Delivers care appropriate for an outpatient, primary care FQHC with a multicultural, multilingual patient population. Duties and Responsibilities: · Examines, diagnoses and treats patients of all ages. · Performs initial history and physical examinations for outpatients. · Provides ongoing outpatient care for assigned patients. Screens outpatients to determine need for further care. · Prescribes medications within defined scope of practice. · Orders diagnostic studies (e.g.-x-rays, lab tests, ECG). · orders blood specimens for testing and performing other comparable lab procedures. · Initiates and expedites requests for consultation and schedules special tests and studies. · Provides education and counseling of patients and families in preventive care, medical conditions, and the use of prescribed treatments and drugs. · Performs all duties and services in full compliance with Joint Commission, Ryan White and funding source standards. · Completes all EMR required fields after each visit including the e-signing of notes. · Participates in on call coverage rotation · Willingness to float to multiple sites as needed with appropriate notice · Other duties as assigned. Education, Experience, Licensure/Certification and Skills/Abilities Related Requirements: · Must be in possession of a valid license to practice in the State of Texas.Basic Life Support (BLS) Certification · Have a thorough knowledge of the principles and practices of medicine and allied services commensurate with his/her training. · Demonstrate a special interest in the area of community medicine. · Ability to relate to culturally diverse patients and community. · Bilingual in English and Spanish preferred. · Minimum 2 years of pediatric experience or family medicine experience preferred Continuing Education and Training Requirements: Participates in trainings required by the funding source and/or as required by licensure. Role Overview The Psychiatric Nurse Practitioner is responsible for delivering comprehensive adult psychiatric and addiction treatment services, including active participation in our Medication-Assisted Treatment (MAT) and Medications for Opioid Use Disorder (MOUD) programs. Areas of Accountability This position is directly accountable for the following key areas: Conduct adult psychiatric evaluations, formulate diagnoses, and develop individualized treatment plans. Prescribe and manage psychotropic medications for adults and MAT/MOUD therapies, including buprenorphine, naltrexone, and other approved medications for those enrolled in addiction services. Provide follow-up care, medication management, and support for patients engaged in psychiatric treatment, substance use recovery and mental health treatment. Collaborate with an interdisciplinary team including physicians, therapists, case managers, and peer recovery specialists to ensure coordinated, patient-centered care. Perform risk assessments, crisis interventions, and safety planning as needed. Educate patients and families about psychiatric conditions, recovery principles, medication adherence, and harm reduction strategies. Maintain accurate and timely documentation in the electronic health record in compliance with organizational and regulatory standards. Participate in quality improvement initiatives, case conferences, and program development activities to strengthen Avenue 360's behavioral health and MAT/MOUD services. Education, Licensure/Certification: Master's or Doctoral degree in Nursing from an accredited institution. Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). Current, unrestricted license to practice as an APRN in the state of Texas. DEA license with X-waiver (or eligibility and willingness to obtain). Excellent communication and teamwork skills, with a commitment to health equity and culturally competent care. Experience, Skills/Abilities Related Requirements: Minimum of 2 years of clinical experience in psychiatry, behavioral health, or substance use treatment preferred. At least 1-2 years of direct clinical experience providing psychiatric care in outpatient, community mental health, or integrated primary care settings is preferred.     Job Description Under general supervision, provides independent acute and/or routine medical care to a specified outpatient population. May supervise support staff in and out-of-hours or remote clinical setting. Primary Responsibilities 1. Developing and implementing a treatment plan. 13. Monitoring the effectiveness of therapeutic interventions. 14. Offering counseling to meet patient needs. 15. Following meaningful use requirements while giving quality care to patients. 16. Requesting, receiving, and signing for the receipt of pharmaceutical sample prescription medications and distributing the samples to patients in a specific practice setting in which the physician assistant is authorized to prescribe pharmaceutical medications and sign prescription drug orders. 17. Signing or completing a prescription as authorized by Chapter 157, Texas Occupations Code. 18. Making appropriate referrals. 19. Other duties as assigned by supervisor. Education/Certifications/Licensure Bachelor's degree State of Texas Certified Physician's Assistant/Nurse Practitioner CPR certified. Must have a valid driver's license and a safe driving record and be able to pass a pre-employment physical and a criminal history background check. Skills Ability to perform medical examinations using standard medical procedures. Knowledge of patient care charts and patient history. Knowledge of Electronic Medical Records. Ability to react calmly and effectively in emergency situations. Skill in preparing and maintaining patient records. Knowledge of primary care principles and practices. Ability to maintain quality, safety, and/or infection control standards. Knowledge of drugs and their indications, contraindications, dosing, side effects, and proper administration.
