, and physicians on rehabilitation services and equipment. Patient Assessment: Collaborate with physicians to establish and evaluate treatment programs, plans of care, goals, and discharge plans. Develop a plan of care in partnership with the patient and/or family members. Create a realistic discharge plan incorporating the goals of the patient and/or family. Documentation: Document evaluations and treatment plans according to departmental guidelines. Record each treatment session following departmental protocols. Complete and submit family education, discharge, and other required documentation within facility and program guidelines.
