• Performs medical procedures including, but not limited to: laceration repairs, nail removal, pap tests, injections, splinting and abscess incision and drainage
• Establishes and monitors a medically appropriate level of care for clinic patients
• Oversees, directs and supports on site laboratory, x-rays, injections, and collections for direct access services, if applicable
• Maintains clinical core competency
• Works with Center Leadership and other staff to ensure operations are consistent with medical policy and professional standards
• Cooperates and participates in the development, implementation and revision of policies affecting medical practice, quality of care and patient satisfaction
• Participates in and assists with in-service and continuing education programs for physicians, nurses and other personnel employed by or associated with Company
• Recommends potential means of growth and development for new and/or existing services within the Center
• Assures compliance with licensing, certification and accrediting bodies
• Participates in development and presentation of education programs for staff
• Represents Company and the clinic as reasonably required at internal and external meetings or events
• Provides guidance to other medical staff (e.g. physician assistants, nurse practitioners and medical assistants) to ensure quality professional services and patient satisfaction
WHAT'S REQUIRED?
• Minimum of 1 year experience as a Family Nurse Practitioner or Physician Assistant
• Active state license as an APRN or PA
• Current Family Nurse Practitioner (FNP) or Physician Assistant (PA-C) Board Certification
• Current DEA license, or eligibility to obtain upon hire
• Current DOT certification, or willingness to obtain upon hire
• Current BLS certification

POSITION SUMMARY/PURPOSE
Practitioners are responsible for providing physical exams, psychiatric evaluations and addiction treatment including: diagnosis; prescribing; coordination of care, and patient education. ESSENTIAL FUNCTIONS
1. Evaluate patients through detailed physical exam, a full review of psychosocial comprehensive assessments, and development of a detailed medical history.
2. Orders/performs appropriate laboratory diagnostic and other screening tests. Seeks other information as needed, including consultation with primary care physicians and medical specialists.
3. In collaboration with clinical and nursing staff, develops and implements a treatment plan that involves and includes the patient in active participation in treatment. And revises the plan as indicated.
4. Writes orders for medications, treatments and other matters pertaining to patients care.
5. Writes progress notes on patient's charts indicating patient status and treatment recommended.
6. Provides education to individuals or groups as appropriate and requested by the Medical Director.
7. Assumes care for physical and psychological needs of patients during examinations and provides emotional support to family members when appropriate.
8. Charts observations, test results and all patient communication in the medical record as directed by Rushford policy and procedure.
9. Ensures accurate maintenance of patient's medical records and that patient records are treated as confidential information.
10. Advises patients on use of various medication including dosage, side effects and composition.
11. Reviews and interprets results of diagnostic studies and tests, consults with supervisory physician when appropriate, incorporates information into plan of care.
12. Determines and discusses diagnosis with patient, documents information on charts including billing codes.
13. Recommends and develops new services and programs.
14. Participates in peer review, chart reviews, staff education, clinical guidelines development, and other continuing education and quality assurance activities to demonstrate compliance with standards.

Advanced Practitioner Role Scope
· The Advanced Practitioner will work at Hartford Hospital with The Center for Advanced Heart Failure team. Working closely with our Heart Failure Cardiologists, the Advanced Practitioner will provide comprehensive inpatient and outpatient care in our heart failure management program.
· This role is ideal for an individual who is comfortable working within a fast-paced clinical setting as part of a multidisciplinary group of providers including advanced practice providers, physicians, dieticians, nurses, and medical and administrative assistants.
· The outpatient heart failure management program incorporates a Bridge Infusion Program, HF Clinic, PA Sensor program, and VAD Program to actively manage heart failure related volume overload and reduce hospital admissions.
· The provider should exhibit a passion for cardiology and be open to a rapidly growing practice where adaptability is paramount.
Schedule:
· Four 10-hour shifts per week
· Some weekend coverage required (rotating basis)
· No overnight responsibilities
