The Integrated Community Health Worker will serve as a frontline public health worker. The Integrated Community Health Worker serves as a liaison between health and social services and the community. As such, the CHW will facilitate access to services and improve the quality and cultural competence of service delivery. If you have a calming presence, excel at understanding patient needs, and are willing to work on educating patients about the importance of health screenings, we would like to have you as part of our team. The CHW will work as an on-floor team member at our health center and will interact with patients and staff who are racially, ethnically, and culturally diverse, so cultural competence is required.
YOUR RESPONSIBILITIES
Establish trusting relationships with patients and their families while providing general support and encouragement.
Provide ongoing follow up, basic motivational interviewing and goal setting with patients/families.
Serve as a liaison between clinical staff and patient, by following up with patients regarding their appointments, referrals, and needed resources.
Help patients set personal goals and attend appointments.
Provide referrals for services to community agencies as appropriate.
Help patients connect with transportation resources.
Help patients connect to needed resources to carry out plan of care (Meals on Wheels, home health, Prescription assistance, etc.).
Work closely with integrated medical team to ensure that patients have comprehensive and coordinated care.
Remain knowledgeable about community resources appropriate to needs of patient/family.
Continuously validate and evaluate the effectiveness of the community resources.
Provide consistent communication to Medical Provider/Nurse to evaluate patient/family status and report progress and compliance with identified plan of care.
Enable and improve culturally and linguistically competent services and care by making efforts to understand the community culture and resources.
Conduct outreach and implements programs in community that promote, maintain and improve community health needs and provides resources.
Record timely and accurate documentation of patient engagements.
Collect data to help identify community health needs.
Advise community groups on chronic disease and diabetes self- management, hypertension, and nutrition.
Advise clients on general health and exercise and self-care activities.
Identify age-appropriate needs and resources.
Follow up on hospitalizations and no shows.
Provide situationally appropriate social support to the patient utilizing experience lived and learned through training provided at CRCH.
Provide informal counseling as needed.
YOUR QUALIFICATIONS, COMPETENCIES, TRAITS
Bachelor's Degree related field required.
Bilingual speaking preferred in Portuguese or Spanish, but not required.
BLS OR CPR Certification required, must be completed prior to start date, and must be hybrid or practical certificate including adult, pediatric and AED trainings.
Community Health Worker Mass Board certification preferred, not required. Must obtain within 3-4 years of hire date.
Preferred Medical Interpreter Certification.
Must have knowledge of computer systems such as Word and Excel.
Strong interpersonal, organizational, and time management skills required.
Must have a willingness to work flexible hours to meet the organization's needs/demands.
Must have excellent communication skills, particularly with people from diverse cultures, with the ability to understand the community, population, and patients we serve.
Must believe in the work we do at CRCH, with a strong passion to serve underserved populations in diverse settings.
Knowledge of both community and healthcare system resources.
Experience serving the population(s) that will be referred for patient navigation services.
Excellent written and oral communication skills.

**One Time Sign on Bonus of $7,500.00** Schedule: 6am-2:30pm The SPD/OR Liaison will be responsible for planning, organizing, and overseeing daily clinical and administrative operations for their designated specialty surgical service. They may assist in developing, recommending, and implementing internal standards, policies, and procedures to improve quality and cost-effectiveness of patient care and sterile processing. Liaisons will participate in hospital, departmental and multidisciplinary programs, committees, and special projects.
Key Responsibilities:
Supports Operating Room (OR) staff by verifying instrument kits are ready before surgery start times.
Attends daily meetings with OR Specialty Coordinators to forecast instrument needs up to five days in advance.
Monitors daily surgical case flow and communicates emergent or additional instrument needs to SPD, resolving issues as needed.
Conducts daily audits of Key Performance Indicators (KPIs), including case cart and kit audits, room visits, and post-case reviews.
Analyzes KPI data to identify trends and operational issues in the OR and Sterile Processing Department (SPD).
Coordinates communication and planning for priority instruments and turnaround times, informing the surgical team of any delays.
Responds promptly to surgical team calls regarding instrumentation problems or urgent needs.
Cleans and processes equipment per manufacturer's instructions and safety protocols.
Ensures instrumentation is processed and available for next-day, on-call, or weekend surgeries.
Assists with documentation, kit updates, pick-list revisions, and maintaining accurate instrument count sheets with coordinators and physicians.

The Medicaid ACO and iCMP resource navigation teams have recently merged to build on best practices to advance health equity, improve health outcomes, and promote well-being of our MGH Primary care patients by addressing health-related social needs, system navigation, and care coordination as standard of care. The role collaborates to enhance the delivery of patient care services along the continuum of care bridging communication about patient social needs and resources available to them back to the clinical team. This position involves problem solving to ensure each unique patient is evaluated for their unique abilities and capacity in addition to their identified SDH needs.

Research Community Health Worker (CHW): The Disparities Research Unit at Massachusetts General Hospital is seeking Bilingual Spanish Research Community Health Workers to be trained to offer interventions through clinical sites in socially vulnerable communities. Interventions will focus on mental health, obesity/weight management, and cancer screening and are part of a large, multisite grant. CHWs will receive formal training in their intervention of focus. They will conduct group and/or individual sessions with enrolled patients, maintain post-session notes, and conduct careful tracking of intervention sessions. This is a part-time position for someone interested in helping patients with complex psychiatric, medical and social problems improve their access and utilization of psychiatric/health care services.
A Community Health Worker (CHW) is a trusted member of the community who helps patients better access and coordinate their psychiatric care. We believe that CHWs have the skills and experience to understand what patients are going through and help them address the social and medical problems that lead to poor health. The goal of a community health worker is to assist the most high-risk patients with the tasks of getting psychiatric/medical care, working on health goals (such as arranging care, filling medication prescriptions, planning healthy meals, or finding time to exercise), and to help them deal with the "real-life" issues that keep them from staying healthy. The CHW will work with patients receiving care at Brigham and Women's Hospital outpatient psychiatric clinic. CHWs are integrated into the outpatient psychiatric team, serving as a bridge between the team and patients in the community. As a CHW in our department, you will develop trusting working relationships with your patients and be supported by a psychiatric team that includes social workers, nurses and psychiatrists.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
* Provide community health work services for patients identified as high-risk due to psychiatric or psychosocial challenges.
* Attend initial and continuing education training programs including self-directed reading and in-person and online learning.
* Work with patient and interdisciplinary team to set goals for patient's care.
* Meet patients in the emergency department, primary care clinic or hospital to reinforce and advance patient goals.
* Coordinate resources for identified problems including community mental health services, homelessness, substance abuse and food insecurity.

This team can help to address all aspects of a community member's care.

* Help patients develop their plans for getting to various appointments for screening and diagnostic tests, and treatment services.

GLFHC is currently seeking a Community Health Worker to join our REACH Program. This Full Time position This full time position is part of a larger collaboration (REACH) with the City of Lawrence, focused on Healthy Lifestyles. This Community Health Worker (CHW) position will have equal time dedicated to the GLFHC specific aspects of REACH, with half-time focused on our breastfeeding promotion programs and half-time focused on our work developing and implementing programs to increase patient and community access to healthy foods.
Breastfeeding Initiatives Support (20 hours per week)
* Collaborate in planning and implementation of GLFHC breastfeeding programming.
* Provide education to community members, providers, and others on breastfeeding support techniques, challenges and solutions.
* Collaborate with others healthcare professionals to ensure holistic care.
* Coordinate interagency referrals between LGH, WIC, and GLFHC to ensure continuity of care.
* Attend Lawrence Breastfeeding Coalition meetings.
Produce Prescription/Fruit Vegetable Program Coordinator (20 Hours per week):
* Collaborate with REACH Nutrition Lead/Program staff to develop, implement, and promote produce prescription (PP) and fruit/vegetable incentive programs (FVI).
* Collaborate with REACH communication strategies team regarding ongoing communication and outreach to eligible programs participants.
* Represent GLFHC team in regular meetings of Mayor's Health Task Force, REACH participants and Healthy Food Incentives working group.
* Develop and disseminate patient/community education materials. Participate in ongoing efforts to educate patients, community members, and health center staff.
* Collaborate with GLFHC staff, Unite Us Team and community partners to identify, establish and expand and linkages with community-based organizations, private businesses, and governmental agencies to help in meeting PP/FVI goals.
Qualifications
Education and Preferred Skills:
* Associates Degree in Health Services/Human Services, or High School degree with 2 years' equivalent experience.
* Bilingual English /Spanish language skills are preferred; additional languages may be helpful

The Community Health Worker (CHW) Manager plays a key leadership role in guiding, mentoring, and overseeing the work of The Community Health Workers within the Integrated Care Team (ICT). The manager works in collaboration with local practice leaders and other stakeholders to optimize patient engagement strategies. The Manager ensures high-quality care coordination, patient engagement, and transitions of care support services provided by CHWs. The Manager is responsible for training, case review, supervision, performance evaluation, and ensuring CHW activities align with organizational and programmatic goals under the C 3 ACO contract, including ensuring compliance with requirements align with CHW practice.

Qualifications
* 3- 5 years' of Community Health worker experience working with vulnerable populations that have social needs.
* Certification as a Community Health Worker required. Actively participating in a Community Health Worker program acceptable.
* Minimum 2 years' experience in leadership or manager role overseeing CHW practice.
* Strong interpersonal, communication and coaching skills, including building CHW competency.
* Proficient in EHR documentation
* Bilingual/ bicultural (English/Spanish) strongly preferred.
Education
* Associate Degree in health care related field required, Bachelor's degree preferred.
