The individual in this position must demonstrate appropriate knowledge of the principles of growth and development and nutrition through the lifecycle.
Essential Responsibilities: Performs nutrition screening and nutritional assessments on patients according to departmental policy.
Provides follow-up assessments and updates care plans as needed.
Individualizes patient meal patterns and menus to comply with the diet prescription and patient preferences in conjunction with the dietetic interns.
Develops and implements needed care plans.
Works closely with dietetic interns in monitoring and assuring optimal nutrition care to all patients.
Provide individualized diet instructions for patients/caretakers; conducts group classes when appropriate.
Consults with nursing staff and with appropriate interdiscipliary staff on individualizing diet instructions for patients and families.
Develops and coordinates a plan for follow-up nutritional care including completion of referrals to other agencies/facilities or a dietitian in the Medical Nutrition Therapy Clinic.
Serves as preceptor for dietetic interns during their clinical rotations, provides feedback for program improvement and performs intern evaluations in a timely manner.
Provides nutrition education/ training programs to dietetic interns, other health care professionals and the community as needed.
Complies with and utilizes pertinent education materials, clinical forms, policies and procedures.
Assists in developing patient education materials, clinical forms, policies and procedures.
Assists in establishing standard of care for patients.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Screens nutritionist status of each hospitalized patient to determine nutritional needs and the level of nutritional intervention required by reviewing the patient's medical record, especially the patient's medical history, drug history, and clinical laboratory data, and attending team rounds.
2. Performs nutritional and metabolic assessments based on clinical, medical and nutritional data in order to identify patients at nutrition risk. Interviews patients for nutritional history and relates problems to current medical history.
3. Assesses patients' nutritional status, develops medical nutrition care plans, monitors and documents the needs for those patients identified at nutritional risk, transitions eternally or parentally fed patients to discharge regimen. Monitors nutrient intake records and nutrient calculations for selected patients.
4. Reviews physicians' diet orders for appropriateness and recommends clarification or changes as indicated based on nutrition history, medical history, socio-economic factors and laboratory data.
5. Assists in development, implementation and monitoring of quality improvement standards for patient meal service delivery.
6. Educates both patient and families regarding modified or normal diets to meet the individual nutritional needs of the patient and documents counseling in medical record. Prepares patient education materials.
7. Develops and individualized diet plan (based on the evaluation) and counsels the patient and, if necessary, the family, including the planning, purchasing, and preparation of foods to meet the patient's individual needs.
8. Follows progress of patient on successive visits and makes appropriate changes in care plan.
9. Maintains the necessary patient records and statistics, including written evaluations, progress notes, etc.
10. Documents and updates nutritional status, care plan, and counseling in patient's medical record.
11. Provides nutritional consultation to the medical and nursing staff and other members of the health care team.
12. Acts as a resource to community groups regarding nutrition and gives presentations and lectures at professional meetings and to community groups.
13. Provides nutritional in service education to other members of the health care team.
14. Communicates nutrition information to patient service assistant to ensure optimal delivery of medical nutrition therapy.
15. Interacts with other members of the patient's health care team to ensure optimal nutritional care for each patient including discharge planning and continuing care conferences and continued nutritional support in the home environment.
16. Acts as preceptor for the FSNC nutrition interns (graduate students) during their clinical affiliations. Supervises their progress in evaluating patients, developing and implementing care plan, and counseling patients and families. Prepares teaching manuals, resources, etc.
17. Participates in the review, update and implementation of departmental policies and procedures, as assigned.
18. Participates in departmental quality improvement programs as assigned, especially those related to improving patient outcome.
19. Performs necessary administrative duties such as scheduling patient appointments, reviewing medical records and recording billing and insurance information.
20. Responsible for meeting assigned patient billing and volume goals.
21. Participates, as required, in nutrition-related studies and projects.
22. Assists in the training of new personnel.

What You'll Do:
Conduct nutritional screening and assessment for patients/clients to include anthropometric and laboratory data, collection of nutrition history, calculation of nutrition adequacy, and identification of drug-nutrient interactions.
Utilize motivational interviewing to identify desired goals and outcomes of nutrition care in collaboration with the patient/client and health care professionals. Considers the variables that may influence the patient/client's food-related behaviors, including culture and social determinants of health.
Evaluate and provide evidence-based nutrition recommendations to address patient/client medical needs. Understand and evaluate patient/client nutrition status and its relationship to complex disease states, including diabetes, cardiovascular disease, chronic kidney disease, gastrointestinal disorders, cancer, food allergies, and obesity.
Collaborate with physicians and other health care professionals to advocate for and implement optimal nutrition care.
Provide preventative services and counseling at individual patient/client visits and group meetings.
Serve as preceptor providing nutrition education and guidance to dietetic interns and students. Develops training materials and participates in mentoring of staff.
Assist in monitoring compliance to Yakima Valley Farm Workers Clinic (YVFWC) procedures, federal qualified health center quality metrics and/or state and federal WIC requirements.
Maintain awareness of community demographics and resources. Connects clients to resources.
Design and implements outreach activities to raise community awareness of program services.
Complete documentation in the appropriate electronic medical record to document patient/client interaction.
Actively participates in assigned meetings to promote care team collaboration and ongoing learning.

Qualifications
Master's Degree in Dietetics from a U.S. regionally accredited college, university, or foreign equivalent for new graduates after January 2024. Dietitians credentialed prior to Jan 2024 are allowed to be grandfathered in with a Bachelor's Degree. A Master's Degree in Dietetics serves as two years of experience for job offer purposes.
Completion of ACEND (Accreditation Council for Education in Nutrition and Dietetics) accredited academic coursework in a Didactic Program in Dietetics
Registered Dietitian or registration eligible (must obtain RD credential within 180 days of employment).

Position Summary
Works across the institution to provide quality nutritional support services. Consistently contributes to a multidisciplinary, collaborative effort to ensure positive patient outcomes. Documents Malnutrition diagnosis based on ASPEN guidelines.
Implements and monitors nutrition care based on: Nutritional Care: Screening, Assessment, and Plan for the Provision of - CL.0190.
Initiates / provides Medical Nutrition Therapy based upon Licensed Dietitian Ordering Privileges.
Collaborates with and communicates effectively with physicians and members of the multi-disciplinary team to integrate information which refines and contributes to the overall medical care of the patient, with specific emphasis on nutritional care, through routine rounds.
Timely and Accurately completes weekly productivity report.
Educates Medical Residents, Nursing and Nutrition Staff.
Participates in Food Service Quality Initiatives (Test Trays, Meal Rounds, GEMBA Meetings, reports concern to FSD and or CNM, maintains open communication with Clinical Nutrition Manager.
