The Providence Center Case Manager Young Adult ACT II Team (YAHH) will provide case management services for young adults aged 18-25 with mental health and/or co-occurring disorders and their families in the greater Providence area. Will assess, plan, develop, coordinate, and provide treatment and supportive services to young adults with mental health issues.
Must meet Health Home diagnosis criteria.
Provide psychiatry, therapy, case management, nursing, and substance use counseling.
Duties and Responsibilities:
Coordinate a variety of services.
Provide support counseling, problem solving, contracts and limit setting.
Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management.
Make assessments and provide appropriate crisis interventions and risk management as needed.
Teach consumers adult daily living skills.
Maintain a clean, healthy, and safe environment at all times.
Initiate treatment and advocate for consumer within network of care.
Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Transport consumers in own vehicle or Center van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements within required time frames and abide by Center confidentiality policy.
Maintain cooperative relationship with Center staff, consumers, community agencies and the public.
Other duties as assigned.
Requirements:
Bachelors degree from four-year accredited college or university in Human Services preferred. Associate's degree required.
Experience in Human Services Field preferred.
Bilingual Spanish preferred.
Must have valid driver's license, registration, current inspection and insured automobile.

The OCD IOP is a highly specialized program for patients diagnosed with OCD, Panic Disorder, Social Anxiety Disorder or Specific Phobias, who are ready to participate in intensive exposure therapy as part of their treatment in the program. The progress patients make during their time with us has led this program to be highly sought after and respected. Patients are ages 18+ and of any gender. ERP is provided to patients through both group and individual sessions over a 6-8 week course of treatment.
Duties include initial psychiatric evaluations, group therapy and 1:1 therapy sessions alongside an incredible team of Psychiatrists and Psychologists. The program operates Monday through Thursday and the programtherapist hours are from 9 am to 5:30 pm, Monday through Thursday.

The Providence Center Housing Stabilization Case Manager provides housing case management support for the Providence Center s housing department. Provide Housing stabilization services to participants. Complete filing, documentation preparation and use/development of spreadsheets in Excel part of daily work. Data entry in EPIC and coordinate referrals for supportive services.
Duties and Responsibilities:
Perform a variety of duties to support consumers and to staff in the department.
Work in collaboration with supervisor on scheduling interactions with recipients to meet monthly target measures.
Collaborate with staff to open Housing Episodes in Epic to ensure participants have a medical record number in order to complete documentation of services provided.
Respond to incoming telephone calls from internal/outside agencies, insurance companies, staff and consumers. Refer to the appropriate staff member, service component or community resource.
Answer calls in a courteous manner; transfer calls appropriately; ensure messages are initialed and contain correct spelling of names, phone number and brief message regarding the call.

Work with clinical programs on identifying services needed for recipients.
Copy, scan and fax materials as needed. File charts, notes, etc.
Work with EOHHS on Prior Authorizations, correspondence, and required data for tracking.
Provide direct alcohol and drug counseling/case management to individuals with substance use disorders.
Maintain cooperative relationships with other employees, consumers and the public.
Treat consumers and peers with respect.
Work in cooperation with co-workers and supervisory staff.
Deal with co-workers regarding problems in a constructive manner.
Accept and use direction and supervision.
Attend trainings and conferences, participate on TPC committees.
Provide feedback to staff on the content of educational programs attended.
