Job Summary
We are currently seeking exceptional applicants for a per diem position as a Supervising Physician. As a Carbon Health Supervising Physician, you will join a growing community of healthcare providers and support a team of Advanced Practice Clinicians in delivering compassionate, evidence-based care. You will enjoy flexible scheduling working in-clinic as a per diem physician at minimum 2 shifts per month with the remainder of the time providing supervision remotely.
Responsibilities and Duties
Work in-clinic 2 shifts per month
Be available for messaging or phone consultations (usually between 8am-8pm) for urgent care or primary care cases from Advanced Practice Clinicians (not patients) within 15 minutes of request. No need to be present in clinic for consultations.
Conduct monthly or quarterly in-person or virtual visits with Advanced Practice Clinicians as needed per state guidelines to provider individualized feedback and teaching based on consultations and chart reviews
Review and sign 5% of charts as listed in your task list each day you are on call (signatures are expected within 72 hours of shift)
Review and sign 100% of charts for which controlled substances were prescribed or administered (signatures are expected within 72 hours of shift)
Provide chart reviews as mandated by individual state legislation
Escalate any concerns to the Clinic Medical Director or Area Medical Director
Qualifications and Skills
BC/BE MD or DO - Family Medicine or Emergency Medicine
Demonstrated excellent written/verbal communication skills
Active medical licenses (in good standing)

We are seeking a dedicated and collaborative Physician specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team. This new role is designed for an American Board of Internal Medicine (ABIM) certified Endocrinologist with demonstrated expertise in obesity medicine, comprehensive diabetes management, and a strong interest in supporting endocrine Advanced Practice Providers (APPs) in providing high-quality patient care.
The physician will currently provide direct clinical services and structured supervision for up to three Endocrine-Specialized APPs. A critical aspect of this role is ensuring the safe and effective management of patients with complex co-morbidities, including Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Cardiovascular conditions. The physician will help enhance the consistency, safety, and effectiveness of patient care across the team, specifically integrating the use of Continuous Glucose Monitors (CGMs) and insulin pump therapy.
This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.
This is a full-time position based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.
What You Will Do:
Provide direct patient care in a telemedicine clinical setting, focusing on evidence-based treatment for obesity and comprehensive diabetes management, including insulin titration and care of patients who may already be using insulin pumps and/or continuous glucose monitors (CGMs).
Collaborate with APP colleagues and Registered Dietitians specializing in diabetes treatment, primarily Type 2 Diabetes, to co-manage a shared patient panel.
Review charts and conduct initial and return visits for APP patients every third visit to ensure quality, continuity of care, and appropriate management of CKD/ESRD and CAD/CV conditions.
Provide advanced treatment planning and medical management support for complex endocrinology cases, escalated concerns, and patients requiring specialized management protocols.
Prescribe non-controlled medications, including those for obesity and diabetes, for patients under both direct care and in coordination with APPs.
Conduct weekly 30 minute collaborative meetings and 30 min asynchronous communication time with each assigned Endocrine-Specialized APP to review cases, address clinical challenges, and provide ongoing mentorship.
Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team, with a focus on guidelines for managing complex diabetic populations.
Assist in the professional development of colleagues by fostering clinical growth, offering feedback, and supporting continuing education in both endocrinology and obesity medicine.
Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.
Attend relevant departmental meetings and contribute to quality improvement efforts within the diabetes and obesity medicine program.
About You:
Required: Board certification in Endocrinology by the American Board of Internal Medicine (ABIM)

Be an integral part of the treatment team for each individual who consults with all staff on coordinating services and contacting all existing treatment providers.
Work with peer providers on developing discharge plans for each individual that link them to post-discharge providers.
Facilitate groups on social behavior, peer interactions, stress tolerance, positive coping skills, medication management, substance use and relapse prevention, and symptom reduction.
Provide individual crisis intervention services.
Assist individuals with developing and staying connected to a positive support system.
Be responsible for coordinating group activities for house members.
Reassess individuals' needs on a daily basis including evaluating mental status and assessing for suicidal ideation, homicidal ideation, and audio/visual hallucinations.

CLINICAL

QUALIFICATIONS AND REQUIREMENTS
Degree(s): M.D. or M.D., Ph.D. in a field appropriate to faculty role.
Board Certification: Board certification in Anatomic Pathology/Clinical Pathology or Clinical Pathology is a requirement. Board specialty certification by The American Board of Pathology in Transfusion Medicine or Transfusion Medicine board eligible is also required, with achievement of Transfusion Medicine board certification within 3 years of employment if of board eligible status.
