Responsibilities
* Assesses patient's status through interview, observation and physical assessment of actual problems and outcomes.
* Develops an individualized plan of care using a collaborative approach with the patient/family and health care team. Uses appropriate interventions identified in the plan of care to achieve expected patient outcomes. Modifies plan of care based on evaluation and as needed based on changes in patient condition.
* Systematically evaluates and documents patient's progress toward expected outcomes/goals on an ongoing basis throughout the shift.
* Completes required documentation as appropriate, including patient chart and event reports.
* Acquires and maintains current knowledge and skills in evidence-based nursing practice.
* Shares clinical knowledge through informal teaching and in response to the learning needs of others.

The CCOT RN performs ongoing surveillance of patients seen and intervenes as appropriate. This clinician is a resource for nursing staff in the recognition and management of decompensating and high acuity patients. A dynamic environment requires the CCOT RN to make independent decisions and judgments that support high quality care. The Critical Care Outreach RN manages requests through autonomous triage, negotiation and prioritization. The CCOT RN serves as a resource and assures care is provided in a compassionate, ethical, and knowledgeable fashion, is based on the Caring Model and exemplifies the mission, vision and values of Rush Copley and in accordance with policies and procedures. The CCOT RN assures the highest degree of quality care is followed at all times keeping the care and safety our patient population as a top priority. Emergency response education, outreach and quality data management in support for continuous process improvement for patient emergencies is within the scope of this position. Position Requirements:
