The Nurse Practitioner (Primary Care) provides quality patient care in a variety of settings in collaboration with and under the direction of the Attending Physician. The Nurse Practitioner follows established nursing and organizational standards, procedures, and practice. This position gives specific patient care directions to nursing and other staff and provides education and support for patients, families and staff members.
Location: Washington, DC/Maryland - Home Visits in the DC/Maryland area are required - In person - Not Remote

Qualifications
Experience Requirements
Must have one year of NP experience in primary care or geriatrics, 2 - 3 years is desired. Home or Community-based experience, preferred.
Education Requirements
Must be a graduate of a Masters or Doctorate-level Nurse Practitioner program accredited by the American Association of the Colleges and Nursing (AACN) or the Commission on the Collegiate Nursing Education (CCNE).
Licenses
Must have an active NP license in the state of service. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. GAPNA certified GS-C is preferred but not required.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.

LICENSURE/CERTIFICATION

The Advanced Practice Provider (APP) will provide comprehensive outpatient care to patients under the supervision and collaboration of interventional cardiologists. The APP will evaluate, diagnose, and manage patients with cardiovascular disease, focusing on continuity of care before and after interventional procedures. This role emphasizes outpatient clinic management, coordination of patient care, and integration with inpatient and procedural services to optimize outcomes and patient satisfaction.
Key Responsibilities
Clinical Care
Conduct new patient consultations, follow-up visits, and post-procedure evaluations in the outpatient clinic.
Perform focused histories, physical examinations, and order/interpret diagnostic test results (e.g., echocardiograms, stress testing, vascular studies, labs).
Supervise cardiac stress tests (training provided if not experienced)
Manage cardiovascular risk factors and chronic conditions (hypertension, hyperlipidemia, CAD, PAD, heart failure) under established protocols.
Provide pre-procedure evaluation and education prior to interventional cardiology procedures.
Deliver post-procedure follow-up, including medication adjustments, wound care, and complication monitoring.
Collaborate with supervising physicians in the formulation and execution of treatment plans.
Care Coordination
Facilitate seamless transitions of care between hospital, cath lab, and outpatient clinic.
Communicate with referring physicians and primary care providers regarding patient progress and treatment plans.
Assist with triage of referrals to the interventional cardiology clinic, prioritizing urgent cases (e.g., unstable angina, critical limb ischemia).
Coordinate acquisition and review of outside medical records and imaging.
Education & Counseling
Educate patients and families on disease processes, procedures, lifestyle modification, and medication adherence.
Provide counseling on cardiovascular prevention and rehabilitation strategies.
