The Per Diem RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process, coordinating the Plan of Care with the Interdisciplinary Group (IDG), and providing palliative and supportive care to the patient/family unit.
Qualifications:
Current license as RN in the state where the employee will be working
Active BLS for healthcare professionals from the American Heart Association or Red Cross.
Minimum one (1) year nursing experience; hospice or hospital experience preferred
Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system
Mobile Driver - Valid driver's license and automobile insurance per Company policy
Reliable transportation to meet visit schedule
Ability to use equipment with visual and auditory mechanisms
Ability to effectively communicate in English (verbal and written)
Flexible schedule to meet the needs of Hospice House patients
Ability to manage the emotional stress of working with and caring for terminally ill patients and their families
Ability to physically access patient rooms, other areas of the Hospice House/Hospital/ALF, etc., and any home (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning) within any part of the service area

Completes clinical admissions for all Company services including, but not limited to:

Acts as the Company representative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families, and the general public.
Communicates frequently with Admissions to ensure timely follow-up. When assignment is complete, informs Admissions and prepares for next assignment.
Provides all necessary clinical communication timely using SBAR.
Discusses any potential needs with after-hours staff.
Obtains consents for services and arranges safe discharge in an efficient manner for referred and eligible patients while meeting productivity expectations.
Obtains medical record information for the referral.
Develops strong relationships with case managers, physicians, etc. at facilities.
Works with Professional Relations Representatives (PRRs) to provide education on all available services.
Provides care for any GIP patients in a hospital including daily visits, professional management, documentation and efficient discharge.

WHERE YOU'LL WORK
Each day is different. You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility. Most VITAS nurses make 4-6 visits each day.
Once a week, you'll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and nurses, who collaborate to provide holistic support for the patients and families we serve.

2. RegisteredNurse, RN, reports any request to practice outside of scope of license to the manager/supervisor
3. RegisteredNurse, RN, performs supervision of (VITAS employed and contracted) nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses (and contracted RegisteredNurses) and Volunteers per the plan of care
4. RegisteredNurse, RN, completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement in order to provide palliation of end-of-life symptoms
5. RegisteredNurse, RN, appropriately identifies/utilizes resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults
6. RegisteredNurse, RN, completes documentation at the time of the event and at the location of care unless otherwise directed
7. RegisteredNurse, RN, provides education and training to patients, families, caregivers and referral sources in the care of the patient, including pain management
8. RegisteredNurse, RN, participates in patient care meetings as assigned
9. RegisteredNurse, RN, participates in weekly team meetings
10. RegisteredNurse, RN, provides effective communication/shift reports through written, verbal, reporting and body language
11. RegisteredNurse, RN, participates in bereavement support
12. RegisteredNurse, RN, participates in the on-call rotation and/or provides shifts during non-business hours such as evenings, weekends and holidays based upon program-identified needs

a) Connecticut: For activities other than those listed in section 19-13D69(d)(3) of the Connecticut Department of Public Health delegated to a Hospice/Homemaker, the RN will document in the clinical record that:
i. All factors pertinent to the patient's safety, including the competence of the Hospice Aide/Homemaker have been assessed
ii. Determination that this activity can be delegated safely to a Hospice Aide/Homemaker
19. RegisteredNurse, RN, provides nursing care including nursing tasks that may be delegated to other roles such as LPNs, HAs and Volunteers
20. RegisteredNurse, RN, participates in and monitors patient and family care compliance activities
21. RegisteredNurse, RN, professionally manages/coordinates the hospice plan of care

Each day is different. You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility. Most VITAS nurses make 4-6 visits each day.
Once a week, you'll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and nurses, who collaborate to provide holistic support for the patients and families we serve.
