Per Diem Hospital CaseManager
The RN Hospital CaseManager is a registerednurse responsible for the coordination of clinical care, quality, and financial outcomes of a designated population of patients. The Hospital CaseManager is responsible for overseeing an efficient plan that will ensure continuity and effective community or post acute reentry. The Hospital CaseManager leads the interdisciplinary team in developing complex discharge plans.
Responsibilities:
Facilitates patients care plan as called for in the plan of care
Leads daily patient care huddles with nursing to direct timely progress along the continuum
Works cooperatively as a leading member of the health care team to insure appropriate use of hospital resources, patient satisfaction, and achievement of quality of care. Manages input from all health care professionals and conducts assessments to determine patients health care and transitional needs.
Evaluates and ensures appropriateness of admissions and continued stay using evidence based criteria
Collaborates and manages outcomes with the payers through ongoing written and verbal communications to appropriate parties
Leads discussions with Physician advisors and MD's about safe plan for transitions
Leads the team to ensure appropriate utilization of patient resources and directs care to other settings when appropriate
Collaborates with community casemanagers and other health care advisors to ensure a smooth and safe continuum of care plan
Conducts extensive assessments and implements interventions for patients at risk for readmissions
Manages concurrent denials and works with physicians to overturn for appropriate reimbursement
Required Work Experience:
BSN Strongly preferred
3-5 years clinical Nursing experience
Hospital CaseManagement, Care Coordination experience strongly preferred
Skills and Competencies:
Requires a working knowledge of community resources and Utilization/Quality Review standards and activities conducted by third party payers
Ability to organize and prioritize workload in order to meet deadlines
Must possess excellent interpersonal skills in order to interact with all levels of health care providers, support staff, and third party payers where appropriate

The RegisteredNurse provides care that addresses the specific needs of age groups from infant to elderly and is also individualized to a variety of needs specific to impairments such as hearing, sight, substance abuse, non-English speaking, and potential victims of abuse. The RegisteredNurse incorporates knowledge and understanding of our commitment to patient rights, our mission vision and values into nursing practice.
The RegisteredNurse assumes full and ultimate responsibility for the quality of nursing care she/he provides to individuals and groups. Included in such responsibility is health maintenance, teaching, counseling, collaborative planning and restoration of optimal functioning and comfort or for the dignified death of those they serve. The RegisteredNurse, within the parameters of his/her generic and continuing education experience, may delegate nursing acuities to other registerednurses and/or health care personnel, provided that the delegating RegisteredNurse shall bear full and ultimate responsibility for the patient outcomes.
The RegisteredNurse must have excellent assessment skills, clinical skills, organizational skills, and computer knowledge and charge capabilities. The RegisteredNurse must be able to lift, transfer and move patients. Must be able to communicate verbally and in writing in the English language. Able to complete mathematical computations required to perform treatment protocols.
Requirements:
Education: Nursing Diploma, ASN or BSN (preferred); Ability to obtain BSN within 4 years required.
License/Certifications: Current RN MA license. BLS + ACLS required
Experience: 1-2 years of acute Med/Surg experience strongly preferred in hospital setting

Job Summary
The RN CaseManager (CM) facilitates a discharge plan including the coordination of post acute care/services for patients admitted to Medical, Surgical, Pediatric, or Obstetrical Services, and as needed, for hospital outpatient areas including but not limited to the Emergency Department and Henderson Surgical Center. The CM may also oversee the coordination of care for both inpatient and outpatient of those patients identified as high risk for readmission. The CM will develop and implement discharge plans for those patients identified as needing post acute discharge services or post acute follow up in collaboration with the patient/legal representative, health care team/provider, insurer, and others, including community based organizations, as needed. The CM uses a collaborative process of assessment, planning, facilitation, and advocacy. She/he may use a high-risk screening criteria or other acceptable screening tools to determine what patients may benefit from specialized care coordination services. The CM consults and collaborates with physicians regarding medical necessity of admissions and appropriate level of care for hospitalized patients utilizing nationally accepted criteria. He/she routinely reviews patients' health records and communicates with the health care team/provider to ensure continued appropriate level of care, compliance with Federal and State regulations related to discharge planning, compliance with CMS regulations or other third party payers and appropriate utilization of hospital and medical resources. The CM completes timely and accurate insurance reviews as requested by third party payers. The CM will serve as a resource for physicians, hospital staff, and patients and their families in providing information about insurance coverage, limitations of coverage related to discharge planning, community resources, community referrals and post acute care options. He/she participates in quality and risk managementcase finding activities. The role requires the ability to offer creative, problem solving solutions using sound and prudent clinical judgment and within the scope of licensure and according to casemanagement standards of practice. The CM seeks out educational and self-development opportunities related to care coordination, transitions of care, healthcare reimbursement, and other pertinent areas.
Qualifications
Education/Licensures/Certifications · Graduate of an accredited school of nursing. BSN preferred · Current licensure as a RegisteredNurse in the Commonwealth of Massachusetts, with minimum of three years' clinical experience · Evidence of continued education and professional development. ESSENTIAL RESPONSIBILITIES / DUTIES:
UM Reviews and Denial Support
* Perform Interqual Admission Assessments on all new admissions and forward the reviews to insurers as needed. Answer questions from the insurers and continue to provide any additional clinical information they request. Timely reviews to be provided so payers have sufficient time to review case and respond quickly.
* Communicate in real time with physicians on any patients not meeting criteria and establish a course of action. Work collaboratively with the MDs to help them understand documentation issues or any leveling issues. Inform physician partners on Inpatient vs. Observation criteria or acute care criteria.
* Act as liaison to managed care casemanagers for evaluating medical management of patients, referring questions to Medical Directors and/or payers when appropriate.
* Perform daily InterQual reviews on assigned patients and document when InterQual criteria is not met. Forward all reviews to insurers on a timely basis. Answer any questions from insurers. Perform concurrent denial management to resolve issues and ensure authorizations prior to discharge of patient. Inform Directors /designee regarding outcome.
* Upon receipt of admitting or daily denials from insurers, review the case and provide the insurer with additional clinical information for the insurers' reconsideration.
* Complete the clinical record and patient profile in Allscripts or a BMCHS-designated software tool. Discharge Planning and Execution:
* Review initial Admission Assessments and proposed discharge plans outlined by the SW Care Coordinator. Collaborate with SW Care Coordinator on discharge plan. Identify the patients/discharges that may be complicated and review these discharges with the Social Worker.
* Coordinate and monitor discharge planning activities for an assigned patient population and provide support as needed to the SW Care Coordinator and administrative staff managing the discharge process.
* Collaborate with the Interdisciplinary team to create an individualized discharge plan for high-risk patients, as needed, ensuring appropriate level of services are scheduled for the patient.
* Inform PCPs, attending physicians and clinical staff on alternative discharge options including high-tech home care, skilled nursing facility capabilities, and disease management initiatives in collaboration with SW Care Coordinators.
* Communicate pertinent patient information, on an as needed basis, with skilled nursing facilities, community health agencies, physicians and other staff to insure all post-acute clinical information is provided. Information to be provided on a timely basis to not delay discharge.
* Be aware of disease management programs and services in existence within BMCHS to use network resources, as appropriate.
Boston Medical Center South:
* Provide patient education and family teaching, on an as needed basis.
* Act as an advocate for the patient.
* Facilitate/coordinate multidisciplinary rounds on assigned patient care units, at a minimum of Monday - Friday.
* Attend UMCM meetings as appropriate

* Participate in the Hospital Quality Improvement Plan through unit and/or divisional quality control/quality improvement activities.

* Provide skilled nursing visits to patients and families, including informational or evaluation visits, admission visits, re-visits, or discharge visits.

* Follow the plan of care and carry out interventions established by the R.N. CM.
* Recognize and report changes in patient condition to the RN/CM or CTM as directed.
