SLH-ICU/SICU is part of a Level II Trauma center and is comprised of 24 beds. Our Critical Care Unit provides exceptional care to a multitude of critically ill patients. This includes hemodynamically unstable patients of varying diagnoses including, but not limited to: acute respiratory failure, acute MI with and without intervention, cardiogenic shock, heart blocks, life threatening arrhythmias, sepsis/septic shock, renal failure, DKA, acute CVA, hypertensive crisis, acute GI bleed, multiple trauma, multi-system organ failure and major surgical cases including vascular and neurosurgical services.

We are searching for a talented RN Per Diem, Level I
Hours: Per Diem
Shift: Level I per diem nurses will normally be required to be available to work a minimum of forty-eight (48) hours within a six (6) week time schedule, including two (2) weekend shifts or one (1) Weekend and one (1) off-shift. Such weekend or off shifts must be a minimum of eight (8) hours each. The forty-eight (48) hours that a per diem nurse must be available to work will be available to the per diem nurse from the open shifts in the schedule after full and part-time nurses have indicated their preferred schedule. Per diem nurses will be required to provide availability to work two (2) holidays per year, one (1) summer holiday from Memorial Day, Independence Day, and Labor Day, and one (1) winter holiday from among Thanksgiving Day, Christmas Day, and New Year's Day. Such holiday shifts must be a minimum of eight (8) hours each.

Job Summary
The registered nurse renders highly professional and technical nursing care to assigned patients. Patient Assessment and Monitoring:
Conduct detailed assessments of patients with neurological conditions, including vital signs, neurological exams (e.g., Glasgow Coma Scale, pupil response, limb movement), and monitoring for signs of intracranial pressure (ICP).
Continuously monitor for changes in neurological status, including altered consciousness, pupil reaction, motor function, and speech.
Utilize advanced technology and equipment to assess patient status, such as intracranial pressure (ICP) monitoring devices, electroencephalography (EEG), and cardiac monitoring.
Critical Care Management:
Administer prescribed medications, including pain management, sedatives, anticonvulsants, and intravenous medications, ensuring correct dosages and monitoring for side effects.
Provide specialized care for patients undergoing procedures such as strokes (tPA administration), neurosurgical interventions, brain injury management, or spinal cord surgery.
Monitor and manage ventilator settings for intubated patients, ensuring proper oxygenation and ventilation.
Manage patients with external drains, such as ventriculostomies (for ICP monitoring), and ensure proper positioning and drainage.
Neuro-Assessment and Interventions:
Perform neurological assessments using appropriate tools (e.g., Glasgow Coma Scale, pupil reactivity, motor and sensory function).
Monitor for signs of deterioration, including increased ICP, seizures, or changes in consciousness, and intervene as necessary, following established protocols.
Administer anti-seizure medications for patients at risk of or experiencing seizures and ensure continuous monitoring.
Provide hyperventilation or other interventions to decrease ICP if necessary.
Family Education and Support:
Offer support and guidance to families of critically ill patients, explaining the nature of the neurological condition, prognosis, and treatment options.
Provide education on the care process, expected outcomes, and potential complications, ensuring families are informed and involved in decision-making.
Address emotional needs of family members, providing a comforting presence and helping them cope with the uncertainty of critical illness.
Collaboration with Multidisciplinary Team:
Collaborate with neurologists, neurosurgeons, intensivists, respiratory therapists, physical therapists, and other healthcare providers to deliver comprehensive care for neurocritical patients.
Participate in case discussions and rounds to provide input on the patient's condition, treatment plan, and progress.
Communicate effectively with the team to adjust care plans based on the evolving needs of the patient.
Post-Operative and Recovery Care:
Provide post-operative care for patients who have undergone neurological surgeries, including monitoring for signs of complications such as hemorrhage, infection, or changes in neurological function.
Assist in the rehabilitation planning process by preparing patients for further care, such as transitioning to a step-down unit, long-term acute care, or rehabilitation facilities.
Monitor for complications related to immobility, such as pressure ulcers, and take appropriate preventive measures.
Documentation and Compliance:
Accurately document all patient assessments, interventions, medications, treatments, and changes in condition in the electronic health record (EHR).
Ensure all documentation is detailed and complies with hospital policies, nursing standards, and regulatory requirements.
Maintain up-to-date records for all patient interactions and treatments, ensuring clear communication between team members.
Emergency Response and Crisis Management:
Respond quickly to emergency situations, including stroke alerts, seizures, and deteriorating neurological conditions.
Provide advanced life support (ALS) and manage critical neurological emergencies with a focus on stabilizing the patient and preventing further brain injury.
Participate in rapid response teams and code blue situations, providing immediate care for patients in critical need of intervention.
Patient and Family Advocacy:
Advocate for the patient's best interests by ensuring the care plan aligns with their clinical needs and family preferences.
Provide guidance to families on long-term care planning, including possible neurological rehabilitation, home care, and follow-up treatments.
Professional Development and Education:
Participate in continuing education and training related to neurological care, critical care nursing, and advancements in neurology and neurocritical care.

In this Float - Nurse Practitioner job, float nurse practitioners care for patients at 15 Primary Care practices, which include Family Medicine, Health Centers, Pediatric Care and Adult Internal Medicine.
The practice locations range from Main campus in Boston, and in nearby towns which may include Jamaica Plain, Chestnut Hill, Brookline, Norwood, and Foxboro. Patient population is primarily adult however may include some pediatrics at certain practice sites.

Job Summary
GENERAL OVERVIEW
The Nurse Practitioner is an advanced practice registered nurse who has acquired specialized knowledge and skills through completion of a nurse practitioner program. The practitioner has the credentials to function in an expanded nursing role and is responsible for the daily management of selected patients. PRINCIPAL DUTIES AND RESONSIBILITIES
A. Patient Care Organization
1. Provides direct care, counseling, and teaching to a select patient population.
2. Performs physical examinations and provides preventive health measures appropriate to patient needs.
3. Orders, interprets, and evaluates appropriate lab and diagnostic tests to assess patients' clinical problems and health care needs.
4. Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab, and physical assessment findings.
5. Performs laboratory tests appropriate for specific patient population.
6. Orders medications and writes prescriptions according to organizational and regulatory policies and procedures.
7. Records pertinent patient care findings in the medical record.
8. Sets priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment.
9. Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals.

B. Quality of Care
1. Assists in the development, implementation and revision of nurse practitioner guidelines and monitoring requirements for prescription writing in collaboration with their designated physician and Nursing Director or Executive Nursing Director.
2. Evaluates outcomes of own patients' care and makes necessary revisions in care plans.
3. Seeks and provides consultative services as required.
4. Participates in departmental and organization quality improvement program.
5. Utilizes quality improvement data and research findings to enhance patient care.
C. Coordination
1. Demonstrates appropriate collaborative and consultative relationships with other providers within the institution and community.
2. Serves on Nursing and Hospital Committees and assumes responsibility for disseminating and communicating information to staff.
D. Budget
1. Participates in setting productivity metrics and in cost effective management of personnel, time, supplies, and equipment in his/her area.
2. Collaborates with the Nursing Director/Practice Manager/Executive Nursing Director in the recommendation of appropriate staffing patterns for their clinical areas.
E. Personnel Management
1. Participates in selection process of new employees in group or team practice as requested.
2. Provides input into evaluation of other staff as appropriate.
F. Education
1. Acts as preceptor for nursing students and staff as requested.
2. Participates in orientation of new staff.
3. Participates in Departmental and organization continuing education programs.
4. Identifies own learning needs and updates clinical knowledge and skills through formal and informal education and review of current literature.
5. Educates lay and/or professional groups through lectures, papers, publications, and educational materials.
G. Planning
1. Develops annual objectives and reviews with Nursing Director or Executive Nursing Director and collaborating M.D. at time of performance review and at appropriate intervals. Completes annual credentialing process through the Nursing Department credentialing program.
2. Assists in determining, evaluating, and/or implementing clinical area goals and objectives.
3. Maintains interdisciplinary approach for planning and communicating information for patient care and operation of practice.
4. Maintains current knowledge of legislation that may affect the health care delivery system.
H. Research
1. Participates in nursing research at a level consistent with educational background and experience.
2. Maintains understanding of current research of others in related clinical areas through review of relevant publications.
3. Participates in grant writing as appropriate.
Qualifications
Education:
* Master's in Nursing (MSN) (or) DNP (or) PhD Degree in Nursing required.
* Completion of a Master's or post Masters' degree in Adult or Family NP program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).
* Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioner.
* Current Adult/Gerontology (AGNP) or Family Nurse Practitioner (FNP) Certification by an accredited APRN certifying body required.
* Current APRN License to practice in Massachusetts Required.
Experience:
* 1. Five (5) + years of recent experience in office-based Primary Care as an NP required.
* 2. Bilingual English/Spanish preferred.
* 3. Pediatric experience preferred.
* 4. Demonstrated ability to care for a diverse patient population, including acute and chronic disease management as an NP required.
* 5. Demonstrated ability to care for a broad range of ages as a nurse practitioner required (ages 16 and up).
* 6. Previous experience working in a float or temporary/seasonal position required.
* 7. Experience managing inter-visit care via asynchronous modalities including phone visits, electronic communication and in-basket management as an NP required.
* 8. Prior experience working in a team-based care environment required.

The Registered Nurse is a professional nurse registered in Massachusetts who is responsible and accountable for planning and providing patient care for assigned patients in accordance with Nursing Standards of Care as set forth in the Nursing Department's Clinical Practice Manual. The registered nurse demonstrates initiative, knowledge and clinical skills in caring for the patient with complex needs. The Registered Nurse demonstrates the ability to effectively manage patients by assuming full responsibility for the assessment, plan, implementation and evaluation of patient care and is directly responsible to a designated nurse manager, or supervisor.
Qualifications
Education:
* Graduate of an approved school of nursing with current registration in Massachusetts.
* For newly licensed nurses a Bachelor of Science Degree in Nursing is required
Experience:
* 2 years medical-surgical nursing required
* Critical care nursing experience preferred
* Neuroscience nursing experience preferred
Other:
* Membership in professional organizations is recommended and certification in a specialty area is encouraged.
* Able to work weekend rotation per the MNA contract
