    1. Dresses, grooms, bathes, feeds, takes and records vital signs of patients. 2. Completes all ADL functions and documentation of such. 3. Answers call lights, assists in toileting, oral care, nail care, catheter care, assists patients in wheel chairs, transports, turns bedridden patients, reports patient changes, and takes out trash. 4. Requires certification as a Nurse Assistant; functions may not exceed Standards of Practice as accorded by certification. 5. Provides simple or uncomplicated patient care while caring for medically complex patients/residents requiring specialized treatment and services. 6. Bathes and dresses bed patients, combs hair, and otherwise attends to their comfort and personal appearance. 7. Cleans room and changes bed linen. 8. Takes and records temperature, pulse, and respiration rate. 9. Sterilizes equipment and supplies, using germicides or sterilizers. 10. Prepares food trays, feeds patients, and records food and liquid intake and output. 11. Must maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information; and adhere to Resident's rights. 12. Monitor, Observe, and report changes in patients medical status to Licensed RN 13. Provide care for post-operative patient, orthopedic patient, Neuro-compromised patient, cardiopulmonary patient, etc. 14. Maintains patient safety when caring for patients with oxygen therapy, IV Infusion Therapy, saline locks, traction, G-Tubes, NG Tubes, catheters, and CPM Machines. 15. Must be a supportive team member, contribute to and be an example of teamwork and team concept. 16. Must possess the ability to make independent decisions when circumstances warrant such action. 17. Must have knowledge of therapeutic diets and distributes meals/nourishments according to the specific order. 18. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. 19. Must possess the ability and willingness to work harmoniously with other personnel. 20. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. 21. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. 22. Must be able to relate information concerning a resident's condition. 23. Performs other work related / personal nursing care duties as requested. 24. Challenges current working practices; identifies process improvement opportunities and presents recommendations and solutions to management. Engages and commits to the organization's culture of continuous improvement by actively participating, supporting, and promoting CCHC Pillars of Excellence. Must be able to read, write, speak, and understand the English language; Must possess, as a minimum, a 10th grade education; Current Massachusetts Nurse's aide certification; Basic Life Support Certification for Health Care Providers within 30-days of start date; Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served; Strong interpersonal, customer service, time management skills; Experience with current office technology, proficient with computer office applications (Electronic Medical/Health Records, MS Office, etc.) preferred.

Our patient population is diverse and includes individuals experiencing: * Acute, complex medical conditions * Heart failure * COPD exacerbations * Pneumonia * Cellulitis * Dehydration These patients may require close monitoring, mobility support, assistance with daily personal care, and compassionate presence as they stabilize and recover in the comfort of their own homes. You will support patients across the North Region, including: Beverly, Salem, Peabody, Lynn, Melrose, Medford, Somerville, Lexington, and surrounding communities. Job Summary What You'll Do In this role, you'll assist patients with daily personal care needs, including bathing, dressing, toileting, ambulation, and other essential activities that promote independence, safety, and comfort. You will follow individualized care plans, document your support accurately, communicate clearly with families and clinical teammates, and report any concerns promptly. Your day will include traveling between patient homes across the North Region. Assignments are sent the night before, giving you the chance to plan your route and prepare for the specific needs of each patient.
