Our Family Child Care Homes provide free Early Head Start and Head Start services to young children and their families. Early Head Start provides services to children six weeks to age 3. Families are respected as partners in the early care and education process with their children. The Family Child Care Providers' home are nurturing learning environments that promote their cognitive, social, emotional and physical development. Providers are caring and competent educators who guide the children's learning experiences by exposing them to a variety of developmentally appropriate Montessori activities.
Statement of the Job
The Early Head Start (EHS) Family Worker is responsible for the programs' parent involvement. The job responsibilities include but not limited to maintain necessary records, make referrals to appropriate agencies as needed, and work with families in a supportive fashion. The Family Worker in consultation with Family Service Coordinator and other staff serves as a liaison between the center and parents from the community the program serves. Under the supervision of the Family Service Coordinator, the Family Worker will be responsible to assist in recruitment, interviewing, and registration efforts in accordance with the Agency's policies and Head Start Regulations to determine eligibility of qualified individuals.
Conducts In-Takes and Family Partnership Agreements; shows parents around the center.
Elicits social and developmental information on the child, and shares this information with teachers and other staff keeping with the requirements of the family's confidentiality.
Responsible for inputting information in the data master, completing documentation, preparing required reports to document compliance with guidelines on in-takes, eligibility, priorities, classroom attendance and records; responsible for In-Kind Reports.
Receives and verifies financial and other information needed to determine eligibility as required.
Contacts the families to receive information, referrals, and other services such as employment mental health, counseling, housing, etc. as needed.
Performs other duties as assigned.
Qualifications
High School Diploma required.
2 years of experience in Human Services required.
College Degree or Family Development Credential preferred.
Bilingual Spanish/English; Russian/English; Chinese/English; Korean/English; English/Creole speaking preferred.
Must be able to occasionally lift and/or move up to 25 pounds.
Able to travel to multiple locations within the five boroughs as needed.

STATEMENT OF THE JOB:
Under the direction of the Clinical Director, the SocialWorker renders full range of clinical, rehabilitative, recovery, case management and support services to MI clients. The SocialWorker works in collaboration with the client and staff to ensure treatment adherence by monitoring medication, reminding clients of appointments, offering health care escorts, & making necessary referrals.
Conduct intake (including communicating/coordinating with government and community partners).
Ensure that the tenant population has access to service according to contractual requirements and the Agency's Mission.
Develop comprehensive wellness and service plans to meet needs of tenants
Provide consultation for case management staff and leadership for care coordination with on and off-site service partners; ensure all documentation meets agency and funding source requirements.
Follow-up on referrals made on behalf of the consumer/family and respond to all referrals in a timely and professional manner, recognizing the need for effective collaboration in order to ensure the highest levels of service provision.
Participate in cross-program service reviews, support and planning, and coordination of service delivery and service closure.
Identify need/interest and developing and facilitating groups as well as adult education workshops using rehabilitation recovery model.
Participate in training opportunities as assigned.
QUALIFICATIONS:
Masters Degree in social work or related field/MSW preferred.
Experience with homeless, mentally ill, MICA, geriatric populations.
Excellent oral and written communication skills.
Bi-lingual Spanish/English preferred.
Able to lift up to 10 pounds.
Able to climb stairs and make home visits.

FEC offerings are available to everyone, regardless of current or past child welfare involvement.
The goal of the Family Enrichment Center (FEC) is to enhance child and family well‑being by providing access and support to enable the strengthening of family protective factors inherent in its residents and community by building mutual community connections, capacity, and resilience.
In partnership with FEC staff, families will drive the planning, development, and sustainable co‑ownership of the centers. Families who participate in the FEC programming will be encouraged to share their expertise by volunteering at the center and given meaningful opportunities to engage with neighbors and peers. The goal of the FEC is to strengthen families' protective factors and build community connections, capacity, and resilience.
Position Summary
The Family Advocate is responsible for contributing to the day‑to‑day operation of the FEC including building connections with other staff, residents, and community partners; co‑designing and delivering relevant programming and activities (offerings); providing advocacy and making referrals for participants; and creating a warm and inviting gathering place for families and individuals using the protective factors model to support community resilience.
Responsibilities
Coordinate meetings, provide support, and collaborate with a team of local parents, subject experts, and other residents to deliver monthly events and activities (offerings) held at the FEC.
Foster a culture of respect built on the belief that families and communities possess the inherent strengths and knowledge needed to succeed given the right access to opportunities and support.

Qualifications
Minimum qualifications include a bachelor's degree and three (3) years of professional experience; or a high school diploma and five (5) years of professional experience in community-driven programs working directly with children, youth, and families.
Reside in or near the FEC community, or reside close enough to arrive promptly, in the event of an emergency.
Bi‑Lingual either /Mandarin &/or Cantonese is required.
Strong interpersonal skills and the ability to warmly connect with and engage children, youth, and adults individually and in groups.
Strong verbal and written communication skills.
Familiar with the community and its assets and resources, such as other programs, services, supports, local civic groups, and community leaders.
Experience in developing and running a group, a club, or coaching a team preferred.

The Program offers a safe and affirming living environment and supportive services to help our young adults to move towards long term stability. Position Summary: Founded in 1866, Brooklyn Community Services (BCS) empowers children and families to reach their full potential, strengthen families, and foster self-sufficiency in adults. With programs in 40 locations, BCS serves more than 20,000 individuals a year.
1921 Cortelyou is a 46 unit supportive housing program located in Flatbush Brooklyn. The Program specifically services young adults 18-25 years old with Serious and Persistent Mental Illness and have a chronic history of being unhoused. Most of the young adults living at 1921 Cortelyou are members of the LGBTQ+ community. The Program offers a safe and affirming living environment and supportive services to help our young adults to move towards long term stability. This Program is under the NY/NY 15 population C agreement and funding is being provided through the Department of Health and Mental Hygiene.
Position Summary:
The crisis intervention specialist will respond to situations where stress, depression, or any other mental health issue may create a personal crisis including but not limited to the following: suicidal thoughts, emotional trauma, serious mental illness, anxiety, housing crisis .models the best practices of hospitality and positive culture and work enthusiastically as part of a team.
Responsibilities:
Conduct routine behavioral risk assessments and safety plans
De-escalate high risk behaviors that may be creating an unsafe environment for a client or others around them.
Provide guidance to clients in dealing with housing-related issues
Recommend appropriate Person-Centered interventions strategies.
Provide routine follow-up.
Provide supportive counseling and motivational interviewing on a short term basis
Provide crisis management support as necessary.
Facilitate one-on-one sessions and groups.
Participate in clinical team meetings
Make referrals for clients and linkages
Complete psychosocial assessments for 2010e applications.
Create Wellness Self-Management Services Plans
Serve as an advocate and liaison for person in crisis
Complete documentation in a timely manner in Apricot database
Emergency On-call rotation with Program Director and Assistant Program
Follow-up on hospital admissions and discharges
Attend staff meetings and trainings as required by Assistant Program Director
Other tasks, as may be required
Qualifications Required:
Master's in counseling, psychology, public administration or mental health required, MSW/LMSW preferred
Experience working with individuals with mental illness and substance abuse, preferably with homeless adults and at-risk populations. Experience working with the LGBTQIA+ community strongly preferred
Strong communication and organizational skills; great attention to detail
Proven ability to work with a range of people from diverse backgrounds and circumstances
Maintain client and staff confidentiality
Comfortable providing services in BCS office and tenant's apartments
Computer literacy (Microsoft Word, Excel, Gmail, etc.)
Ability to work until 8 pm, twice a week and rotating weekends
Previous work experience in a social service agency or non-profit organization
Interest in working with underserved populations.
Familiarity with electronic data records systems
Bilingual in Spanish preferred

The Team Leader ensures an effective therapeutic milieu is established and ingrained in all aspects of the Suicide Prevention Program through the application of best practices in their work with youth and families. The Youth Peer Advocate program provides training to youth and families to develop skills and strategies to empower and support other young people and families by helping participants to obtain the YPA or PEP credential. The core required training for this is either the Youth Peer Advocate (YPA) training or Parent Empowerment Program (PEP) training, in addition to a wide range of other training.
POSITION SUMMARY: Peer Specialists use their “lived experience” to promote resiliency, recovery, wellness, and self-efficacy, and support the practice of youth-guided and family-driven approaches. The Peer Specialist position is community-based. Services are delivered in a variety of settings that meet the needs of the program participants.
RESPONSIBILITIES INCLUDE:
The Peer Advocate works with the Program Coordinator in collaboration with the NYC Youth/Family Peer Advocate Training programs to introduce and motivate young adults with mental health and/or lived experience towards professional development and purpose.
Establish structured, supportive relationships with youth and family peer advocate trainees.
Build trusting supportive relationship with Peer trainees, offering hope and motivation towards goal setting and professional development
Meet participants in the office, or in the community to educate around YPA or PEP training Programs, while promoting self-efficacy and personal development

Connect participants to community resources and services;

STATEMENT OF THE JOB The Agency has embraced the concept of a fully integrated system of care for all consumers of service. Provide case management services, assessment, and intake of participants to determine eligibility for services, including responding to telephone and walk-in inquires, conduct pre-screenings and provide brief and full services.
Diligent effort to target, engage, and outreach routinely to publicize the availability of the program to make persons at risk aware of Homebase and other available services and program including attending community meetings, community fairs/events.
Advocacy, linkage, and escort services for participants to prevent homelessness.
Provide and/or assure that each case assigned receives regular contact as needed.
Establish and maintain case records including completing progress notes. Ensure all relevant case related information is inputted into case files and corresponding databases.
Timely, accurate submission of financial assistance requests with the approval of supervisory staff.
Provide guidance to clients in dealing with housing-related issues including developing a list of reliable landlords/brokers for the purpose of finding affordable housing; outreach to property owners/landlord to mediate issues that help participants locate or retain housing; inspect housing dwellings to ascertain that they do not pose a threat to the health and safety of its occupants; accompany clients to apartment viewings.
Ensure that referrals are made to appropriate internal and external services as required to meet the consumer's/family's needs.
Supervise bachelor and master level interns.
QUALIFICATIONS:
Master's in social work from an accredited social work school required.
Experience supervising interns is a plus.
Bilingual skills (Spanish) a plus.
Ability to work flexible hours, including weekends, evenings according to the needs of program operation.
Able to conduct home/field visits and travel throughout the five boroughs as needed.
Frequently lifts and/or moves up to 10 pounds.

STATEMENT OF THE JOB The Agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that staff assess the client's needs holistically and see it as their responsibility to connect the client with services needed both internal and external to the Agency, regardless of the door through which the consumer enters the system. Under the direction of a designated Clinical Supervisor, the Licensed SocialWorker is responsible for direct service to clients in the Agency's outpatient mental health programs. All services are to be delivered in a professional and ethical manner, consistent with best practice tenets. The Licensed SocialWorker will function as part of a treatment team, maintains mandated levels of service, participates in Agency meetings as required and fully comply with Agency policies and procedures.The Licensed SocialWorker will perform initial interview, crisis intervention, and ongoing therapy with individuals, groups, and families. The Licensed SocialWorker will adhere to all standards of contract compliance, audit readiness, Agency policies and procedures, and all other regulatory requirements. The Licensed SocialWorker will work as a member of a team to help ensure the overall integrity and welfare of the Agency.* Meet Agency requirements for qualitative and quantitative contacts with clients. Responsibilities also include supervision of staff in the outpatient programs, assuring that program staff carries out their job responsibilities in a professional and ethical manner and maintain availability to staff for consultation during all program hours of operation.The Clinical Supervisor will be responsible for overseeing the delivery of care in order to ensure the appropriateness and effectiveness of outpatient services, identification of gaps in service delivery system, representing the Agency at meetings, conferences and public forums, strategic planning and implementation, development and implementation of processes and services that will improve the service delivery system, program development - including monitoring revenue, productivity, outcomes, staff training and development and staff supervision, compliance with contracts, agency policies and procedures, regulatory requirements and audit readiness and new initiatives.• Monitor and oversee all aspects related to the delivery of outpatient services - including program site visits on all staffing work shifts.• Plan, coordinate, identify and implement program outcomes and evaluation, strategic planning, accreditation efforts and program development.• Provide and/or oversee supervision of staff and graduate students.• The position requires the ability to provide coverage and supervision at the programs or in situations.• Facilitate program compliance with fire, safety, and health standards as per licensing and or quality assurance requirements and standards.• Perform initial interviews, crisis intervention, and on-going therapy with individuals, & children, run groups, work with families, & couples as dictated by the client's clinical needs.
QUALIFICATIONS• Master's Degree in Social Work from a nationally accredited institution. • Valid NYS LCSW required.• Clinical experience in a treatment program under Article 31 of the Mental Hygiene Law preferred.• Excellent clinical & supervisory skills along with three years of mental health &/or substance abuse experience.• Bilingual Spanish/English; Russian/English; Creole/English; Chinese/English; Korean/English speaking preferred but not required.• Able to work flexible hours and days - according to needs of program.
