$20,000 / Two (2) Year Sign On Agreement* # SUMMARY:# Responsible and accountable for completing a physical therapy evaluation for each referred patient and for creating and implementing a physical therapy care plan based on the completed evaluation to restore a patient#s function, alleviate pain, and prevent disability. JOB REQUIREMENTS Education: Graduate of and degree holder from a Physical Therapy Education Program accredited by the Commission on Accreditation in Physical Therapy Education Preferred experience: 12 to 18 months related experience, excluding clinical affiliations required to complete a degree Licensure:# Current Arkansas PT license in good standing required.# Valid and clean Driver#s License, reliable transportation and agency required liability insurance to complete home visits.# Active personal automobile liability insurance coverage consisting of $100,000 per person and $300,000 per accident for bodily injury required.# # Certification:# Current CPR certification Preferred Education: Master#s or DPT Degree Other:# Ability to read, write and communicate effectively.# Basic computer knowledge required.# Driving required. Position Type and Expected Hours of Work This is a position in a home setting which is open 24 hours a day, 365 days a year. Direct patient care staff: Monday # Friday 8 hour shifts.
