The Per Diem - CNA performs personal care, homemaker assistance and defined nursing procedures for patients and their caregivers while under RN direction and supervision.
Qualifications:
High school diploma or GED or an equivalent combination of education and experience
Current certification as CNA in the state in which employee will be working
Minimum of six (6) months of CNA or Home Health Aide experience preferred
Active BLS for healthcare professionals from the American Heart Association or Red Cross.
For current Hope Employees: BLS should be obtained by January 1, 2025
For current Capital Caring employees: BLS should be obtained by November 1, 2024
Mobile Driver - Valid driver's license and automobile insurance per Company policy
Reliable transportation to meet work schedule
Ability to use equipment with visual and auditory mechanisms
Ability to communicate (written and verbal) in English
Ability to manage the emotional stress of working with and caring for ill patients and their families
Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation
Must be able to physically access any home or facility in assigned area (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning)

Job Responsibilities:
Maintains competencies/contact hours as required by Company and regulations.
Provides only patient/caregiver services according to written Aide/Caregiver Plan of Care and Company policies and procedure.
Practices safe and proper body mechanics relative to moving, positioning and transferring patients.
Assists patient and or caregiver in maintaining a clean and safe home.
Provides caregiver relief as part of the Plan of Care.
Transports supplies, lab specimens, documents and other items needed to meet the Plan of Care as requested by the RN.
Under the direction or supervision of an RN, the Aide may perform activities that promote activities of daily living.
Contributes to the development of the Plan of Care by reporting observations to RN.
Communicates any significant issues or symptoms to the RN or Clinical Manager.
During times of emergencies (i.e. Hurricanes, etc.), the LPN may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency
Performs other duties as assigned.
Some locations may require:
Cross training to field (home/facility/admission) positions of equal licensure/certification and scope when hospice house census calls for flexible staffing.

Compensation Pay Rate:
$20.00

**Job Description:**
Follows CDC isolation guidelines, demonstrates appropriate use of PPE, performs hand hygiene according to policy, and enforces safety regulations. Assists in the delivery of patient care by performing tasks under the direction of a registered nurse. Documents and reports all procedures accurately according to department policy. Participates in obtaining nursing history and evaluates age-appropriate behavior, motor skills, and physiological norms, reporting abnormalities to the registered nurse. Performs treatments according to policy and procedure, providing care appropriate to age, mobility, acuity, and culture, and functions appropriately in clinical emergencies. Participates in practice changes from performance improvement activities and supports quality standards and initiatives. Evaluates patients' developmental level, identifies increased physical/emotional dependency needs, and assesses the availability of support systems. Exhibits a desire to learn and upgrades skills.
**Knowledge, Skills, and Abilities:**
- Must demonstrate ability and understanding of the age-specific care requirements of geriatric, adult, adolescent, and pediatric patient populations, including appropriate selection of patient equipment, communication, and safety needs.
- Requires close attention and mental effort in executing multiple assigned duties, such as recording blood pressures, input & output (I & O), obtaining and documenting blood glucose monitor results, and distributing and collecting dietary trays.
- Ability to float between assignments and adapt to varying healthcare settings.
- Knowledge of regulatory requirements and compliance with all safety and infection control policies to prevent injury to patients, employees, or self.
- Effective communication skills, both verbal and written, and the ability to document information accurately.
- Demonstrated ability to remain calm and focused in stressful and/or emotionally charged situations.
- Strong organizational skills with demonstrated ability to be highly detail-oriented.
- Demonstrated customer service skills with a patient-centric approach.
- Basic hands-on computer knowledge and working knowledge of using computers for data entry.
- Ability to move or lift patients and equipment, and willingness to work a flexible workweek including weekends, nights, and overtime as required.
- Commitment to purposeful hourly rounding and efficient, effective interaction with a wide variety of individuals.
- Knowledge of and ability to perform accuchecks and hemoccults, obtain vital signs correctly, and knowledge of normal parameters for vital signs for age-specific populations.
- Ability to remove peripheral IV catheters, empty Foley bags, JP's, and hemovacs.
- Excellent analytical and problem-solving skills, effective oral and written communication skills, with the ability to articulate complex information in understandable terms to all levels of staff.
- Effective computer skills, particularly Microsoft Office Suite, and the ability to work in a matrix-management environment to achieve organizational goals.
- Requires judgment to ask for supervisory input when needed and adherence to efficient time management and utilization of patient resources.
- Interfaces with fellow employees, physicians, and patients continually, and with external contacts to establish good relationships with community agencies.
- Works in a modern well-equipped environment with exposure to bloodborne pathogens; may encounter aggressive and violent outbursts and physical harm from non-compliant patients.
- May be required to work weekends, irregular hours, and holidays, with the possibility of extended hours. Subject to prolonged extensive or considerable standing/walking and occasionally lifting supplies/equipment. Personal protective equipment required.
- Must meet physical requirements per hospital standard and have accurate typing skills.
- Medical terminology knowledge preferred.

**Field of Study:**
- Must be currently enrolled in an accredited school of nursing program and has completed the first semester of Nursing Fundamentals OR has completed an accredited nursing program within one (1) year and has not obtained their license for Registered Nurse or Licensed Practical Nurse.
