Job Summary

Job Duties & Responsibilities
Record the history of present illness, past medical, family, and social history
Perform physical examinations
Establish and follow plan of care in conjunction with MD
Prescribe appropriate medications to a specialty pharmacy that will handle prior authorization and other necessary paperwork; order refills as needed
Patient and family teaching
Order appropriate labs, tests, and procedures and follow up with patient as needed
Coordinate care and refer to other medical professionals as needed
Document clinic, telephone, and other patient encounters in the Epic system
Review lab results in Epic, communicate and interpret results
Collaborate with IBD Resource Nurse regarding IBD patient triage and return patient calls within 24-48 hours; provide feedback regarding scheduling of urgent patient appointments
Review outside records including clinic notes, hospital records, outside radiographs, lab, and other test results
Collaborate with nursing staff to complete patient paperwork including medication pre-certifications for insurance companies, durable medical equipment requests, disability paperwork, and handicapped permit applications
Attendance at GI Grand Rounds, IBD meetings, and IBD conferences is expected.Participation in other divisional educational events is also encouraged.
Job Eligibility Requirements
Education
Nurse Practitioners:Work requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.
Physician Assistants:Completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.

License

Job Summary
We are seeking a part-time (0.5 FTE) Advanced Practice Provider for our Duke Cardiology South Durham Clinic in Durham, NC to provide direct patient care services in collaboration with our general cardiology team. Patient population will include those with general cardiovascular disease/conditions, heart failure, and structural heart disease.
The APP will work in collaboration with physicians and advanced practice providers, collecting and documenting data, conducting diagnostic and therapeutic procedures, ordering and scheduling laboratory studies and professional consultations, and providing direct patient care services.
The position is part-time (20 hours per week). Primary hours will be Monday through Friday with hours flexible to meet the needs of the patients and the division.
Job Duties & Responsibilities
Take patient histories, conduct physical examinations, and record data in the patient's record; write progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care, and assume responsibility for developing therapeutic relationships with patients.
Order and schedule laboratory studies and diagnostic procedures; explain necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient
