    Clinic Float RN The Center for Elders' Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, including an Adult Day Health Center, and promoting autonomy, quality of life and the ability of individuals to live in their communities. The Position: Under the direction of the Clinic Supervisor, the Clinic Registered Nurse provides nursing services to participants at the CEI clinic by using the nursing process, collaborating with providers and interdisciplinary team members. The Clinic Registered Nurse communicates effectively with all members of the clinic team and other program staff to ensure that participants are receiving care that is appropriate and adheres to the participants' plan of care. The rate range for the Clinic RN - Float position at Center For Elders Independence is $45.12 to $67.68 per hr. Hourly rate based on the market for the Clinic RN position, as well as experience, skills, abilities and work history **This is a fulltime, fully benefited position. The "Float" RN will not be anchored to one specific center, but will provide support to multiple centers according to operational needs. The Clinic RN Float will primarily support the Concord, East Oakland and Temescal Locations The Clinic RN, Float position is eligible for a 5K Sign On Bonus** General Duties and Responsibilities: Assesses the Participant's status in person initially, semi-annually and as needed; assists in the development and maintenance of the participant's plan of care. Monitors participant for any acute physical or behavioral changes and adjusts plan of care accordingly. Provides skilled nursing care to participants as needed in a timely manner. Provides health education, information and counseling to participant, family members and care givers regarding participant's care needs and safety. Implements preventative and rehabilitative nursing procedures as appropriate for the participant's care and Participates in IDT meetings and provides input to IDT, offering clinical expertise in team planning and problem solving. Documents in Participant's medical record and Department records accurately and in a timely manner according to agency standards. Provides direction to clinic staff as assigned when the Clinic Supervisor is not present. Offers nursing consultation to other staff members as appropriate. Assures quality of care by adhering to therapeutic standards, making or recommending necessary treatment adjustments; following standards of care set by Board of Registered Nursing, Nurse Practice Act, and governing agency regulations. Participates in CEI's quality assurance performance improvement program (QAPI) as assigned, measuring health outcomes against patient care goals and standards. Participates in departmental and agency meetings as directed by the Clinic Supervisor. Maintains the confidentiality of all company procedures, results and information about participants or families. Complies with all agency training requirements. Maintains safe working environment by following CEI's safety P&P's. Maintains a courteous, helpful and professional attitude on the job. Displays a willingness and ability to be responsive to all customer groups. Performs other assigned duties in a positive and helpful manner. Qualifications: Graduate of an accredited school of professional nursing. Possess a current California Registered Nurse License. Have a minimum of (1) year acute or ambulatory care experience, working with the frail or elderly population. Current CPR certification on file. Maintains eligibility for licensure by obtaining required continuing education units. This role ensures the delivery of safe, effective, and individualized care, following the participant's care plan, physician's orders, and regulatory standards. The RN communicates effectively with all members of the clinic team and other program staff to ensure that participants are receiving care that is appropriate and adheres to the participants' plan of care. The Home Health RN position is eligible for a $5,000 Signing Bonus. The pay range for the Home Health RN position at Center for Elders Independence is $45.12 to $67.68 per hour. Duties and Responsibilities: Assesses participants in clinic, or in participant's home, for post hospital, SNF, and fall assessments for medication, compliance and disease management or as indicated.     Description Center for Elders' Independence (CEI) provides a Home and Community-Based Alternatives (HCBA) Waiver Program that allows Medi-Cal beneficiaries with high-level care needs to continue to live at home or in the community by identifying and ensuring they receive services that will sustain their health and safety outside of an institution. The Position: Reporting to the Director of HCBA Waiver, the Intake RN is responsible for ensuring that individuals seeking enrollment in the HCBA Waiver Program can be safely and effectively cared for in a community setting and that they meet all eligibility requirements. The Intake Nurse conducts assessments and reviews medical data to complete the required DHCS documentation to support enrollment. This role requires expert clinical skills, sound judgment, and critical thinking to evaluate collected information accurately. All nursing duties must comply with the Nurse Practice Act, CMS PACE Regulations (42 CFR §460.180), the Center for Elders' Independence Code of Conduct, and established professional education standards. The pay range for the Intake RN position at Center for Elders Independence is $45.12 to $67.68 per hour. Hourly rate based on the market for the Intake RN position, as well as experience, skills, abilities and work history. Duties and Responsibilities: Follow program applicants through the intake process to enrollment. Through a combination of telehealth and home visitation, perform intake assessments of program applicants including completing the assessment form, creating a plan of treatment with the applicant, and visiting the residence to perform a home safety evaluation. Determine if the applicant meets Level of Care program requirements. Determine level of services needed to allow participants to live safely in their homes. Assess non-waiver service levels and connect clients to additional service agencies. Develop collaborative, productive relationships with healthcare facilities and partner agencies to achieve timely discharges that include optimal participant care and outcomes across the continuum. Assists with development, review and revision of departmental policies and procedures which support positive outcomes and organizational objectives. Offer input and comply with department standards of care and practice as well as quality improvement activities. Demonstrate openness to creative and innovative approaches to providing service. Comply with all DHCS reporting requirements. Completes assigned nursing training and competency validation requirements on time. Performs other duties as assigned. Qualifications: Graduate of accredited school of nursing. Current California Nursing RN license required. CPR certificate required. 3-5 years of Case Management experience preferred; Case Manager Certification a plus. Knowledge of Nurse Practice Act and applicable federal, state, and local regulations. Able to work independently in a dynamic, fast-paced environment. Excellent verbal and written communications including demonstrated strong interpersonal and presentation skills. Strong organizational and analytical/problem-solving skills in participant care environments. Bilingual: Spanish/English or Cantonese or Mandarin/English, preferred.
