Tell me about the role:
The Senior LVN is a supportive role to the department lead LVN and/or department leadership. This role is responsible for delivering direct patient care, coordinating patient care workflows, coordinating, and monitoring staffing to meet the needs in the operations of the practice. The senior LVN will work collaboratively with clinical staff, physicians, leadership and other fields of the medical team to deliver safe and efficient patient-centered care.
Job duties and responsibilities:
Provides orientation and training for new clinical staff.
Conducts departmental orientation and assures that onboarding measures have been met to meet the needs of the department and of the employee.
Maintains a functional work environment by maintaining quality assurance/quality control logs, regulatory needs within the clinical space, needed supply levels, and housekeeping throughout the work environment.
Administers prescribed medications, injections, or IV medications using standard process and as advised by provider.
Provides basic patient care and treatments such as taking vitals, performing diagnostic tests, phlebotomy, and specimen collection advised by the provider.
Observes patient, documents, and reports any changes in condition to the provider for instructions in taking necessary actions for care.
Answers patient and physician incoming messages with provider input.
Provides review of MA skills and performance abilities using designated evaluation tools.
Performs quality assurance and quality control measures to maintain compliance in the clinical environment.
Organizes and assures proper clinical team coverage needed to support providers and deliver safe patient care

About the Role

You will play a key part in medication oversight, assessments, staff supervision, and coordination of care.
Key Responsibilities
Perform admission and ongoing assessments of clients, including monitoring vital signs
Manage the medication monitoring process and medication counts daily
Accept and transcribe physician phone orders accurately
Supervise and assign duties to technician staff
Participate in treatment planning and client documentation audits
Conduct and document 30-minute patient rounds (frequency based on census/staffing)
Coordinate medical appointments and external referrals with primary physicians
Oversee medication refills, new orders, and discharge medication packages
Input medications into the KIPU system
Prepare and organize all incoming client files and urine analysis (UAs)
Provide daily Detox reports and performance improvement data
Maintain nursing and over-the-counter medication supplies
Complete any additional tasks as assigned by the Clinical Supervisor
Qualifications
Minimum 1 year of experience required; prior work in a residential healthcare setting is highly preferred
Must possess a valid California Drivers License and liability insurance if using a personal vehicle
Must be physically capable of performing job duties (vision, hearing, dexterity, mobility)
Must obtain First Aid and CPR certification within 90 days of employment and maintain current status
Medication training must be completed within 30 days (for Licensed Clinical Supervisors only)

Work as part of a multi-disciplinary team to provide comprehensive, integrated care to individuals experiencing street homelessness, mental illness and substance addiction
Complete medical assessments in the field
Coordinate PCP appointments or other medical follow ups appointments
Provide medical case management and supportive services to individuals assigned
Provide wound care to members as needed
Document and chart all services provided in appropriate charting form with accuracy and in real time
Provide medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens
Work in collaboration with the rest of the team, and assist with any medical and / or medication concerns
Work in collaboration with other agency and program staff, in order to coordinate services
Collaborate and communicate effectively with internal and external programs on behalf of clients to ensure continuity of care
Maintain medication logs and medication storage in compliance with safety standards and industry standards of care
Provide crisis intervention assistance when needed
Triage to appropriate community agency in an emergency (i.e., police, paramedics, fire department)
Maintain open communication with direct supervisor and leadership team
Cooperatively and cohesively work with all other providers (internal or external) involved in clients care
Participate in weekly staff meetings and weekly individual supervision
Ensure that medical services are reflective of the agency's treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation
Conduct Health/Wellness and medication management groups
Maintain confidentiality at all times per HIPAA and all applicable guidelines
Assist members with becoming involved in daily activities, socialization, health maintenance, sense of community
Qualifications:
Current, California license as a Licensed Vocational Nurse with a clear license.
Minimum one years' experience working with individuals experiencing chronic homelessness, mental health, and substance abuse issues
Experience completing DHS documentation
Able to physically lift, move, and position members as needed.
Experience completing medication logs to accepted industry standards.
Proficient in Microsoft Word, Outlook, Excel, and database applications.
Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record.
Able to obtain and maintain CPR/1st Aid certification.
Job Description Work Environment:
Office environment with occasional field activities
Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds.
May be exposed to elements like cold, heat, dust, noise and odor.
May need to bend, stoop, twist, and sit throughout the day.
Will necessitate working in busy and at times loud environments
