If applicable, completes annual education as assigned to comply with DNV Hip and Knee Replacement Program requirements. ?Evaluates and implements intervention plans utilizing activities that are functional in nature and take into consideration the functional and emotional needs of the patient/family. Modifies approach in response to patient behavior and performance. Utilizes standardized outcomes measures in a uniform manner to ensure functional performance is accurately reflected. Completes documentation in accordance with regulatory and organizational standards. Collaborates with patient, family and significant others throughout the rehabilitation process and provides education/family training as necessary. Provides supervision to PTA and rehabilitation technicians in accordance with regulatory agencies. Clearly communicates patient performance, change in status, activity/transfer recommendations, and recommendations for discharge disposition and equipment required for the next level of care with the medical team. Maintains all requirements outlined in the Therapy Clinical ladder to maintain Senior status.

What you will do:
Provide high quality, innovative, tele-therapy to Headspace patients over a HIPAA compliant video conferencing platform
Complete, sign and lock clinical case notes in EMR within 48 hours of session as part of the per-session reimbursement rate.

What you will bring:
3+ years experience providing clinical psychotherapy to individuals, couples or families
An independent clinical license to practice in good standing (without need for clinical supervision), at the Master or Doctoral level

Ability to offer at least 10 sessions per week for bookings, with the ability to flex up or down as needed

Purpose:
The Transition Therapist is a member of the interdisciplinary team providing basic clinical services to persons with mental illness or physical disabilities targeted to transition from long term care to the community. The therapist will provide counseling and supportive services to assist class members that experience fear or anxiety around their move.
Essential Responsibilities, include but are not limited to:
Conduct assessments
Provide on-site, supportive counseling to CMs residing in nursing homes and SMRFS
Utilize motivational interviewing and other techniques to assist CMs make decisions and long-term goals
Assist CMs identify crisis triggers, and contingency plans in the event a crisis arises post discharge from the facility.
Maintain and update clients pertinent information within the MH electronic care system i.e. face sheets, contacts, consents, and diagnoses.
Achieve required productivity targets and complete documentation in a timely manner, including clinical documentation and service reporting.
In conjunction with the clients treatment team, keep clients family, guardian, and other Envision Unlimited staff informed of the clients progress, including any critical incidents, emergency room visits, hospitalizations, and medication changes/adjustments you become aware of.
Provide information to outside agencies as requested to coordinate and assure the appropriateness of services.
Provide and document crisis interventions as needed, within 24 hours of an incident, and complete an incident report when necessary.
Requirements:
MINIMUM QUALIFICATIONS: Masters degree in social work, counseling, psychology or related field with at least three years experience in social services. Must have excellent written and verbal communication skills, sufficient decision making, problem solving, computer and organizational skills. Individual must be able to work autonomously. 1. Clinician

- Leads and co-leads milieu activities per program.

General Summary of Position Responsibility:
Primary responsibilities include conducting psychological evaluations and providing psychological treatment services to children and adolescents being seen in a clinical setting within the Pritzker Department of Psychiatry & Behavioral Health. Services provided by the individual in this position will include evaluations and provision of evidenced based treatment modalities including individual, family, and group therapy and may include participation in specialty care clinics. Additional responsibilities include teaching and supervising trainees, including students in the APA-accredited psychology internship program and post-doctoral fellows. Engagement in scholarly activities, including research, program development and presentation of findings at national conferences is expected. The applicant will understand and integrate the growth and development needs of children and adolescents as they affect the delivery of patient care.
Essential Job Functions:
Conducts diagnostic evaluations, assigns appropriate DSM and ICD diagnoses, and provides treatment for patients in the Pritzker Department of Psychiatry & Behavioral Health at Lurie Children's Hospital. Treatment includes crisis intervention, individual, group, and family therapy as needed.
May provide psychological testing of children and adolescents, including administering and interpreting objective or symptom-based questionnaire measures as patient needs and clinical focus area indicates.
Serves as a team member, working collegially with others within their service area.
Maintains appropriate clinical documentation in the electronic medical record. Writes letters, notes, and formal clinical reports on patient contacts. Communicates progress over the telephone and via the electronic medical record, as appropriate.
Collaborates and consults with medical providers, schools, social service agencies as well as city, county, and state funding and placement agencies through oral and written presentation of clinical findings and recommendations.
