Internal Title: Clinical Lead Care Manager
We're seeking an exceptional Clinical Lead Care Manager (CLCM) to join our team. Under the direction of the ECM Clinical Manager, the CLCM serves as the client's primary point of contact and works with all their providers such as doctors, specialists, pharmacists, social services providers, and others to make sure everyone is in agreement about the client's needs and care. Hybrid (in-person and remote) care management duties as described below:

Your experience & qualifications
Active LCSW, LMFT, LPCC, or LVN license in California required
1-2 years of experience as a care manager, care navigator, or community health worker supporting vulnerable populations.
2 or more years preferred.
Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely, with flexibility for potential evenings and weekends.
Working knowledge of government and community resources related to social determinants of health

Job Description
This is a full-time position that will be located at our new Child and Family Mental Health Clinic in Jackson Heights (Queens, NY).
Responsibilities:
Provide therapy to an ethnically and culturally diverse population of children, adolescents, and adults
Develop, implement, and review treatment plans
Write progress notes for all sessions conducted
Evaluate and assess clients
Maintain and update clinical records
Provide case management and/or crisis intervention

Qualifications

What You Will Do
You will report to the Associate Director of Adult Clinical Services. This position has flexibility for some telehealth and also in person. In this role you will provide mental health services to our clients and help support the staff with psychoeducation and professional consultation as it pertains to mental health. This position will also provide you the opportunity to share your knowledge of mental health through presentations and collaboration with other providers within and outside of RP.
Responsibilities include
Carry a full caseload and complete progress notes and other documentation in a timely manner each week.
Run curriculum-based groups, life skills and psychoeducation focused groups at various assigned sites and through telehealth.
Assist in shaping and developing curriculum through client feedback.
Visit assigned shelters and partnered sites (including transitional/reentry housing and jail/prison settings) as needed with possible travel to all Bay Area regions in order to maintain a level of support to clients and staff.
Conduct clinical intakes of new clients using a trauma sensitive approach and identify service needs when appropriate. Assessment includes identifying presenting problems, psychosocial history, mental status exam, and DSM-5 diagnosis.
Clinical interventions include: crisis intervention on-site at any of the shelters and/or while providing telehealth.
Clinical services also include short-term, trauma-sensitive individual and group psychotherapy as well as comprehensive clinical case management, liaison with other health care providers, and referral and accompaniment to community services when appropriate.
May visit clients at home, shelter and/or in the community for the purpose of outreach and follow-up if the client is unable to keep regularly scheduled appointments.
Caseload consists of complex clients, all of whom have histories of trauma, including acute and chronic physical and sexual abuse, and are victims of interpersonal violence such as sexual assault, physical assault, domestic violence, gunshot wounds, political torture, and immigration trauma. Many clients also have concurrent medical problems, psychosocial problems, and may have substance abuse problems as well. Clients also consist of those who have done harm and have had prior incarceration.
Utilize knowledge of the mental health, medical, and community resources in Oakland, San Francisco and other surrounding counties and maintain familiarity with new programs.
Develop a comprehensive treatment and service plan for each client, integrating knowledge from a wide range of human service resources.
Facilitate coordination of services among various agencies and complete required case management services to meet the client's needs.
All training as required by program grants must be completed.
Complete functional timesheets by assigned due dates in compliance with time allocations.
Actively participate in reflective supervision sessions.
Create and maintain documentation of processes and procedures related to job duties.
Perform other duties as assigned.

May provide supervision to clinical interns if meets the qualifications to supervise.
Participate in in-service training and case conferences.

Working Conditions
This position is eligible for hybrid in person & remote work. Time spent on-site versus remote working will be discussed and agreed upon with your supervisor. You are expected to be available on-site as necessary given business needs. You must be able to stand for long periods of time and perform physical activities, including lifting up to 25 pounds. You will be subject to frequent interruptions and must be able to multitask and handle stress. Local travel in the greater Bay Area may be required.

What You Will Do
You will report to the Adult Director of Clinical Services. This position has flexibility for some telehealth and also in person. In this role you will provide mental health services to our clients and help support the staff with psychoeducation and professional consultation as it pertains to mental health. This position will also provide you the opportunity to share your knowledge of mental health through presentations and collaboration with other providers within and outside of Restorative Pathways.
Responsibilities include
Carries a caseload of 15-18 clients continuously and completes progress notes for all clinical services in a timely manner.
Visits assigned shelters as needed with possible travels to all bay area regions on our mobile unit in order to maintain a level of support to clients and staff.
Conducts clinical intakes of new clients using a trauma sensitive approach and identifies service needs when appropriate. Assessment includes identifying presenting problems, psychosocial history, mental status exam, and DSM-5 diagnosis.
Clinical interventions include: crisis intervention on-site at any of the shelters and/or while providing telehealth.
Clinical services also include short-term, trauma-sensitive individual and group psychotherapy as well as comprehensive clinical case management, liaison with other health care providers, and referral and accompaniment to community services when appropriate.
May visit clients at home, shelter and/or in the community for the purpose of outreach and follow-up if the client is unable to keep regularly scheduled appointments.
Caseload consists of complex clients, all of whom have histories of trauma, including acute and chronic physical and sexual abuse, and are victims of interpersonal violence such as sexual assault, physical assault, domestic violence, gunshot wounds, political torture, and immigration trauma. Many clients also have concurrent medical problems, psychosocial problems, and may have substance abuse problems as well. Clients also consist of those who have done harm and have had prior incarceration.
Utilizes knowledge of the mental health, medical, and community resources in Oakland, San Francisco and other surrounding counties and maintains familiarity with new programs.
Develops a comprehensive treatment and service plan for each client, integrating knowledge from a wide range of human service resources.
Facilitates coordination of services among various agencies and complete required case management services to meet the client's needs.
All training as required by program grants must be completed.
Fill out a functional timesheet by assigned due dates in compliance with the time allocations.
Actively participate in reflective supervision.
Create and maintain documentation of processes and procedures in relation to duties of the job.

What You'll Do:
As a School Counselor, you'll serve as a trusted mental health professional for students across elementary, middle, and high school settings.
Responsibilities include:
Delivering on-site, school-based therapy services to students during school hours
Offering telehealth sessions (optional) to supplement caseload or allow for early onboarding
Conducting mental health assessments and developing individualized treatment plans
Collaborating with school staff, families, and the Backpack clinical team
Maintaining timely and accurate documentation in the EHR
Participating in supervision (if pre-licensed) and team case consults
What We Are Looking For:
A Master's degree in Counseling, Social Work, or a related field
Active Virginia license: LCSW-C/Supervisee in Social Work/LPC/LPC-R
Experience working with youth in schools, outpatient, or virtual settings
Strong clinical skills in individual and family therapy
Cultural competence and a trauma-informed care approach
Passion for making mental health more accessible to kids and teens
