Summary & Objective
The Nurse, Registered (RN) is responsible for managing the individualized patient care by promoting and restoring patients' health through the nursing process The Nurse, Registered will effectively interact with patients, family, significant others and health team members while maintaining all the standards required of a licensed Registered Nurse.
Essential Functions
Provides patient and/or resident care utilizing the nursing process and in compliance with facility nursing policies and procedures.
Conducts an individualized assessment and evaluates the total needs of the patient and/or resident, prioritizing the data collection based on the patient's immediate condition or needs within 24 hours of admission.
Continuously evaluates patient and/or resident status and adjusts plan based on patient/resident condition.
Develops plan of care that is individualized for the patient and/or resident reflecting collaboration with other members of the healthcare team.
Complete thorough documentation of patient and/or resident care, including nursing intervention, patient and/or residents response, needs, problems, capabilities, limitations and progress towards goals in a timely manner
Performs appropriate treatments as ordered by physician in an accurate and timely manner.
Ensure supplies, medications and equipment is properly stored.
Answers call lights within appropriate established timeframe.

Administers medications and treatments according to nursing policies and procedures.
Provide orientation on a regular basis to new employees.

Essential Functions
Oversees and coordinates patient and personnel assignments to ensure safe and effective care delivery for the home health patients.
Reviews electronically submitted documentation and provides feedback to clinicians regarding deficiencies or corrections to ensure the medical records reflect compliance with medical necessity, homebound status, and visit utilization and are supported by individual patient assessment/ documentation support and transition (discharge) planning.
Coordinates patient referrals.
Oversees and assists in assigning the personnel to patient care teams and cases.
Works with other departments in the development of personnel qualifications and development of the personnel policies.

The Registered Nurse is responsible for identifying and coordinating patient/family needs and for providing preventive and supportive care to the terminally ill patient/family unit. The Registered Nurse utilizes assessment, intervention and teaching skills in caring for the patient/family unit.
Essential Functions
Assess physical, psychological, social and spiritual needs of the patient and family within 24 hours of admission into the Inpatient Unit.
Completes accurate and appropriate documentation of patient/family services within 24 hours of visit and maintains accuracy of electronic medical record including accuracy of medication profile.
Coordinates care to the patient and family within the Interdisciplinary team and availability of all resources.
Collaborates with Nursing Home staff by initiating the written plan of care, maintaining professional management of hospice care and attending care plan meetings.
Actively participates in quality assurance and performance improvement projects as directed by Clinical Team.
Serves as case manager and advocate to the patient and family while collaborating with attending physicians, insurance case managers, skilled nursing facility staff, assisted living facility staff as well as all health-related practitioners.
Administers medications and treatments prescribed by the attending physician and/or hospice physician.
Attends and participates in Interdisciplinary Team Meetings as required.
Participates in hospice activities such as Memorial Services, Community Education, etc., as requested by the Team Manager.
Provide orientation on a regular basis to new employees.
Ensure effective communication with Interdisciplinary Teams.
Participate with on-call rotations.
Ability to interact with the public and other employees in a positive manner and promote a cooperative working environment.
