The BY YOUR SIDE OccupationalTherapist (OT) is responsible for collaborating with multidisciplinary teams to create and maintain therapy plans that address sensory processing, attention, executive functioning, fine motor, gross motor, self help, ADLs, adaptive play and leisure skills.
Oversight for Occupational Therapy direct services will be provided by the Manager - Speech and Occupational Therapy. The OccupationalTherapist is responsible for defining goals, collecting data, establishing treatment plans, and communicating with families. Ability to implement skills obtained during trainings and adhere to the companies goals and policies.
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit; use hands to finger, handle, or feel and talk or hear. The employee is frequently required to reach with hands and arms. The employee is occasionally required to stand; walk; climb or balance; stoop, kneel, crouch, or crawl. Job Description
Participate in an initial multidisciplinary evaluation process to help evaluate new clients.
Evaluate, create, and maintain a comprehensive therapy plan for each client.
Provide individual therapy to clients focusing on a variety of areas including but not limited to: fine and gross motor, sensory processing, feeding and social skills.
Interact with and educate caregivers on helpful therapy approaches and in-home strategies to facilitate generalization of skills.
Provide team-based, collaborative approach for therapy planning and implementation.
Maintain current Illinois license, NBCOT certification, and necessary continuing education requirements.
Adhere to the ethical standards of the profession- AOTA and ILOTA guidelines and ensure HIPAA Compliance.
Supervise support personnel COTAs/OTAs and interns when necessary.
Other Duties and special projects as assigned.
Direct Therapy Responsibilities
Provide a minimum of 32 hours of therapy per week.
Maintain timely professional documentation of clinical notes for direct patient care provided.
Completion of documentation for all pre-authorizations, initial evaluations, assessments, insurance reports, and progress reports.
Qualifications
Master s or Doctorate Degree in Occupational Therapy.
State Licensed OccupationalTherapist.
Must complete and maintain certification in Safety Care training (Will train on site).
CPR/First Aid Certified or will train on site.
Knowledge of discipline specific assessment tools and procedures
Ability to think critically, solve problems, and deliver therapy independently.
Must possess strong communication skills, both written and verbal
Strong Interpersonal skills and the ability to work as a team member.
Excellent time management skills with the ability to multi-task, prioritize, execute on deadlines and produce high quality deliverables with attention to details.
Proficiency in Microsoft Office Products, Word, Excel, PowerPoint and Outlook.

Optimize the functionality and skills of patients
Responsible for the evaluation and treatment of residents after a written order received from physician
Screening of facility residents
Completes all necessary documentation
Attend all required meetings and in-services, including but not limited to family meetings, care plans, staffing, utilization review
In conjunction with Director of Rehabilitation, develop programs appropriate to the needs of the facility residents through the continuum of care
Compliant with all infection control procedures and environmental protocol while following OBRA Resident's Rights Guidelines
Provide consultation and clinical supervision to COTAs (may or may not apply)
Maintain valid state license and awareness of issues related to the profession of OT and healthcare environment
Responsible for appropriate scheduling of residents
Facilitates and completes family and facility staff in-services and education
Assist the facility in meeting standards necessary to maintain state and federal certification

Participate in an initial multidisciplinary evaluation process to help evaluate new clients.
Evaluate, create, and maintain a comprehensive therapy plan for each client.
