· Assess patient and coordinate cares with hospital care team including MD, Case Manager, RN and hospice team.

Preferred Requirements:

+ American Heart Association - ACLS certification preferred.
+ American Association of Critical Care Nurses Progressive Care Registered Nurse (PCCN) certification or (CCRN) certification.
