STATEMENT OF THE JOB The Licensed Practical Nurse works as an integral member of the team providing services to senior residents who are aging in place or are clients in the Mental Health Clinics/PROS who are diagnosed with a behavioral health concern. The Licensed Practical Nurse is responsible for providing a range of wellness services to meet the behavioral health, medical, and care issues affecting those living independently within the Housing buildings and clients attending the Mental Health Clinics, Care Coordination or PROS programs. The Licensed Practical Nurse will be responsible for developing and implementing practices and protocols for clients ranging the entire life span. Residents in crisis or with behavioral health needs (such as hoarding, depression, anxiety, substance use disorders) will be reached out to, engaged and treated by the Licensed Practical Nurse. Additionally, the LPN will address and assess for medical concerns presented by clients in housing and the clinics/PROS. The professional will develop an individualized plan to address needs and include family, significant others and healthcare providers whenever possible. The primary goal of this employee is to support residents to maintain permanent affordable housing or, where appropriate, aide in transitioning to more appropriate housing. Within the Mental Health Clinics/PROS and Care Coordination, the primary goal is to assess medical concerns, perform health monitoring services, and provide linkages to treatment, in addition to providing health teaching and counseling around current and future health concerns. Assesses clients to establish a care plan according to their needs. Perform health monitoring services and assessments Responsible for tracking information/ deliverables for the Cabrini grant funds. Lead workshops for clients on issues related to health and wellness. Encourage clients to participate in on-going wellness and social activities. Provide advocacy and education to clients Facilitate coordinated care for targeted critical/high need cases. Works through team approach in partnership with program staff to identify and respond to clients in need. Works with client and their families to implement planning. Completes documentation, both paper and electronic in a timely manner. Provide counseling and therapy. Refer and link clients to service providers in the local community. Establish and maintain contacts with families and programs when clients are in hospitals and rehab facilities, and helps formulate aftercare plans. Responds to emergency situations and provide crisis intervention. Contact and follow-up with service providers put in place for clients such as Adult Protective Services, outpatient behavioral health, substance use treatment and healthcare providers. QUALIFICATIONS NYS Licensed Professional Nurse Must have certification in behavioral health clients or 3 years of experience working with the behavioral health population. Excellent time management, organizational and computer skills. Ability to appropriately evaluate and intervene in crisis situations. Excellent oral and written communication skills required. Bilingual preferred (Chinese, Korean, Russian, Spanish) Ability to work flexible hours (evenings/weekends) as assigned. Able to travel throughout the five boroughs as needed. Ability to lift up to 10lbs. May be required to meet with clients and / or staff throughout programs. Catholic Charities provides comprehensive care coordination and treatment services to individuals living with serious mental illness, complex medical needs and substance use needs. Our Queens ACT team provides an integrated set of evidence-based treatment, rehabilitation, case management, and support services delivered by a mobile, multidisciplinary mental health treatment team. ACT supports client recovery through a highly individualized approach that provides clients with the tools to obtain and maintain housing, employment, relationships and relief from symptoms and medication side effects. The LPN is part of this multi-disciplinary team and is responsible for delivering intensive in-home and community-based services to individuals living with severe mental illness, whom have a treatment history characterized by frequent use of psychiatric hospitalization and emergency rooms, involvement with the criminal justice system, alcohol/substance abuse, and lack of engagement in traditional outpatient services. RESPONSIBILITIES: The Licensed Practical Nurse (LPN) is responsible for working in conjunction with and in support of the RN in conducting health and medical screenings and interventions for clients being served by the ACT Team. This includes assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist, psychiatric nurse practitioner and RN; providing a range of treatment, rehabilitation, and support services. The role will involve delivery of nursing care services, community-based assessment and treatment of clients, and crisis management. The LPN will perform a wide variety of tasks related to the care, support, education, socialization, habilitation, recovery/healing, safety, security, and support of clients served. The LPN will provide interventions supportive to or restorative of life and well-being; and execute medical regimens prescribed by a physician, PNP or another licensed health care provider. Duties of the LPN include, but are not limited to: Support the RN in the development of individualized nursing screenings and interventions for clients. Conduct intake, physical health assessments, assessing the health needs of clients and make appropriate referrals to community based medical care. Develop/implement nursing care plans. During screening obtains information: a) to establish the degree of medical risk, medical care and interventions required to promote positive health outcomes, and b) to support the RN as s/he leads the team in primary care issues.
