The primary responsibility of this position is to administer medications to residents and assist with other aspects of their daily care and routine.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
Provide residents with medication administration and treatment assistance according to health care provider's orders
Monitor health, safety, and well-being of the residents. Promptly report any changes in resident's physical condition and/or behavior to Health Services Director
Accurately maintain resident records regarding medication distribution, leisure activities, incidents, and observations
Properly store medications and equipment and keeps the medication room clean and orderly
Ensures prescription label and storage procedures meet state standard
Properly follow shift-change procedures and documentation
Assist with activities of daily living such as bathing, dressing, and eating
Encourage and/or remind residents to participate in activities. Promote activities to enthusiastically to pique residents' interest
Assist in keeping the environment safe and happy for residents
Immediately respond to resident emergencies; provide first aid assistance; debrief and document incident following company procedures
Performs other duties as assigned by Supervisor
OTHER JOB DUTIES AND RESPONSIBILITIES
Perform all work assigned for the agreed-upon salary, accepting no additional payment or non-perishable gifts from residents, vendors, Community Management, or others
Meet the assigned work requirements of the job. Regular attendance is required

What You'll Do

Scheduled to work every other weekend as well as a holiday rotation.

The overall purpose of the Wellness Nurse position is to provide Person-Centered Care consistent with accepted standards and expectations of Sunshine Retirement Living and state regulations. What you will do:
The Ambulatory Nurse Clinician promotes optimal wellness by promoting health maintenance, participating in the management of acute illness, managing the effects of chronic disease and disability across the continuum of care. Responsible for dating all appropriate vials or bottles at time of initial opening and following pharmacy protocol on length of time they can be used.

Ensure safety of residents through appropriate behavioral interventions
Promotes employee relations, facilitates communication and problem solving
Ensures that community staff follow facility policies and procedures
Coaches, mentors, and counsels resident care staff as needed and keeps Health Services Director informed of any concerns
Ensures appropriate and timely response to customer service concerns
Demonstrates knowledge of resident care and its impact on residents and families
Assists resident care staff in performing duties as needed
Assists with immediate incident reporting and analysis of incidents and corrective action, when appropriate
Maintains confidentiality of community and resident information
Attend community meetings and in-service trainings as required
Know the emergency plan, fire evacuation routes, extinguishers, and other facility safety features
Cart and chart audits as scheduled
Clinical Evaluation Skills
Create monthly resident lists for Home Health, Hospice, and Oxygen
Ability to work with 911 and attend to emergency calls
Ability to work/run a medication cart
Report to work on time as scheduled and consistently demonstrate dependability and punctuality. Consistent and regular attendance is required
Accept assigned duties in a cooperative manner
Complete all other duties assigned by the Health Services Director and/or Executive Director
Maintains certifications as necessary such as First Aid, to fulfill state regulations (if applicable)
This position has supervisory responsibilities of the care staff

Meet the assigned work requirements of the job. Regular attendance is required

Job Description
Salary: $26 - $30 per hour, based on experience
Schedule: Part Time
The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support, provides assistance to community nurses, caregivers, and medication personnel to help ensure regulatory compliance and clinical excellence. The ability to work effectively as part of a team with flexibility and adaptability to change and pace of duties while maintaining a warm and encouraging disposition is important.
Assist in coordination of the resident admission process including screening, assessment, and care needs
Ensure care is delivered as indicated in resident care plans
Medication and treatment administration and ensure medications are administered per physicians' orders
Assistance with coordinating care and services with other outside providers including resident physicians
Document and report care provided and resident observations including unusual or significant changes
Maintain a working knowledge of state regulations, policies, and procedures
Qualifications
To be successful in this position, we believe that you need the following experiences, strengths, and skills:
Working knowledge of state regulations, policies, and procedures, dictated for compliance
Graduate from an accredited school of practicalnursing
Current nursinglicense and state registration (LPN or RN)
Three years' experience in assessment and geriatric nursing, preferred
