This role is pivotal in ensuring the safe and effective management of clients undergoing detoxification. You will play a key part in medication oversight, assessments, staff supervision, and coordination of care. Key Responsibilities Perform admission and ongoing assessments of clients, including monitoring vital signs Manage the medication monitoring process and medication counts daily Accept and transcribe physician phone orders accurately Supervise and assign duties to technician staff Participate in treatment planning and client documentation audits Conduct and document 30-minute patient rounds (frequency based on census/staffing) Coordinate medical appointments and external referrals with primary physicians Oversee medication refills, new orders, and discharge medication packages Input medications into the KIPU system Prepare and organize all incoming client files and urine analysis (UAs) Provide daily Detox reports and performance improvement data Maintain nursing and over-the-counter medication supplies Complete any additional tasks as assigned by the Clinical Supervisor Qualifications Minimum 1 year of experience required; prior work in a residential healthcare setting is highly preferred Must possess a valid California Drivers License and liability insurance if using a personal vehicle Must be physically capable of performing job duties (vision, hearing, dexterity, mobility) Must obtain First Aid and CPR certification within 90 days of employment and maintain current status Medication training must be completed within 30 days (for Licensed Clinical Supervisors only) Ability to develop basic computer skills for documentation within 90 days of employment Bilingual skills are a plus (please specify language) Preferred Licenses & Certifications Class C or B Drivers License Personal vehicle with liability insurance for mileage reimbursement (short distances) Join us in delivering compassionate, high-quality care in a dynamic clinical setting. Regular Hours: Monday to Friday, 8am to 5pm Job Description Providing nursing care to hospice patients in private homes, skilled nursing facilities, board and cares, and assisted living facilities under the supervision of an RN. Assist in obtaining the full assessment, following accurate orders, diagnosis and projection of visits for all disciplines necessary to support each patient. Perform and document nursing interventions including patient education according to the Plan of Care. Understands and is competent in workflows, problem charting, interventions and accuracy of input by review of POC at each visit. Demonstrates knowledge and competency in performing baseline nursing skills. Identifies and determines priority of the patient's needs/problems and reports any variance to the RN. Administers all medications and treatments. Communicates effectively with the RN, patients, families and physicians. Accurately document patient status, patient needs/problems, nursing interventions/patient response, safety interventions and education/teaching. Maintain patient rights and dignity. Assures the care of the dying patient, optimizing his/her comfort and dignity. Triage calls when needed. Attend death visits. Make scheduled, requested visits or Continuous Care (as needed). Performs other duties as assigned. Skills and Qualification Current California LVN License (required). At least one year of experience in hospice or home health (required). Current CPR certification, California driver's license and proof of auto insurance (required). Great communication and organizational skills. Phlebotomy and/or IV Certification (preferred). Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients. Able to lift supplies and equipment. In an inpatient setting, under the supervision of registered professional nurses, Licensed Vocational Nurses perform a wide range of patient care activities including general bedside nursing and patient hygiene, safety and comfort. They perform catheterizations, enemas, nasogastric tube insertions and irrigations; take and record vital signs and chart observations. They care for tracheotomy, jejunostomy, colostomy, and chest tubes.
