The Clinical Staff Pharmacist provides direct patient care related to medication prescribing, administration, and monitoring for patients of Carle Foundation Hospital. This individual also participates in decentralized care and focused drug studies, and will provide clinical expertise to assist the other staff pharmacists in completion of their duties.

Responsibilities
Assist in focused studies and drug usage evaluations as assigned. Assist in the development of clinical guidelines and order sets as related to appropriate drug therapy. Participate in decentralized care interacting with physician groups such as family practice, hospitalists, and critical care. Provide direct patient care by intervening as appropriate to improve prescribing, administration, and monitoring practices in the facility as related to medications. Represent the pharmacy on committees or work groups as assigned. Interpret physician medication orders and send the appropriate medication in a timely and accurate manner. Screen medication orders for potential interactions with other medications, diet orders, disease state, or stated allergies. Supervise the technicians in the preparation and dispensing of medications and parenterals. Make dose recommendations based on age, weight and clearance function. Assist in the training and orientation of all new employees as assigned. Provide accurate and complete accounting of all controlled substances. Provide Medication Therapy Management Services as defined in the Illinois Pharmacy Practice Act. Performs utilization management reviews. Serves as drug information resource for other departments. Conduct medication reconciliation, perform clinical interventions, uncover adverse drug reactions, and refer patients to pertinent clinics (i.e. anticoagulation).

The Clinical Pharmacist helps develop and implement Monogram's medication therapy management (MTM) program which will ensure appropriate use of medication management resources. The Clinical Pharmacist will perform timely written member medication reconciliations and drug therapy action plans for all Monogram members and communicate them to Monogram clinical staff and treating nephrologists, primary care physicians and other providers. He or she conduct telephonic consultations with medical providers when medication changes appear necessary and speak with members about medication use concerns. Performs quality assurance and performance improvement of the medication use system as appropriate
Collaborates with the healthcare team to manage formulary adherence and the medication-use process. Assists in the acquisition of non-formulary, compassionate use, and investigational medications.
Practices in compliance with organization's policies and procedures, as well as local, state, and federal regulations
Prepares, dispenses, and oversees the proper storage and handling of medications in accordance with best practice, policies and state and federal regulations
Serves as preceptor for pharmacy students, residents, and healthcare learners as assigned

Essential Duties
Medication Management: Review and interpret medication orders for accuracy, appropriateness, and potential drug interactions. Adjust and optimize dosages based on individual patient needs, including renal/hepatic function, age, and comorbidities. Clinical Pharmacist to provide comprehensive support on both the clinical floor and central pharmacy; ensuring seamless medication management and patient care.
Clinical Consultation: Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized patient care plans, offering expert advice on the selection, dosing, and monitoring of medications.
Patient Care: Participate in patient rounds, assess medication therapies, and provide recommendations to the healthcare team as needed. Conduct patient education sessions regarding medication use, side effects, and adherence as needed.
Therapeutic Drug Monitoring: Manage and monitor patients on complex therapies, including anticoagulation, antimicrobial stewardship, and parenteral nutrition. Adjust treatment plans based on laboratory results and clinical assessments.
Order Verification: Ensure timely and accurate verification of medication orders in the hospitals electronic health record system. Monitor for potential medication errors and implement corrective actions as needed.
Quality Improvement: Contribute to hospital-wide initiatives aimed at improving medication safety, reducing adverse drug events, and enhancing overall patient care.
Regulatory Compliance: Ensure adherence to all federal, state, and local regulations related to medication management. Maintain accurate and up-to-date documentation in accordance with hospital policies.
Non-essential Duties
Inventory Management: Assist with the oversight of medication inventory, including ordering, storage, and disposal of pharmaceuticals.
Education & Training: Provide education and training to pharmacy students, residents, and staff. Participate in continuing education programs and professional development opportunities.

Preferred Experience
Minimum of 2 years of experience in an acute care hospital setting preferred. Completion of a PGY-1 pharmacy residency in acute care specialty is highly desirable.
Required Licensure/Education
Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy
Current and active pharmacist license in the state of Illinois, in good standing.
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required or completed during first week of hire.
Preferred Education
Board Certification in Pharmacotherapy (BCPS) or a related specialty is preferred.
